About this file
This is a Questions and Answers document addressing contractor inquiries regarding the removal and de-installation of two GE INNOVA 4100-IQ imaging systems from the George E Wahlen Veterans Affairs Medical Center in Salt Lake City, Utah. The document clarifies critical project parameters, including that the contractor must furnish, install, maintain, and remove a complete Level IV ICRA (Infection Control Risk Assessment) containment system for the work areas. The equipment is located on the same floor as a loading dock with main hallway access; a secondary dock with elevators is available for lower floors. Work should be scheduled early or late in the day to minimize foot traffic disruption.
The Q&A confirms that the VA will provide electrical isolation and place control locks on utility disconnects, with the contractor responsible for LOTO (Lockout/Tagout) devices if equipment remains energized downstream. Floor anchors must be removed and the floor restored to a flush condition without finished flooring replacement. The contractor assumes ownership of the removed equipment for recycling, scrapping, or disposition. Both rooms are available simultaneously, allowing concurrent removal of both systems during a single mobilization. Work should commence as soon as possible following contract award, with an online planning meeting via Microsoft Teams to be conducted post-award to discuss the ICRA containment approval process and project timeline. The two GE INNOVA 4100-IQ units have serial numbers 589832BU5 and 436339BU6, with equipment identifiers 57204 and 40402 respectively.

GE Innova Systems Removal – Questions and Answers
Questions:
1. ICRA Level IV Containment: The SOW requires Level IV containment. Is any existing containment available at the facility for these rooms, or should the contractor plan to furnish, install, maintain, and remove the complete Level IV containment system?
1. Equipment Removal Path: Can you provide any information regarding the anticipated removal path from the two INNOVA rooms to the loading area? In particular, are there freight elevators, loading-dock access, unusual doorway restrictions, stairs, or other conditions that should be considered when planning removal?
1. Floor Restoration: The SOW requires removal of floor anchors and restoration to a flush condition. Can you confirm that the requirement is to remove/cut the anchors and fill/patch the resulting penetrations flush with the existing floor, rather than replacement of the finished flooring?
1. Electrical/Utility Isolation: Will VA Facilities provide electrical isolation/disconnection of the systems from building power and other facility utilities prior to deinstallation, or is the contractor expected to perform these building-level electrical/utility disconnects?
1. Equipment Disposition: Following removal from the facility, does ownership/title of the removed equipment transfer to the contractor for recycling/scrapping/disposition, provided that all applicable environmental requirements and required disposal documentation are satisfied?
1. Project Schedule: Is there currently an anticipated date or date range when the VA would like the two INNOVA systems removed?
1. Room Availability: Will both rooms be available consecutively so the contractor can complete both systems during a single mobilization, or should we anticipate separate removal dates?
1. You mentioned that after award there will be a planning meeting to discuss the ICRA Level IV containment approval process and general project timeline. Will the contractor's project representative be required to attend that meeting in person at the Salt Lake City VAMC, or can the initial planning meeting be conducted remotely by Teams/telephone, with our local ICRA containment contractor coordinating any necessary on-site assessment?
Answers:
1. Contractor should plan to furnish, install, maintain and remove the complete Level IV containment.
1. Equipment is located on the same floor as a loading dock that the contractor will have access to. There is also a secondary doc with elevators that can be used for the floor below. It will be main hallways that have to be utilized, so moving things to or from the space will be easiest early or late in the day to avoid foot traffic.
1. Confirming that the SOW is to return the floor to “flush” condition. There is no expectation to replace the finished flooring.
1. VA will identify and place a control lock on the utility disconnect. Vendor shall have LOTO device if the equipment still has potential to be energized downstream of the work.
1. The ownership of the removed equipment transfers to the contractor for recycling/scrapping/disposition.
1. We are looking to get the equipment removed as soon as possible once contract is awarded. Once awarded, a planning meeting will occur to discuss ICRA containment approval process and general timeline to ensure proper personnel are available to assist the vendor.
1. Both rooms are currently available and will remain available so all of the work can be done at the same time. The rooms are adjacent to each other, so it will be easy to move from one space to the other.
1. The planning meeting can be online by Teams. The intention is to ensure all parties are aware of who/what/when/where of all steps in order to avoid needing multiple visits.
Questions:
Do you have any photographs of the units? I also need the dates of manufacture and serial numbers, if available.
