The D-B contractor shall ensure that no damage occurs to any part of the 1st Floor, H Zone during this project.
A set of drawings with a previous design for the new addition shall be made available for reference. However, the D-B contractor shall be responsible for determining the best location for the containment wall that shall ensure the remaining building remains structurally sound and compliant with all applicable standards.
Design shall incorporate structural integrity requirements to support ongoing hospital operation. The D-B contractor is responsible for investigating all existing conditions including locating and analyzing structural systems and determining the adequacy of the structural system. This includes identifying options for new components (if required by the design) within the building to ensure proper support after G Zone demolition. If remaining structure is found to be seismically deficient, the D-B contractor shall be responsible for providing the required design and construction services for seismic retrofitting of the remaining structure in accordance with seismic zone that Salt Lake City is located in.
D. Utilities In general, any utilities in the demolition area shall be terminated appropriately to prevent damage and maintain service to the main hospital during the waiting period for a new design and construction of the new addition. All abandoned utilities shall be removed and capped appropriately. For water lines that need to be capped, no dead legs shall be created by capping. The utilities providing service to the B Zone (from G Zone) and the rest of the hospital shall be rerun for continued operation. Utilities not removed with demolition and near the installation of the containment wall shall be prepared for winterizing for protection of approximately 5 years until construction begins on the new addition.
The electrical panel boxes located in room 1G18 have been evaluated and provide power only to the G Zone. The D-B contractor shall verify the only load on the electrical panel is for the G Zone. Remove conduit and any electrical power components in the demolition area that is not supporting operations in the remaining portions of Building 1. Re-route any fiber optic and telephone wires back to Building 1, 1st Level, B Zone. Any fiber optic and telephone wire requiring abandonment must be removed as part of the demolition.
The air handler and HVAC ducts located in the Northwest wing of the 1st Floor, G Zone supplying the 1st Floor, B Zone shall be removed with the demolition. The HVAC duct in B Zone shall be terminated, and the HVAC system shall be connected to another air handler and re-balanced. HVAC ducts shall be relocated and balanced to provide service to the 1st Floor, B Zone until the new addition is designed and constructed. D-B contractor is to work with the VA Engineering Contracting Officer’s Representative (i.e. Project Manager) for any coordination that is necessary in performing this work that may be inherent to the knowledge of VA HVAC Technician personnel. As part of design submissions, the D-B contractor is to ensure HVAC Design Guides are adhered to, to the extent practicable.
Steam pipes and plumbing from the main hospital to B Zone (designated to be part of the demolition) shall have steam traps installed to prevent dead legs. Steam lines shall have an end of line LP steam trap installed to maintain continuity. Waste water and storm drain lines shall have an installed floor or wall clean out where line is demolished. The hot water supply and hot water return shall have a balancing valve at the end of the line. Recirculation pump(s) may also be needed. Cold water shall be demolished up to the closest ‘T’ to eliminate legionella potential.
E. Civil The demolition shall impact the surrounding area. The sidewalk that is connected to NE wing door of G Zone shall be removed. The sidewalks next to Baker and Bennion Street shall not be removed or damaged. Upon removal of demolition debris, the surrounding area shall be graded for proper drainage. The site shall be stabilized to prevent erosion and runoff of topsoil. Please note there is an adjacent active construction site to the project area covered under this scope; as such, construction activity cannot infringe on adjacent established construction areas nor adjacent sidewalk that must be maintained for ingres/egress to Building 1.
F. Other Any power outages and other utility interruptions that D-B contractor determines are required shall be scheduled with VA with lead times consistent with the VA Master Construction Specifications.
Infection Control Contractor shall incorporate infection control measures per VA Standards into the design with care that must be taken for construction near patient care areas. PART I - INSTRUCTIONS, CONDITIONS AND NOTICES TO OFFERORS
PART I/SECTION A – REQUEST FOR PROPOSAL
I.1A JOINT VENTURES
If submitting an offer as a joint venture as defined by P.L. 109-461 AND Department of Veterans Affairs I.L. 049-06-4, http://www.vetbiz.gov/library/jtventure.pdf, an offeror must provide a copy of the joint venture agreement specific to the project that is consistent with the percentage of required work and net proceed. Additionally, the offeror must provide a breakdown of labor of that which is to be performed by the SDVOSB firm and that which is performed by the small business joint venture firm and that which is sub-contracted. All Joint Ventures will be required to be registered, verified and visible as an entity in VETBIZ prior to award.
I.2A SELECTION PROCEDURES
This acquisition will utilize the two-phase design-build selection procedures in accordance with FAR 36.301. Phase I shall include the scope of work, evaluation factors as stated in FAR 36.303-1, Phase II evaluation criteria. A maximum number of five (5) offerors will proceed to Phase II. Once the Contracting Officer selects the most highly qualified offerors, they will be asked to submit Phase II proposals. Phase II of the solicitation shall be prepared in accordance with FAR Part 15, with evaluation criteria developed in accordance with FAR 15.304. The Government will award a contract resulting from this solicitation to the offeror whose offer conforming to the solicitation is determined by the Source Selection Authority to provide the best overall value to the Government. It is the intent of the Government to award without discussions; however, the Government reserves the right to have discussions if such are needed.
I.3A PRE-PROPOSAL SITE VISIT
A pre-proposal site visit will be conducted in Phase II. The most highly rated firms selected to proceed to Phase II will be invited to attend the pre-proposal site visit. The Contracting Officer will establish the date and time and notify Phase II offerors in writing. See FAR Clause 52.236-3, Site Investigation and Conditions Affecting the Work, and FAR Provision 52.236-27, Site Visit (Construction) of this solicitation.
I.4A SELECTION CRITERIA
General: Phase I proposals will be evaluated to determine the most highly qualified offerors to advance to Phase II. Phase II Proposals will be evaluated and award will be made on the basis of both price and technical considerations most advantageous to the Government per FAR Part 15.101(a), Tradeoff Process. The Government may consider award to other than the lowest priced offeror or other than the highest technically rated offeror if it is in the best interest of the Government.
For Phase 1, Factors 1-4 will be evaluated and are listed in descending order. Factor 1— Experience and Factor 2—Safety will be rated on an acceptable or unacceptable basis. An unacceptable rating on Factor 1 and/or Factor 2 will result in the offeror not progressing to Phase II.
Offerors who are selected to move on to Phase II will be requested to submit a Phase II proposal. Factors 5-7 will be evaluated in Phase II and are listed in descending order of importance. The combined non-cost/price factors are significantly more important than price.
Phase I:
Factor 1 – Experience Factor 2 – Safety Factor 3 – Past Performance Factor 4 – Technical Approach
Phase II:
Factor 5 – Technical Solution Factor 6 – Schedule Factor 7 – Price
Factors 1 and 2 will be evaluated on an acceptable or unacceptable basis. An unacceptable rating on either Factor 1 or 2 will result in the offerors proposal being eliminated from further evaluation and ineligible for award. Acceptable and unacceptable ratings are defined below Definitions
Acceptable
Proposal clearly meets the minimum requirements of the solicitation
Unacceptable
Proposal does not clearly meet the minimum requirements of the solicitation
For Factor 3 (Past Performance Evaluation) Past Performance shall be evaluated when using the two-phase design-build process in accordance with FAR 36.303-1. For Factor 3, based on the evaluation of all recent and relevant past performance for an offeror, an overall performance confidence rating shall be assigned. The performance confidence ratings are defined below:
Definitions
Substantial Confidence
Based on the offeror’s recent/relevant performance record, the Government has a high expectation that the offeror will successfully perform the required effort.
Satisfactory Confidence
Based on the offeror’s recent/relevant performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort.
Limited Confidence
Based on the offeror’s recent/relevant performance record, the Government has a little expectation that the offeror will successfully perform the required effort.
No Confidence
Based on the offeror’s recent/relevant performance record, the Government has no expectation that the offeror will be able to successfully perform the required effort.
Unknown Confidence (Neutral):
No recent/relevant performance record is available or the offeror’s performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned.
For Factors 4, 5 and 6, based on the evaluation of the information provided in the construction schedule, an overall adjectival rating will be assigned. The adjectival ratings are defined below:
Definitions
Outstanding
Proposal meets requirements and indicates an exceptional approach and understanding of the requirements. The proposal contains multiple strengths and no deficiencies. Risk of unsuccessful performance is very low.
Good Proposal meets requirements and indicates a thorough approach and understanding of the requirements. Proposal contains at least one strength and no deficiencies.
Acceptable
Proposal meets requirements and indicates an adequate approach and understanding of the requirements. Strengths and weaknesses are offsetting or will have little or no impact on contract performance. Risk of unsuccessful performance is moderate.
Marginal
Proposal does not clearly meet requirements and has not demonstrated an adequate approach and understanding of the requirements. The proposal has one or more weaknesses which are not offset by strengths. Risk of unsuccessful performance is high.
Unacceptable
Proposal does not meet requirements and contains one or more deficiencies. Proposal is not awardable.
A strength, weakness, or deficiency used to evaluate offerors’ proposal for each of the non-price factors are as defined below.
Definitions
Strength
An aspect of an offeror’s proposal that has merit or exceeds specified performance or capability requirements in a way that will be advantageous to the Government during contract performance.
Weakness
A flaw in the proposal that increases the risk of unsuccessful contract performance.
Deficiency
A material failure of a proposal to meet a Government requirement or a combination of significant weaknesses in a proposal that increases the risk of unsuccessful contract performance to an unacceptable level.
Phase I Factor 1 – Experience:
(a) Solicitation Submittal Requirements:
The Offeror shall submit the following information:
1. Design/ Build Experience:
Submit a minimum of one (1) and a maximum of three (3) design/build construction projects for the offeror that best demonstrates your experience on relevant projects that are similar in size, scope, and complexity to the RFP. The offeror shall have relevant design/build experience as a prime construction contractor managing multiple subcontractors as well as an Architect/Engineering firm (A/E) as a subcontractor. For purposes of this evaluation, a relevant project is further defined as a contract that was officially awarded as a design/build contract consisting of: 1) modification of utility systems effecting an occupied space with minimal impact on the existing space during construction and/or 2) demolition of a building attached to occupied space. Project(s) shall have a minimum value of $500,000 and shall have been completed, including all options, within the past five (5) years of the date of issuance of this RFP.
A project is defined as a design/build project performed under a single task order or contract. For multiple award and indefinite delivery/indefinite quantity type contracts, the contract shall not be submitted as a project; rather offerors shall submit the work performed under a task order as a project.
The attached Construction & Design Experience Project Data Sheet (Attachment D) is MANDATORY and SHALL be used to submit project information. If the same project is being used to demonstrate construction and design experience, submit separate Project Data Sheets for construction and design. Except as specifically requested, the Government will not consider information submitted in addition to this form. Individual blocks on this form may be expanded; however, total length for each project data sheet shall not exceed one (1) double-sided page (or two (2) single-sided pages).
For all submitted projects, the description of the project shall clearly describe the scope of work performed and the relevancy to the project requirements of this RFP (e.g.: unique features, area, design/build methods). In addition, the description should also address any sustainable features for the project, including specific descriptions of those features. Provide applicable documentation on projects that were validated and/or certified through U.S. Green Building Council (USGBC) or the equivalent organization or process.
If the offeror is a Joint Venture (JV), relevant project experience should be submitted for projects completed by the Joint Venture entity. If the Joint Venture does not have shared experience, projects shall be submitted for each Joint Venture partner. Offerors who fail to submit experience for all Joint Venture partners may be rated lower. Offerors are still limited to a total of three (3) projects combined.
The offeror may utilize experience of a subcontractor that will perform major or critical aspects of the requirement to demonstrate construction experience under this evaluation factor. The Offer must provide a letter of commitment and an explanation of the involvement that the subcontractor will have in performance of this contract.
1. Design Experience
Submit a minimum of one (1) and a maximum of three (3) design projects for the A/E firm that will be used for this project that best demonstrates design experience on relevant projects that are similar in size, scope, and complexity to the RFP. For purposes of this evaluation, a relevant project is further defined as consisting of: 1) modification of utility systems effecting an occupied space with minimal impact on the existing space during construction and/or 2) demolition of a building attached to occupied space. Project(s) shall have a minimum “construction” value of $5000,000 and shall have been completed, including all options, within the past five years of issuance of the RFP.
For design-build projects, the design portion of the contract shall have been completed within the past five (5) years of the date of issuance of this RFP.
A project is defined as a complete design effort performed under a single task order or contract/subcontract. For multiple award and indefinite delivery/indefinite quantity type contracts, the contract as a whole shall not be submitted as a project; rather offerors shall submit the work performed under a task order as a project.
For all submitted projects, the description of the project shall clearly describe the scope of work performed and the relevancy to the project requirements of this RFP (e.g. unique features, area, construction methods).
The offeror may utilize experience of a design subcontractor to demonstrate design experience under this evaluation factor. The Offer must provide a supporting joint venture agreement, partnership agreement, teaming agreement, or letter of commitment and an explanation of the involvement for the design subcontractor.
(b) Basis of Evaluation:
To be considered acceptable for this factor, the offeror must demonstrate construction experience as a prime construction contractor managing multiple subcontractors and an A/E as a subcontractor on a minimum of one (1) design/build project consisting of at least one of the following: 1) utility systems modifications effecting an occupied space that had to be minimally impacted during construction or 2) demolition of a building attached to occupied space. Project(s) shall have a minimum value of $500,000 and shall have been completed, including all options, within the past five years of issuance of the RFP.
Additionally, the offeror must demonstrate design experience for the A/E being indicated on the team as a subcontractor on a minimum of one (1) project consisting of at least one of the following: 1) design of utility systems effecting an occupied space that had to be minimally impacted during construction or 2) design of demolition of a building attached to occupied space. Project(s) shall have a minimum “construction” value of $5000,000 and shall have been completed, including all options, within the past five years of issuance of the RFP.
Factor 2 – Safety
(a) Solicitation Submittal Requirements:
The Offeror shall submit the following information: (For a partnership or joint venture, the following submittal requirements are required for each Contractor who is part of the partnership or joint venture. EMR shall not be submitted for subcontractors.)
1. OSHA or EPA Violation(s):
Offerors shall provide a letter affirming that the offeror has no more than three serious, or one repeat, or one willful OSHA or EPA violation(s) in the past 3 years.
(2) Experience Modification Rate (EMR):
Offerors shall provide documentation to show that the offeror has an Experience Modification Rate (EMR) of equal to or less than 1.0.
(b) Basis of Evaluation: The Government will evaluate the documentation to ensure that the contractor has no more than three serious, or one repeat, or one willful OSHA or EPA violations(s) in the past three years; and must have an Experience Modification Rate (EMR) of equal to or less than 1.0.
NOTE: Failure to submit the OSHA or EPA violation documentation and EMR will result in the proposal being considered deficient and an overall rating of unacceptable will be assigned.
Factor 3 – Past Performance:
(a) Solicitation Submittal Requirements:
If a completed Construction Contractor Performance Assessment Reporting System (CPARS) evaluation is available, it shall be submitted with the proposal for each project included in factor 3 for design/build experience. If a completed Architect-Engineer (A-E) CPARS evaluation is available, it shall be submitted with the proposal for each project included in factor 3 for design experience. If there is not a completed CPARS for either construction or A-E evaluation then submit Past Performance Questionnaires (Attachment C) for each project included in Factor 3 for both Construction Experience and Design Experience. The offeror should provide completed Past Performance Questionnaires (PPQ) in the proposal. Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other RFPs. However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation. If the offeror is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the offeror shall complete and submit with the proposal the first page of the PPQ, which will provide contract and client information for the respective project(s). The Government may make reasonable attempts to contact the client noted for that project(s) to obtain the PPQ information. However, offerors should follow-up with clients/references to help ensure timely submittal of questionnaires. If the client requests, questionnaires may be submitted directly to the Government’s point of contact, Joseph Wingfield, joseph.wingfield@va.gov.
Offerors may provide any information on problems encountered and the corrective actions taken on projects submitted under Factor 1 – Experience. Offerors may also address any adverse past performance issues. Explanations shall not exceed two (2) double-sided pages (or four (4) single-sided pages) in total.
The Government reserves the right to contact references for verification or additional information. The Government’s inability to contact any of the offeror’s references or the references unwillingness to provide the information requested may affect the Government’s evaluation of this factor.
Performance award or additional information submitted will not be considered.
(b) Basis of Evaluation:
This evaluation focuses on how well the offeror performed on the relevant projects submitted under Factor 1 – Experience and past performance on other projects currently documented in known sources. In addition to the above, the Government reserves the right to obtain information for use in the evaluation of past performance from all sources including sources outside of the Government. Other sources may include, but are not limited to, past performance information retrieved through the Past Performance Information Retrieval System (PPIRS) using all CAGE/DUNS numbers of Contractors who are part of a partnership or joint venture identified in the offeror’s proposal, inquiries of owner representative(s), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), and any other known sources not provided by the offeror.
The Government will consider the currency and relevance of the information, the source of the information, context of the data, and general trends in the Contractor’s performance. This evaluation is separate and distinct from the Contracting Officer’s responsibility determination. The assessment of the offeror’s past performance will be used as a means of evaluating the offeror’s probability to successfully meet the requirements of the RFP.
Offerors lacking relevant past performance history will not be evaluated favorably or unfavorably in past performance and will receive an Unknown Confidence rating.
Factor 4 – Technical Approach:
(a) Solicitation Submittal Requirements:
Provide a brief narrative (page limitation: one page (in 12-point Times New Roman font) for brief narrative) describing the offeror’s method to manage the project to achieve design and construction design objectives as described in the RFP.
