STATEMENT OF WORK

Modify Surgical Clean Core Project 575-20-201 VA Western Colorado Health Care System

OBJECTIVE:

Provide labor, materials, equipment, and other services as necessary to improve fire safety through separation and storage improvements in the Surgical Clean Core (Room 3635) and adjacent storage rooms on the 3rd floor in Building 1 at the Grand Junction VA campus, 2121 North Ave., Grand Junction, CO 81501.

The project area is in the surgical suite, and requires the highest levels of infection control. Effectiveness of infection control measures and quality of renovation work rather than efficiency of construction will be the measures of success for this project.

All work in each phase shall be completed, inspected, accepted and occupied by the Government prior to any work associated with the next phase begins.

SCOPE OF WORK:

All work in each phase shall be completed, inspected, accepted and occupied by the Government prior to any work associated with the next phase. Inspection and acceptance shall be by Contracting Officer’s Representative (COR) and other staff as designated by the COR.

1. Purchase modular reusable temporary containment wall system meeting requirements of Specification Section 01 35 30 for use as infection control barrier. Ceilings in this area are 10 feet from floor. Infection control barriers will be under control of the contractor during work but will remain the property of the VA at the end of the project.

2. Phase I: Modifications in OR4 (Room 3643)

a. VA will remove and store furnishings and equipment from the work room.

b. Install infection control walls as shown on plans. Provide air scrubber for each separate area. Infection control measures may be left in place until work on this phase is complete.

c. Cut 2ft x2ft access as needed for application of fire insulation (next paragraph.) VA Maintenance staff will inspect the area through the access holes and point out J-boxes, labels, and other utility elements that must be protected from insulation application. Contractor must be prepared to apply protection for ten (10) such utility elements.

d. Apply 3M 1-Hour fire insulation on walls above ceilings between OR4 and the Clean Core and Sterile Supply (Room 3650).

e. Install two 2’x2’ access panels meeting specification section 08 31 13 at locations selected by station M&O Foreman. Selection of location will take place once this phase of construction starts.

f. Repair hard-lid ceiling after insulation has been inspected and accepted.

g. Install differential pressure transducers between each OR and the corridor, and between each OR and the clean core. Each transducer to operate from -0.5 to +0.5 in of water, have a pressure display on the sensor, support future output via 4-20ma signal, and be installed in room 3667. Polytube from sensors to be 3/8”. Sensor ends to be terminated at single gang stainless steel pressure pickup wall plate located 10’ to 20’ from the monitored door and located more than 10 feet from any supply air.

h. Clean work area thoroughly, including infection control walls and equipment.

i. Remove infection control measures.

3. Phase II: Modifications in Sterile Supply 3650

a. VA shall remove equipment and supplies from the room and store.

b. Disconnect existing sterilizer from northern wall, remove and dispose. (Allow VA to prepare disposal paperwork before removal.) Disconnect and move southern sterilizer to northern location.

c. Install infection control in clean core (room 3635) and in Anesthesia Office (room 3649) as shown on plans. Provide two air scrubbers as shown. Infection control measures may be left in place until work of this phase is complete.

Contractor must be prepared to apply protection for ten (10) such utility elements.

f. Apply fire caulking to any openings in walls in room 3650. Apply 3M 1-hour rated fire insulation on walls above ceiling to deck, and over ceiling. Cut access panels in hard-lid ceiling as needed for the work.

g. Provide permanent access panels in at least two locations that between them will allow visual inspection of all of the completed work. VA will select locations when project is under construction. Repair drywall in other locations.

h. Clean work area thoroughly, including infection control barriers.

i. Remove Infection Control measures

4. Phase III: Relocate Omnicell within Clean Core (room 3635)

1. Please see FAR 15.207(c) for a description of the steps the Government shall take with regard to unreadable offers.

You may send both the spreadsheet and the spreadsheet saved in PDF format to ensure that your quote is readable.

(d) Alternate offers.

(1) When an offer includes foreign solicitation in paragraph (b)(2) of the clause at FAR 52.225-9, the offeror also may submit an alternate offer based on use of equivalent domestic construction material.
