About this file
This document contains a request for information log related to a construction project to replace operating room flooring at the Greater Los Angeles VA Health Care System facility.
The log details sixteen questions received from contractors regarding clarification of specifications, schedules, and requirements. Key clarifications provided include allowing a single individual to fulfill multiple quality control roles, confirming certain shelving units and concrete pads are to be removed, and outlining personal protective equipment and infection control requirements for working in operating room suites. Infection control barriers are required to isolate work areas from the remainder of each operating room suite. The document provides relevant details to inform contractor questions on scope and schedule requirements for the project.

Will the government require a transition period between each phase? Assuming there will be a transition phase, how many calendar days will the government require between each phase?
01 00 00 General Requirements G. Phasing:
TWO (2) days between each phase are allowed for users to move equipment out of the next phase. This is subject to Hospital Operational Requirements
002. 4 MAR 2020
On the plans there are multiple items that say that they will be relocated by others, will the government confirm that if a note says it will be done by others, the other is someone other than the contractor?
Yes – the word “others” is someone other than the contractor?
003. 4 MAR 2020
The project duration does not appear to be enough to complete the work considering the specification require the contractor to acquire PIV badges for all employees and the specifications say that the contractor shall allow 4‐6 weeks for badges to be issued; in addition, the submittal process will take at least 30 days after NTP
1.3 STATEMENT OF BID ITEM(S)
A. ITEM I, Base Bid: GENERAL CONSTRUCTION: Replace Operating Room Suites Flooring in Building 500, 5th floor, North wing as indicated on the drawings, specifications and any additional document that may have been provided. Work includes demolition, disposal, general construction, alteration and certain other items. Construction Duration time: 76 Calendar days (page 4) and it will take at least 4 weeks after the submittals are approved to get the materials before we can even start construction. It is safe to say that 60 of the 76 days will be consumed before we even start demolition. Assuming the contractor can work uninterrupted, will the government extend the period of performance to 230 calendar days? This will allow for 60 days to get badges, submittals and materials procured, then 30 days of construction for each phase and 14 days for government transition between each phase plus a few days for inspections and punch list. If the contractor may be interrupted by normal operations or must work limited hours or days, the contract duration could extend out by at least another 30 days.
The construction duration period noted is strictly for construction on the site and must be strictly adhered
to. 30 days for submittals and order/delivery of materials after NTP before on‐site construction is added.
004. 4 MAR 2020
The plans and specification talk about options, yet the RFP bid sheet does not provide for any options. Will the government please provide a bid sheet that list each of the options?
Provide all your bids (base, options, etc) in the same areas you would place a regular bid. These areas are section 17 on the SF 1442, Section A.2 “Price/Cost Schedule” and/or on a separate sheet to be delivered with your bid. There is not a separate “bid sheet” for options.
005. 4 MAR 2020
Note D‐214 on sheet A1.51 states to “remove/replace or reuse the existing Mop sink”, this is in 2 HAC closets. For bidding purposes will the VA please determine prior to the bid due date if the mop sinks need to be replaced as there is a cost involved in replacing the mop sink versus just reusing?
Clarification:
1. Delete the word “or reuse”
2. The VA desires to replace the mop sinks versus just reusing
3. Omit room 5023 from scope
006. 4 MAR 2020
Project Schedule Specification 01 32 16.15‐ part 1.3 Contractor’s Consultant. This specification seems to indicate that the VA expects the contractor to hire an outside consultant to prepare the project schedule. Can the contractor utilize a current member of the project team (Project Manager) to prepare and update the project schedule instead of hiring a consultant to prepare the schedule?
Yes ‐ the contractor can utilize a current member of the project team to prepare and update the project schedule.
007. 4 MAR 2020
Quality Controls Specification Section 01 45 00 part 3.2.B.1 indicates that the CQC system manager reports to the project superintendent. This paragraph seems to indicate that a separate QC Manager that is not the project superintendent is required. Will the VA conform that the role of Project Superintendent and QC Manager must be fulfilled by two separate individuals, or if qualified may the Project Superintendent fulfill the role of Superintendent, QC Manager & SSHO?
A single person can cover more than one area provided that the single person is qualified to perform QC activities in each designated and that workload allows. (page 84)
008. 4 MAR 2020
Section 01 45 00 – Quality Control, Sub‐Part 3.4(C) requires additional personnel above the CQC personnel specified elsewhere. These include Architects and Engineers or Construction Professional with 5‐years’ experience and a TAB Specialist. These additional personnel positions are required to be
A single person can cover more than one area provided that the single person is qualified to perform QC activities in each designated and that workload allows. (page 84).
on the job site whenever work in their area of expertise is being performed. Will the VA clarify whether all these costly requirements will be enforced for this project? Also please confirm that there is no Test and Balance requirement for this project?
No ‐ Test and Balance is not required for this project. However, upon completion, restore HVAC system where work was performed. (page 55). And, Remove or Isolate HVAC system in area where work is being done to prevent contamination of duct system. (page 56)
009. 4 MAR 2020
Specification Section 09 05 16 Subsurface Preparation for Floor Finishes part 2.1 &
3.3 include instructions for Moisture Remediation Coating. Until the proper moisture tests can be performed after the NTP, it is unknown the quantity if any of moisture remediation coating that will be required. Will the VA treat Moisture Remediation Coatings as an unforeseen condition if the existing concrete floor does not meet moisture requirements?
No – The OR Room floor is neither newly poured nor on grade. We do not expect to encounter any moisture in the floor. However, if moisture is encountered during construction please follow the specifications section for moisture remediation.
3.3 MOISTURE REMEDIATION
COATING:
A. Where results of relative humidity testing (ASTM F2170) exceed the requirements of the specified flooring manufacturer apply remedial coating as specified to correct excessive moisture condition.
B. Prior to remedial floor coating installation mechanically prepare the concrete surface to provide a concrete surface profile in accordance with ASTM D4259.
C. Mix and apply moisture remediation coating in accordance with manufacturer’s instructions.
(page 106)
0010. 4 MAR 2020
Section 09 65 16 Resilient Sheet Flooring part 3.8.F states that the contractor is to apply Polish to vinyl flooring. Please confirm that this is a contractor requirement and if so, applies to vinyl only and not the Nora Rubber Flooring? Manufacturer does not recommend polishing rubber flooring.
Contractor to follow Manufactures Recommendations for NEW Floor.

0011. 4 MAR 2020
No mechanical plans were included in the documents issued, is there a building exhaust in every phase where the negative air machine can be connected?
Yes
0012. 4 MAR 2020
What kind of protection will the contractor and subcontractors be required to where upon entering the Surgical Suite, such as scrubs/Tyvek suit? Will the contractor have to provide their own Tyvek suit/scrubs including shoe covers, hair covers, latex gloves and surgical masks or will that all be provided by the VA?
Follow OR Room and Infection Control Guidelines (1.11 INFECTION CONTROL) (page 54)
PPE (1.10 PERSONAL PROTECTIVE
EQUIPMENT) is the responsibility of the Contractor. (page 53)
During each phase of construction, the part under construction must be isolated from the remaining OR suite with infection control barriers.
Contractor or his workers must not enter any part of OR suite that has not been made available to them for construction. The path that the construction worker will take to enter the construction site has been shown on the drawings. All infection control requirements to be followed for movements through b500”.
0013. 4 MAR 2020
Section 01 00 00 General Requirements part 1.6.G Phasing has the project hours as 6am to 3pm weekdays but also states that the VA expects the contractor to work weekends and after hours. Please confirm that the VA is expecting work to occur 7 days a week with a second shift in the evenings?
Contractor’s normal working hours shall be from 6.00 am to 3.00 pm.
However, the contractor is expected to work over the weekends and during alternate working hours after
3.00 pm to expedite the work. All noise generating operations must be scheduled after hours or on weekends with prior coordination with COR, to allow emergency surgeries to be scheduled in rooms as far as possible from the area of noise. (page 10)
VA will accept contractor’s option to work over the weekends and after hours with prior notification to COR, in addition to normal business hours to meet schedule. Construction schedule must be strictly adhered to.
0014. 4 MAR 2020
Section 01 35 26 Safety Requirements part 1.11.4.B Class IV infection control requirements state that ICRA is not to be removed until the VA Environmental Services Department has cleaned the area. Please confirm that by this statement the contractor is not responsible for “Critical Cleaning” but is only responsible for normal construction cleaning.
Yes – Contractor is responsible for daily construction cleaning in accordance with the Specs.
Terminal Cleaning is performed by VA Environmental Services Department (page 56)

This facility is known in the past to have asbestos containing materials in certain assemblies. Neither the Drawings nor the Specifications mention any hazardous materials or abatement.
‐Does the VA expect any asbestos containing materials or lead‐based paint to be encountered, demolished, or disturbed during this project?
‐Will the VA provide a current Hazardous Material Survey for the areas included in the scope of work for this project?
‐Will the VA require the Contractor to perform a Hazardous Material Survey for this project?
No – the VA doesn’t expect any asbestos containing materials or lead‐based paint to be encountered, demolished, or disturbed during this project.
No –
‐If a Survey is required, will the VA add 21 days to the Period of Performance to allow for the testing and the return of results?
‐If asbestos or lead‐based paint is encountered during this project, will the VA consider this discovery as an “unforeseen condition”?
Yes – this is covered in the FAR 48 CFR § 52.236‐2 ‐ Differing Site Conditions.
0016. 10 MAR 2020
In specification section 01 00 00 Par 1.2.A is says to “patch & repair, with paint to match existing of any part of the walls damaged as a result of flooring/corner guard replacement operations” and then in the next sentence it says: “Painting of all the walls is included in the scope of work. For paint in Operating rooms and Sterile rooms see Schedule of Finishes.”.
The plans clearly say to repair, patch, prep and paint the doors and door frames, but don’t say to paint all the walls.
Will the Government please clearly state if we are to paint all of the walls within the area we are working or are we to only touch up the walls where we remove the recessed corner guards and patch them with drywall so as to allow for the installation of the surface mounted SS corner guards?
If we are just patch, are we expected to paint from corner to corner or just blend in the paint as best as we can?
Also included is replacement of corner guards as shown on drawings and patch & repair, with paint to match existing of any part of the walls damaged as a result of flooring/ corner guard replacement operations.
Painting of all the walls is included in the scope of work. For paint in Operating rooms and Sterile rooms see Schedule of Finishes. (page 3)
Patch &repair and paint to match existing if wall painting is deducted per deduct alternate.

On sheet A1.51 demolition note D‐207 says the existing fixed shelves are to be temporarily removed and reinstalled. The note is pointing to one of the three section of the fixed “DSI” shelving units, yet sheet A3r.6 says that these same shelving units are to be uninstalled and disposed of.
Is it the governments intent to remove and dispose of all of these “DSI” fixed shelves or is it the governments intent to remove them and reinstall them? If they are to be removed and disposed of, are we to assume that we are to remove the concrete housekeeping pad that they are sitting on? If they are to be removed and installed are, we to remove and reinstall all the shelves or just the one that the note is pointing to?
Clarification:
Contractor to uninstall and dispose of shelving units. Please follow
SECTION 02 41 00
DEMOLITION for disposal
The existing concrete pads to remain.
