Notes/Comments [10/23/2015 U.S. Dept of Veterans Affairs - Scott Kelly]

1. Regarding the Section R 'Upgrade Option", KT is required to provide booms that meet the contract requirements, including having the capacity to route the number of utilities specified by the final design deferred area design. No utilities were revised in the design deferred spaces except JSI 118.3 rooms since submittal 2655 71 -01 R3 was returned to KT on 12/8/14. Based on this, it would appear that KT's previous submittals including 26 55 71-01 R3 were non-compliant because the submitted Amico 90 KG equipment booms were incapable of supporting the current design utilities at the time of submittal based

Received 10/26/2015

RETURN OF SUBMITTAL

VA File No: 265571-001-R4 on the Section R information. Additionally, it would appear that this submittal is non-compliant because the equipment boom drawings in both the previously submitted room sections and the additional sections for JSI 118.3 rooms do not depict the necessary Amico 200 KG equipment booms per the information provided. The previous history of these submittals with respect to equipment boom non-compliance is also referenced, previous submittal reviews have identified the lack of any specified smoke evacuation provisions (26 55 71-01), the lack of an engineered boom-mounted smoke evacuation solution (26 55 71-01 Ri and 2655 71-01R2), and the lack of vertical travel capability (26 55 71-01 Ri). Based on all of the above, this submittal has been reviewed in detail with responses to any and all noted clarifications provided, and is being returned "APPROVED AS NOTED, RESUBMIT", with the expectation that KT will provide surgical booms and lights that meet all requirements. This office is performing a detailed analysis of the surgical boom and light revisions that occurred from the original JSI 66 revision on 9/23/13, 6 months prior to the original submittal, through the final design deferred design completion to determine and document the specific revisions in the surgical boom and light design including utilities and overall number of booms and lights required. Any request for additional compensation must be formally addressed by KT in the proper manner as part of the work authorization. Refer to Item 3 below for a potential alternate solution to the equipment boom size increase.

2. It is KT's responsibility to ensure that submittals are coordinated with the current contract design. The VA's review of submittals shall not take the place of KT submittal Quality Control procedures. Detailed review comments are provided, but KT has the ultimate responsibility for compliance.

3. It is not acceptable for the smoke evacuation exhaust to be openly exhausted into the unventilated, non-plenum interstitial space. Either connect the smoke evacuation outlet to the nearest return duct above the ceiling with airtight connection provisions, or provide information from the manufacturer that exhaust provisions through the boom to evacuate the smoke remotely are not required, and the Buffalo ViroVac can be locally exhausted in the surgical space with no risk to the users. If the latter approach is confirmed, the Section R "Upgrade Option" can be revisited, removal of the Smoke E-vac exhaust hose from the boom may allow use of the Amico 90 KG equipment boom to accommodate all specified utilities.

4. The requested information regarding the camera control connector type needs to be addressed in detail in a RFI for review, it is unclear what clarification information is requested.

5. Referenced Variance Request 359 was approved on 9/23/15.

6. Refer to architectural interior elevations for locations of wall control boxes and camera control boxes.

Submit detailed RFI if clarification is required.

Notes/Comments

1. Refer to all noted comments.

2. There are numerous locations in the Amico shop drawings containing statements such as "It is the responsibility of the hospital", "It is the hospital/GC responsibility", "to be provided by the hospital", or "the hospital must provide. The complete installation of the surgical lights and booms and all provisions for such are specified under the construction contract, thus, it is Kis responsibility to provide and coordinate

all such provisions.

3. There are numerous NOTES in the lower left corner of Amico Electrical Schematic Verifications that have been addressed by previous submittal comments regarding compliance with the contract and applicable codes. It is KTs responsibility to ensure that the submitted items are in compliance with all contract requirements and applicable codes.

4. The location of the ceiling grid on numerous field of motion layout drawings is noted as erroneous. It is KTs responsibility to coordinate with contract reflected ceiling plans and applicable equipment plans.

5. For all rooms utilizing isolated power circuits for the booms, circuits are clearly shown with separate home runs from the boom. Do not combine raceways where independent isolated power or critical circuit home runs are shown on contract electrical drawings.

6. There are numerous discrepancies noted for coordination of surgical boom and light manufacturer requirements with contract drawings. Submit the following two RFIs to address these discrepancies prior to any resubmittal:

a. Section K Cystoscopy - Submit RFI regarding coordination of access panel with 2'x4' light fixtures.

b. Submit RFI regarding electrical circuit coordination:

i. Section G OR 03-03-F2-01; Drawing 03.ES.413.2_JSI 118.3 shows only 2 isolated power circuits for perfusion boom receptacles, 3 are required byAmico.

ii. Section N CVOR 03-03-D2-04; Drawing 03.ES.413.2_JSI 118.3 shows receptacle symbol in lieu of junction box for 3 isolated power circuits for equipment boom receptacles.

iii. Section N CVOR 03-03-D2-04; Drawing 03.ES.413.2_JSI 118.3 shows only 2 isolated power circuits for perfusion boom receptacles, 3 are required byAmico.

iv. Section N CVOR 03-03-D2-04; Drawing 03.ES.413.2_JSI 118.3 shows a circuit fora surgical light at the anesthesia boom, no surgical light is specified at this boom.

Buffalo Filter ViroVac™ Operator’s Manual: Page 3

1.1 Introduction

BUFFALO FILTER® ViroVac™ Smoke Evacuation Systems are designed to remove smoke and noxious odors produced by surgical smoke during laser, electrosurgical, ultrasonic, argon, plasma and mechanical instrument procedures.

The ViroVac™ Smoke Evacuation Systems have been designed with a high suction, high flow rate centrifugal action pump. The ultra-quiet motor is used to draw the surgical smoke from the surgical site through the vacuum tubing and into the ViroSafe® filter where the surgical smoke is processed by a series of filters. A single disposable filter is used to simplify the installation and re-moval during filter changes. The filter is completely enclosed to protect the healthcare personnel from potential contamination during filter changes. One Buffalo Filter ViroSafe® filter contains four different stages within to capture the smoke plume.

The first stage filtration is a prefilter whose function is to trap and remove gross particulate and casual fluid.

The second stage filtration is ULPA grade (Ultra Low Penetration Air) filter whose high-tech patented (U.S. Patent #5874052) design captures particulates and micro-organisms from .1 to .2 microns at an efficiency of 99.999%.

The third stage filtration uses the highest grade virgin activated carbon, especially designed for Buffalo Filter for the removal and adsorption of odors and toxic gases produced by burning tis-sues. These harmful gases may constitute a health hazard to healthcare professionals who are subjected to prolonged exposure. The activated carbon used in the ViroVac™ Smoke Evacuation Systems preferentially removes toxic organic gases rather than water vapor and provides optimal odor removal.

The fourth stage filtration is an expanded foam used to trap activated carbon fines from migrating out of the filter.

The electronic controls on the face panel of the ViroVac™ Smoke Evacuation System has been designed “user friendly” and facilitate unit set up and operation. Please refer to Section 2.0 for Operating Instructions.

1.2 Inspection

The ViroVac™ Smoke Evacuator has been thoroughly tested and inspected before shipment from the factory. Please check the unit before using it to insure that no damage has occurred in transit.

If damage is evident, please contact BUFFALO FILTER Customer Service at 1-800-343-2324 (US Only) or (716) 835-7000 (International)
