Scope of Work Project: 436-20-104 ADA Exterior Improvements at Fort Harrison Purpose The purpose of this project is to correct exterior pedestrian sidewalks and associated asphalt crosswalks, curbs, and irrigation located around the Fort Harrison VA Medical Center campus to improve patient access to the facility and comply with ADA and VHA requirements. The medical center requires a Contractor to furnish all labor and materials needed to completely prepare the site for building operations, including all demolition, removal, and installation required by the drawings, specifications, this scope of work, and applicable standards.
Background and Facility A. Existing Site Conditions The project shall furnish and install new concrete sidewalks (see Appendix documentation for effected areas to be included). According to the VA Site Development Design Manual, primary and secondary walkways must be a minimum of 8’ and 6’, respectively, to meet ADA and VHA Accessibility and Montana Public Works Standards.
Tasks and Requirements B. General Contractor Requirements Contractor shall demolish existing non-compliant sidewalks with associated curb and construct new ADA and VHA compliant sidewalks, observing all applicable guidelines and standards. This involves excavation of existing sidewalk and associated base layer(s), removal of curbs, removal of irrigation and turfgrass, and saw cutting, removing and installing new roadway asphalt at crosswalk locations due to poor existing conditions.
Once demolition is complete, the Contractor shall prepare and install the following: new 6 inch thick layer of 1 inch-minus crushed road mix gravel base, approximately 4,000 linear feet of 5 inch thick, mesh reinforced concrete sidewalk (all concrete shall be minimum 4000 psi compressive strength), ADA tactile mats at all roadway crossings and for access in parking lots, curbs (match existing where appropriate), approximately 15,500 square feet of new asphalt at road crossings and for parking lots, new irrigation/turfgrass including associated accessories to replace what was removed during demolition, and hauling of all construction debris off site for proper disposal.
Contractor shall install all proper marking/striping on the asphalt to identify pedestrian crosswalks, per MDOT guidelines. Contractor shall correct existing crosswalk areas to ensure smoother transitions for pedestrians. Crosswalks shall not exceed the maximum slope of 1:12, per ADA standards. All curb correction/construction of crosswalks shall start from the edge of the asphalt and shall be diagonal curb ramps unless otherwise directed by the VA Contracting Officer’s Representative (COR). Contractor shall repaint all curbs, after installation, to match corresponding traffic rules, i.e. yellow for unloading/loading of passengers, red for fire lane, etc.
Contractor shall provide all applicable traffic control measures and acceptable alternative pedestrian routes as needed or required during the period of performance. Contractor shall follow proper communication, construction, and deliverable guidelines as outlined in the specifications. Contractor shall be responsible for field verification all of quantities and measurements related to the project before construction begins.
C. Specific Contractor Requirements The following areas are where improvements shall be performed. Refer to the supporting documentation for visual representation of locations and effort of work for each corresponding location stated below.
Emergency Department and Building 154B Entrances – Sheets CD-101 & CI-101 The entrance into the emergency department (ED) is not compliant with ADA standards. The existing sidewalk shall be removed, and a new, 8’ wide sidewalk installed in its place. The additional space the new sidewalk requires will take advantage of unused space (currently existing gravel) around the southern perimeter of the existing sidewalk, where there are currently large concrete planters. These planters shall be removed to provide the space required. Planters are to be relocated or discarded at the VA COR discretion at the time of removal. Remove and replace all curb during this time and replace with mountable curb for easier patient loading/unloading. New crash-rated bollards shall be installed to prevent vehicles from entering the protected area of the ED. Bollards are to be installed around the semi-circle of the ED entrance, spaced no less than 4’ apart to maintain ADA requirements and block the minimum width of a vehicle from passing through. Paint bollards to match existing features, such as nearby light poles and/or window frames, upon approval by the COR.
The large ramp serving as the entrance into building 154B (Madison Clinic) includes a raised concrete section that the existing handrailing is mounted into. The existing handrailing and associated raised section are in poor condition and require repair. The raised portion of the concrete ramp shall be repaired for the handrailing to be remounted. The handrailing shall be removed, sandblasted, powder coated for weather resistance and reinstalled into the newly repaired section of concrete. Place the handrailing into the concrete using an epoxy grout or approved equal.
In addition, tactile mats are to be installed in the designated areas around the ED where crossing into a shared route with vehicular traffic is present. The mats will serve as a warning for vision-impaired people of the presence of specific hazards, such as where footways join a shared route.
Building 154/141 South Entrance and Handicap Accessible Parking Lots – Sheets CD-102 & CI-102 One of the main entrances into building 154 is the southern entrance on Veterans Drive. The existing sidewalk width is acceptable, but the cross sloping is not compliant. The existing sidewalk is to be removed and a new sidewalk installed with the appropriate cross sloping to provide a drainage gradient to allow water to run off, mitigating a hazardous walking surface. If required, the Contractor shall provide a basic design for the new entrance sidewalk to meet cross sloping requirements, prior to start of work.
The grass island at the main southern entrance to building 154 contains a single length of sidewalk running east/west across it. This sidewalk does not meet ADA requirements and will need to be removed and replaced with a wider, compliant sidewalk. Expand the sidewalk to the north so that the road verge, or boulevard strip, is not disturbed. Tactile mats are to be installed on either end of the island where it meets a shared route into loading/unloading traffic. The west side crosswalk from the island onto the sidewalk near the entrance to building 154 is not sloped properly and shall be saw cut, removed, re-sloped, and new asphalt placed. The new road crossing shall be painted with MDOT approved reflective traffic paint.
All sections of sidewalk along the south, east, and northeast corner of building 141 shall be replaced to adhere to ADA standards. Expand the south sidewalk to the north and the east sidewalk to the west to ensure the road verge and slope, respectively, can be maintained. Tactile mats are to be installed where the sidewalk meets a shared route. If required, the Contractor shall provide a basic design for the new northeast sidewalk to meet cross slope requirements, prior to start of work.
The two handicap accessible parking lots on Veterans Drive, south of buildings 141 and 154, are cross sloped incorrectly. These two lots shall be repaired to achieve proper cross sloping gradients for water runoff/drainage. Removal of the top layer of the asphalt to a depth necessary to create the new cross sloping shall be performed. A new asphalt layer shall be added, resurfaced, and painted with new ADA parking space requirements. If sloping cannot be achieved by this method, full parking lot replacement may be required.
Building 169 West Sidewalk – Sheets CD-103 & CI-103 The sidewalk and associated curb that runs the length of the west side of building 169 shall be removed and replaced with an ADA compliant sidewalk. Expansion of the sidewalk shall be directed to the east so that existing parking lot ‘M’ will not be encroached upon. Tactile mats are to be installed on the north and south side of the improvement area, where the sidewalk meets a shared route into parking lot ‘M’.
Additionally, the southeastern most handicap parking space is not correctly cross sloped to allow for drainage. The asphalt shall be removed, the designated area cross sloped and new asphalt placed. New parking lines, including handicap spaces, shall be reapplied onto the new asphalt to match the old parking pattern and meet MDOT standards.
Parking Lot C, D and E Sidewalk – Sheets CD-104 & CI-104 The sidewalks, and associated curb, located along the northern and eastern perimeters of parking lots ‘C’, ‘D’, and ‘E’ are too narrow and will require replacement to meet ADA standards. The northern sections of sidewalk, in parking lot ‘C’ and ‘D’ shall be removed and replaced with ADA compliant sidewalks. These sidewalks are to be expanded to the south so that the road verge is not disturbed. Four tactile mats are to be installed where the sidewalk meets the shared route of the roadway and parking lot driving lanes. The farthest west crossing has a storm sewer access point located in the sidewalk. This shall be considered when replacing the sidewalk so that the storm sewer can remain and function properly.
The eastern section of sidewalk shall be expanded to the east into the rock filled median along eastern perimeter of parking lot ‘E’. There is no irrigation is in this area. The west side of the security fence surrounding the large evaporator shall be relocated to accommodate the newly expanded sidewalk, considering any minimum clearance concerns. Tactile mats are to be installed where the sidewalk meets the shared route to the north into parking lot ‘F’ driving lane and to the south where the sidewalk crosses the gravel median. All associated road crossings shall be painted onto the asphalt with MDOT approved reflective traffic paint.
VBA South Sidewalk – Sheets CD-105 & CI-105 There are two existing sidewalks and associated curb sections along Veterans Drive, southeast and west of building 167 (VBA), that either do not meet ADA standards or are in poor condition and shall require replacement. The section of sidewalk running directly in front of the VBA, parallel to the loading/unloading zone, does not need replacement. During installation of the new sidewalk sections, ensure expansion to the north so that the road verge is not disturbed. Nine new tactile mats are to be installed where the sidewalk meets the shared route of the roadway and parking lot driving lanes. Existing asphalt removal and replacement is expected around curb demolition/installation and crossings in order to achieve smooth transitions onto the shared route from the sidewalk. Road crossings shall be painted onto the asphalt with MDOT approved reflective traffic paint.
Building 168 (Liberty House) Sidewalks and Parking Lot – Sheets CD-106 & CI-106 Sidewalks and associated curbs running from the intersection of Veterans Drive, Honor Drive and Liberty Lane, including the secondary sidewalks that provide access to building 168, and the entryway concrete pad shall be removed and replaced with ADA compliant sidewalks.
In addition, building 168 east side handicap parking lot ‘Y’ is cross sloped incorrectly. This lot shall be repaired as needed to obtain cross sloping gradients for proper water runoff/drainage. Removal of the top layer of the asphalt to a depth necessary to create the new cross sloping shall be performed. If sloping cannot be achieved by removing the top layer and more aggressive sloping is needed, full parking lot replacement may be required. A new asphalt layer shall be added, resurfaced, and painted with new ADA parking space requirements.
Three tactile mats are to be installed where the sidewalk meets the shared route of the roadway and parking lot. Repaint the intersection crosswalk onto the asphalt with MDOT approved reflective traffic paint.
Liberty Lane Sidewalk – from the Liberty House to the Service Road Crossing – Sheets CD-106 & CI-106 Sidewalk and curb running from the Liberty House, along the south side of Liberty Lane south, to the service road crossing that building 146 is located on is out of compliance with ADA standards and shall be removed and replaced. During installation of the new sidewalk, ensure expansion to the south/east so that the road verge is not disturbed. Relocate and replace any irrigation, and associated materials, that will be in the way of the new sidewalk location. Install tactile mats at the service road crossing for accessibility.
Parking Lot ‘I’ – Sheets CD-107 & CI-107 The handicap parking spaces in parking lot ‘I’ are not correctly cross sloped to allow for drainage. There are three separate handicap parking sections on the west side of the lot. The asphalt shall be removed, the designated area cross sloped and new asphalt placed. New parking lines, including handicap spaces, shall be reapplied onto the new asphalt to match the old parking pattern and meet MDOT standards.
Island Sidewalk – North of Building 172 – Sheet CI-108 The stretch of land between Devil’s Brigade Drive and Patriots Way, ending at General Frederick Drive, shall require a sidewalk for patients coming from parking lot ‘I’ and walking up to the ED entrance of the facility. There is no existing sidewalk in this location, which puts pedestrians at risk from traffic in both directions when walking on Patriots Way. A new sidewalk shall be constructed from the newer sidewalk inside the island, directly west, and tie into the existing sidewalk running from parking lot ‘K’, directing pedestrians up the hill and into the ED entrance safely. The new sidewalk shall comply with VA barrier free design standards and ABAAS design standards. Use most stringent design standards and clarify them with COR. Contractor to provide a basic design for the new sidewalk, to include new handrailing, prior to start of work.
Tactile mats are to also be installed where pedestrians will meet the shared route of the Frederick Drive while going to and from parking lot ‘I’. Repaint the intersection crosswalk onto the asphalt with MDOT approved reflective traffic paint.
D. Applicable Guidelines and Standards Program guides, design manuals, and other references below may be found in the Veteran Affairs Technical Information Library (TIL) https://www.cfm.va.gov/til/.
PG18-10: Site Development Design Manual PG18-01: Master Construction Specifications Deliverables The Contractor shall supply all deliverables outlined in the specifications. The Contractor will furnish fully compliant ADA accessible routes by the end of the period of performance.
Period of Performance Anticipated period of performance is 180 days and is not expected to require an extension to contract period of performance. Notify the VA of any exceptions before submitting proposal for increase to cost or changes to project schedule.
E. Work Schedule Work must be scheduled to allow for patient access to patient care facilities or a solution must be proposed and approved by the VA COR before work proceeds. Work may be performed during normal business hours or after hours but not on holidays or weekends without prior COR approval. Coordination with COR and hospital staff before demolition and construction is required in order not to disrupt VA operations.

It is the intent of the Government to award without discussions; however, the Government will reserve the right to have discussions if such are needed.
I.3A PRE-PROPOSAL SITE VISIT
See SF 1442, Block 10 and FAR Provision 52.236-27, Site Visit (Construction) of this solicitation.
I.4A SELECTION CRITERIA
This acquisition will utilize the tradeoff process in accordance with FAR 15.101-1. This process permits tradeoffs between non-cost factors and cost/price and allows the Government to accept other than the lowest priced proposal or other than the highest rated proposal to achieve a best value contract award based on established evaluation criteria. Inherent in this process is the necessity to make tradeoffs considering the non-cost strengths and weaknesses, risks, and the cost (or price) offered in each proposal.
The source selection process will be conducted utilizing FAR part 15.101-1 Trade-off Process Source Selection Procedures with Special Standards of Responsibility. Offers will be evaluated using the following evaluation factors:
Factor 1 – Past Performance Factor 2 – Price
Initially offers shall be ranked according to price, including any option prices if applicable. The lowest price offer’s past performance shall be evaluated to determine its performance confidence rating. The past performance evaluation results are an assessment of the offeror’s probability of meeting the minimum past performance solicitation requirements. There are two aspects of the past performance evaluation: (1) the assessment is based on the offeror’s record of relevant and recent past performance information that pertain to the products and/or services outlined in the solicitation requirement; and (2) determine how well the contractor performed on the contracts.
If the lowest priced offer is judged to have a Substantial Confidence performance confidence rating and meets the special standards of responsibility, that offer represents the best value for the Government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers.
If the lowest price offer does not receive a Substantial Confidence performance confidence rating, the past performance evaluation continues until an offeror’s past performance is judged to have a Substantial Confidence performance rating. The Government reserves the right to award a contract to other than the lowest priced offeror if the lowest priced offeror is judged to have a performance confidence assessment of "Satisfactory Confidence" or lower. The Government also reserves the right to award to the lower priced offeror with lower past performance rating if the Source Selection Authority (SSA) determines the price difference does not merit awarding to the higher priced offeror. In that event, the Source Selection Authority shall make an integrated assessment best value award decision.
Competing offerors' past performance history will be evaluated on a basis of significantly more important than, cost or price considerations.
Special Standard of Responsibility
The Offeror shall submit the following information: (For a partnership or joint venture, the following submittal requirements are required for each Contractor who is part of the partnership or joint venture and for the entity itself. EMR shall not be submitted for subcontractors.)
(1) OSHA or EPA Violation(s):
Offerors shall provide Summary of Work-Related Injuries and Illnesses, affirming that the Offeror has no more than three serious, or one repeat, or one willful OSHA or EPA violation(s) in the past 3 years
(2) Experience Modification Rate (EMR):
Offerors shall provide documentation to show that the offeror has an Experience Modification Rate (EMR) of equal to or less than 1.0.
Offerors who fail to meet any special standard of responsibility will not be eligible to receive contract award.
Past Performance Evaluation For Factor 1, based on the evaluation of all recent and relevant past performance for an offeror, an overall performance confidence rating shall be assigned. The performance confidence ratings are defined below:
ASSESSMENT
DESCRIPTION
Substantial Confidence
Based on the offeror’s recent/relevant performance record, the Government has a high expectation that the offeror will successfully perform the required effort.
Satisfactory Confidence
Based on the offeror’s recent/relevant performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort.
Limited Confidence
Based on the offeror’s recent/relevant performance record, the Government has little expectation that the offeror will be able to successfully perform the required effort.
No Confidence
Based on the offeror’s recent/relevant performance record, the Government has no expectations that the offeror will be able to successfully perform the required effort.
Unknown Confidence (Neutral)
No recent/relevant performance record is available, or the offeror’s performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned.
Note: In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305(a)(2)(iv)). Therefore, the offeror shall be determined to have unknown past performance.
(a) Aspects of Past Performance Evaluation. The past performance evaluation results are an assessment of the offeror’s probability of meeting the minimum past performance solicitation requirements. This assessment is based on the offeror’s record of relevant and recent past performance information that pertain to the products and/or services outlined in the solicitation requirements. There are two aspects of the past performance evaluation.
(1) The first is to evaluate whether the offeror’s present/past performance is relevant or not relevant to the effort to be acquired. The criteria to establish what is relevant is unique to each source selection.
(2) The second aspect of the past performance evaluation is to determine how well the contractor performed on the contracts. The past performance evaluation performed in support of a current source selection does not establish, create, or change the existing record and history of the offeror’s past performance on past contracts; rather, the past performance evaluation process gathers information from customers on how well the offeror performed those past contracts. In addition, the Government reserves the right to obtain information for use in the evaluation of past performance from all sources including sources outside of the Government. Other sources may include, but are not limited to, past performance information retrieved through the Past Performance Information Retrieval System (PPIRS) using all CAGE/DUNS numbers of Contractors who are part of a partnership or joint venture identified in the Offeror’s proposal, inquiries of owner representative(s), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), and any other known sources not provided by the Offeror.
(b) Source Selection Evaluation Board (SSEB) members will review past performance information to determine the quality and usefulness as it applies to the performance confidence assessment.
Adverse past performance is defined as past performance information that supports a less than satisfactory rating on any evaluation element or any unfavorable comments received from sources without a formal rating system.
The government intends to award a contract without discussions with respective offerors. The government, however, reserves the right to conduct discussions if deemed in its best interest.
Factor 1-Past Performance Offerors shall provide a maximum of five (5) recent and relevant projects (as defined below) to demonstrate their ability to perform the proposed work identified within the Specifications, drawings and Statement of Work. The contracting officer shall review relevant performance information on each offeror based on (1) the past efforts provided by the offeror and (2) data independently obtained from other government and commercial sources. Relevant performance includes performance of efforts that includes renovation of an existing space in an active hospital that is similar or greater in scope, magnitude and complexity than the effort described in this solicitation. A recent project is defined as being completed within five years of the due date of this solicitation. The purpose of the past performance evaluation is to allow the government to assess the offeror’s ability to perform the effort described in this RFP, based on the offeror’s demonstrated past performance. Past performance regarding predecessor companies, key personnel who have relevant experience, or sub-contractors that will perform major or critical aspects of the requirement will not be considered as highly as past performance information for the principal offeror. Design Build Projects will be rated more favorability. Offerors with no relevant past performance history or the offeror’s performance record is so limited that no confidence assessment rating can be reasonably assigned shall receive the rating "Unknown Confidence," meaning the rating is treated neither favorably nor unfavorably.
If the lowest priced evaluated offer is judged to have a Substantial Confidence performance assessment, that offer represents the best value for the government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers.
The government reserves the right to award a contract to other than the lowest priced offeror if the lowest priced offeror is judged to have a performance confidence assessment of "Satisfactory Confidence" or lower. In that event, the Source Selection Authority shall make an integrated assessment best value award decision.
Offerors are cautioned to submit enough information in the format specified in the solicitation. Offerors may be asked to clarify certain aspects of their proposal (for example, the relevance of past performance information) or respond to adverse past performance information to which the offeror has not previously had an opportunity to respond. Adverse past performance is defined as past performance information that supports a less than satisfactory rating on any evaluation element or any unfavorable comments received from sources without a formal rating system. Communication conducted to resolve minor or clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for proposal revision.

· Comparison of proposed prices received in response to the RFP
· Comparison of proposed prices with the IGCE.
· Comparison of proposed prices with available historical information.
Special Standard of Responsibility Solicitation Submittal Requirements:
The Offeror shall submit the following information: (For a partnership or joint venture, the following submittal requirements are required for each Contractor who is part of the partnership or joint venture and for the entity itself. EMR shall not be submitted for subcontractors.)
(1) OSHA or EPA Violation(s):
Offerors shall provide Summary of Work-Related Injuries and Illnesses, affirming that the Offeror has no more than three serious, or one repeat, or one willful OSHA or EPA violation(s) in the past 3 years
