STATEMENT OF WORK

SECTION 1 - GENERAL: DAMPER REPAIRS

1.1 This statement of work is for performance of repair services for correction of smoke and fire damper deficiencies as identified in reports from the February 2026 inspections conducted by Remedi8. This request is also meant to allow for a contract modification for the correction of deficiencies found during the course of work associated with this contract. Many of our dampers were not accessible for testing during the February 2026 inspection. Therefore, once access is gained, it’s our intent that all subsequent deficiencies need to be repaired with this contract work leaving Sheridan VAMC with no existing deficiencies once this period of performance is complete. Requested services are in support of the Sheridan VA Medical Center. The contractor shall provide appropriate personnel for these duties at the Sheridan VA Medical Center, 1898 Fort Road, Sheridan, WY.

a. Hours of work are defined as Monday through Friday from 7:30 a.m. to 4:00 p.m., excluding Federal holidays or as otherwise arranged with the Contracting Officer Representative (COR).

b. The eleven holidays observed by the Federal Government are New Year’s Day, Martin Luther King Day, President’s Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving Day, and Christmas Day, and any other day declared by the President of the United States to be a national holiday.

1.2 Background: The Sheridan VA Healthcare System (SVAHCS) conducted inspections of fire, smoke and combination dampers during the week of 16 February 2026. The inspections resulted in five (5) buildings with a combined total of twenty-four (24) dampers which were non-compliant. The buildings that pertain to this SOW and require repairs are Buildings 4,6,9,64 and 71.

1.1

1.3 Objective #1: Perform damper repairs, modification and re-inspection of deficient dampers already identified to accomplish compliance with National Fire Protection Association (NFPA) and The Joint Commission (TJC) standards.

1.4 Objective #2: Provide repair estimates for any newly identified deficiencies which may not have been discovered during the February 2026 inspections for various reasons.

1.4.1 This could be for inspections of previously obstructed dampers which when access is gained may reveal undiscovered deficiencies which need correction.

1.4.2 This also covers any damper deficiency discovered within the buildings pertaining to this SOW. Any modification to an Heating, Ventilation and Air-Conditioning (HVAC) system can reveal previously undiscovered problems or create new ones within the system being tested. Repair estimates should be provided for these areas as well.

1.5 This contract period of performance should be for a term of 240-days from contract award for the completion of ALL repairs to include already identified deficiencies, as well as any newly discovered deficiencies discovered during initial repairs.

1.6 The POP identified above is to allow for the extreme possibility of a contract modification need due to other deficiencies being found in untested areas, as well as the delays still being experienced which stem from the covid-19 pandemic.

SECTION 2 – REQUESTED SERVICES:

2.1 Contractor shall provide services, equipment, staff, and tools necessary to repair, inspect and test smoke and fire dampers in the below identified buildings at the Sheridan VA Medical Center, 1898 Fort Road, Sheridan, WY 82801. All services shall be in accordance with TJC and NFPA 80, 90A, 101 and 105, and any other applicable NFPA standards. Contractor will include documentation of all repaired devices and must identify as a minimum which includes date repaired, inspected, unique device identification, device location, status following repair (pass or fail), the type and quantity of devices repaired/retested in each building. This is a base year, one-time contract with no option years. The tenets below were taken from the inspection and deficiency reports. Numbers in parenthesis are number of reported deficiencies for each building requiring correction. Buildings included are: 4 (10), 6 (5), 9 (3), 64 (3) and 71 (3).

2.2 Repair and re-test deficient dampers located in each of the identified buildings

2.3 Remove and reset fusible links on Dampers, to verify dampers full closure and if necessary, replace any fusible link that is compromised (Per NFPA code).

2.4 Clear each damper of any debris that would impede damper operation.

2.5 Coordinate with safety Office and local fire department if needed to place the fire alarm system in test mode for any testing of devices.

2.6 Manually activate actuators on electric and pneumatic smoke and/or combination dampers to verify proper operation of the damper.

2.7 If there’s no existing access to an identified damper, installation of a damper access hatch/door will be required, damper tested and a quote for any found deficiency will be provided so a contract modification can be accomplished.

2.8 If decommission of a damper on the deficiency report is identified, decommission the damper and mark the damper location and repair report as decommissioned.

2.9 Place new Repairs labels on all dampers to confirm each item is inspected and identified with a pass or fail indication

2.10 Follow guidance as directed in section 1 and supply COR with estimates for all discovered subsequent repairs so a contract modification can be completed for repair of ALL deficiencies.

SECTION 3 - SPECIAL CONDITIONS:

3.1 Any assigned Contractor personnel must be able to pass and maintain a background clearance investigation for obtaining a Government-issued identification badge.

3.2 Provide and maintain documentation, reports, and records of Repairs and testing in hard copy and electronic format to better assist the customer with record keeping (doc.x and xls).
