ChemDAQ Sensor Exchange Program (SXP®) and Warranties
INTENT TO SOLE SOURCE
ChemDAQ Sensor Exchange Program (SXP®) and Warranties
The Department of Veterans Affairs (VA), NCO 19, intends to award a sole source firm fixed price contract to ChemDAQ for Steri-Trac 2 Sensor Exchange program (SXP ®) for the VA Fort Harrison and VA Billings Montana Locations. The SXP provides for freshly calibrated sensors shipped quarterly to provide a continuous safe environment for personnel. ChemDaq tracks the time the sensors were last calibrated and provides freshly calibrated sensors as needed. The contractor will also provide at no additional cost unlimited technical support, repairs, or replacement of all hardware, software updates as they become available, loaner units to minimize downtime, overnight shipping for loaner units and any new or replacement part(s) as required for system operation.
Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements per 41 USC §3304(a)(1) as implemented by FAR 6.302-1.
The applicable NAICS Code is 334519 and the size standard is 600 employees. This notice of intent is not a request for competitive quotes. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses received to this notice is solely within the discretion of the Government.
Capability Statement Please *answer the following questions when responding:
*Failure to provide the information requested (items 1-11) will be considered non-responsive and will not be considered a viable source.
1. What is your company’s Socioeconomic Status? (ex: small business, large business, SDVOSB, etc.)
2. Provide DUNS and/or CAGE Code.
3. Ability and experience in managing this same requirement. Include general information and technical background describing your firm's experience on contracts requiring similar efforts to meet the program objectives.
4. List of Projects (Government and Commercial) that are similar in scope and size.
5. Anticipated Teaming Arrangements (if any). If plan on subcontracting, demonstrate that you will meet the limitations on subcontracting per FAR 52.219-14 for Small Businesses; VAAR 852.219-74 for SDVOSB/VOSB.
6. GSA Contract Number, if applicable.
7. Letter from ChemDAQ authorizing your company to perform all services included in the ChemDAQ for Steri-Trac 2 Sensor Exchange program (SXP ®).
8. Does your company have the capacity to provide all the required commodities and services for the ChemDAQ for Steri-Trac 2 Sensor Exchange program (SXP ®)?
9. Is your company primarily engaged in the retail or wholesale trade and normally sells the type of item being supplied?
10. Does your company take ownership or possession of the item(s) with its personnel, equipment, or facilities in a manner consistent with industry practice?
11. What is the manufacturing country of origin of the items and/or products being offered?
