Description of Services:
Licensing for 125 beds to provide access to IPC clinical consulting services accessed via the internet. A digital care education management solution that provides integration with our Clinical Patient Record System (CPRS) to offer personalized education to patients. When patients are assigned education, the education drops into the patient's user interface to complete. Clinical rounding is performed every morning and ensures the patient completes the education. Once the education is completed, a completion note automatically adds an education completion note in the patient's electronic health record. The clinical consulting feature also allows on demand searching for education, has a medication formulary where patients see their real time inpatient medications, and allows for patient feedback on their care experience.
Additionally, when patients review their medications, there is an acknowledgement button that, when pressed by the patient, auto writes a note to the patient's electronic health record that the patient has reviewed their medication. These allow for unique integrations include Admission Discharge Transmission (ADT) data, Health Level Seven (HL7) Scheduling Information Unsolicited (SIU) data, Batch File transfers, and Security Assertion Markup Language (SAML) integration enabling Single Sign On. Through Electronic Management Record (EMR) integration, a clinician dashboard is at the fingertips of the caregiver, and through automation, we are able to deliver educational content, check-ins, two- or one-way communication, and surveys (experience, health status, etc.), enabling top-of-license care while digitally triaging in/outpatients to the right caregiver at the right time across multiple specialties. By sending the right information at the right time, the digital care management software identifies patients in real-time who need help. This allows VA care teams to reach more patients and proactively intervene before costs and complications escalate.
