1. Please provide the PPQ forms to be used for the solicitation. ***I don’t know what is being asked for here. Attached to A001
1. Please provide a price/cost sheet to be used for the proposal. Included in A001
1. Please clarify if the solicitation is for a NESHAP asbestos survey of a building or 3-year reinspection of the buildings as the solicitation says that there is base year and 4 option years as mentioned on Page 5 of the solicitation. Solicitation is for NESHAP surveys. The VA no longer follows AHERA and does not have a 3-year reinspection requirement.
1. Please provide the list of building or buildings to be surveyed for this solicitation along with the square footage and the floor plans of the buildings. This will vary depending upon the requirement at the time.
1. Please provide the ear of construction and age of the buildings to be surveyed. Various. This is a network wide BPA that covers any VA buildings in seven states.
1. Please clarify if there is a previous asbestos survey report for the building or buildings that needs to be surveyed and that report would be made available to the contractor for use. Previous asbestos survey reports can only be utilized as refence if they have been maintained and are less than 3 years old. No assumptions shall be accepted, unless the facility Asbestos Program Manager agrees to assume the material in question is POSITIVE and will be abated accordingly.
1. Please clarify that the contractor can use the previous sampling results available in the survey report for the survey report i.e. any material previously found to be positive or negative does not need to be sampled and tested again. This depends upon the age and validity of the facility’s previous AHERA survey. If the survey is over 3-years old, no the sampling information may not be used.
1. Please confirm that the contractor does not need to sample or quantify any replacement materials that have been installed after the removal of asbestos materials in the buildings. If that facility’s Asbestos Program Manager has notarized affidavit of “Non-use of Asbestos Containing Building Materials” on file for replacement materials that are less than 3-years old, then the contractor does not need to sample. If notarized affidavits are unavailable or are over 3-years old, the contractor shall sample.
1. Please confirm that if the contractor is performing the 3-year reinspection survey, the contractor only needs to inspect the materials identified in the previous report and does not need to survey or sample any additional material. VA does not require triennial AHERA surveys anymore. This is NOT an AHERA reinspection survey.
1. Please clarify if a NESHAP survey is to be performed on any building, does it need to be a destructive or non-destructive survey. NESHAP surveys are destructive.
1. Please clarify if the period of performance means that the contractor needs to complete the field activities in 30 days or complete field activities and reporting in 30 days. Generally the scope of the NESHAP will be of a smaller nature and therefore the expectation is that the field activities and the reporting will be completed within 30 days. This is NOT a full facility triennial AHERA survey. The survey will be performed in anticipation of renovation, demolition, or remodeling related work.
1. Please clarify if the NESHAP survey to be performed for the buildings that will be demolished or renovated or the survey is for record purposes. Both. The NESHAP surveys are legal records and required to be maintained by the facility Asbestos Program Managers for 30 years.
1. Please clarify if the contractor has to update the existing asbestos survey report with the latest quantities or does the contractor needs to prepare a new report. If a new report has to be prepared, will the contractor be provided with a format by the VA or contractor can use their own format based on guidelines. The contractor should prepare a new report. Contractor should use a standardized format based upon guidelines and best industry practices.
1. Please clarify if the contractor will be able to access all buildings in the normal working hours and no weekend or after hour work is required. Typically the work is expected to be conducted during normal working hours. Operations or emergencies may dictate the need for the contractor to work with the local COR on working outside normal work hours.
1. Please confirm that the contractor does not need to access any confined spaces for survey. The contractor should anticipate needing to access confined spaces depending up on the facility and the nature of the planned renovation, demolition, or remodeling work precipitating the NESHAP survey.
1. Please confirm that the contractor does not need to enter any electrical substations or areas for survey. This is highly unlikely; however this is a network wide BPA covering VA facilities in seven states.
1. Please provide up to date survey data for the building or buildings that need to be surveyed. Those should be requested from the applicable local facility at the time of scheduling for a NESHAP survey.
1. Clarification: THERE Is NO IDIQ to be awarded from this solicitation ONLY the work at the SLC VAMC.
