STATEMENT OF WORK (SOW)
06/12/2019
1. Requirement Title. Challenge Coins
2. Background.
The challenge coins will acknowledge the hard work of our partners in a cultural context that is meaningful to them. They will be a symbol of commitment to suicide prevention and a token of appreciation for being a part of Together With Veterans (TWV). The phrases on the coin like “collaborative” and “Veteran-Driven” will serve as a reminder of the key principles to the coin holders who can always refer back to these mementos.
3. Scope.
Provide challenge coins to program partners to distribute to members in their community who are working towards suicide prevention in the Together With Veterans Program and other Mental Illness Research, Education, and Clinical Centers (MIRECC) projects.
4. Specific Tasks.
4.1 Provide graphic design services for finalizing coin design within cost estimates. Graphic design services to be available between each order to finalize coin appearance towards the program purpose as needed.
4.1.1 Each order will have the opportunity of a one-hour discussion with the contractor and/or designer and three rounds of edits after the phone call to finalize appearance of the coin as needed.
4.2 Produce a challenge coin with a different imprint on each side of the coin.
Product features:
• 3mm thick metal (brass or copper)
• 2” in diameter
• Matte nickel plating
• Color on both sides
• Option for edging features
• Coin should be individually package in a clear plastic sleeve
4.2.1.1 Sample Mock up for pricing purposes:

One side will have a colored (red, white, blue) TWV logo surrounded by the text “Together with Veterans” and “Preventing Suicide.” The other side will have two hands shaking with text over them “Veterans helping Veterans.” On the outer circle, principles of the TWV project will be printed which are: Veteran-Driven, Collaborative, Community-Centered, Evidence-Informed. Stars will serve as spacers between the text.

5. Performance Monitoring The COR/POC will require a proof or sample of the product prior to large productions or if there are any changes to the product between productions.
6. Security Requirements There are no Security concerns or requirements as this contract requires the production of a physical product.
7. Other Pertinent Information or Special Considerations.
a. Identification of Possible Follow-on Work.
The initial batch purchased will be 5000 coins. All follow up batches will be purchased in batches of 1000 coins at a time. Batches will be purchased as needed with a maximum batch in a year equaling 10000 coins.
b. Identification of Non-Disclosure Requirements.
Not Required
8. Risk Control If for any reason the coins would be found to be faulty (i.e. metal or color wear or flaking) from normal storage or use the remaining undistributed coins would be returned and refunded or replaced at the Vendors expense.
9. Place of Performance.
All production of the product will be completed at the contractor’s site and then shipped via pallet and/or individual boxes to the purchaser’s desired location.
10. Period of Performance.
Base year plus four one-year option periods.
11. Delivery Schedule.
Initial batch of 5000 and subsequent batches of 1000 shall be delivered as requested with a minimum of one-month notice provided prior to expected delivery date. Coordination and cost of shipment to be covered by the contractor with informational assistance from the COR/POC. No individual box of coins should weigh over 40 lbs.
