The purpose of this amendment is to provide responses to questions that have been submitted.
The receipt of offers due date has changed from 8/14/19 to 8/19/19 at 1700 EST.

This Amendment references some of the question numbers referenced in Amendment 1.
1: Question: The descriptive paragraph talks about a not to exceed quantity of 245 containers, yet the table lists out ~264 containers across the various locations. This does not make sense – are we to bid a maximum of 245 containers despite the larger number in the detail?
A: A recount has been made and there are 270 total containers for SAVAHCS for the Main Campus and all CBOCs, this includes the 6 large containers to that are considered part of the total count (considered the not to exceed amount and are spare bins not on property).
1 Q: Also, for the main campus there needs to be a break out of how many large and how many small containers, and the size of the “wheeled totes” should be listed (64- or 95-gallon?)
A: For the Main Campus there are 223 large, 6 small, 10 wheeled totes-95-gallon, additionally 6 large (considered the not to exceed amount and are spare bins not on property).

1Q: Prescott: The descriptive paragraph quantity appears to match the table quantities. The size for small containers “27-30” in size and will fit under a desk” – they should know that a container that size will rarely fit under a desk since most desk writing surfaces measure between 29-30” in height, leaving the clearance under the desk at about 26-27”.
A: The size of bin needs to fit under the desk and should be between a 24” to 27” height to be considered acceptable.

A: NAVAHCS – Prescott the not to exceed amount is a total 190 of these, 20 are considered spare containers. The 20 spare containers breakdown using - 6 large that are located at the main campus in Prescott, 6 wooden executive and 8 small, are also considered spares but are not held on site those will be requested by the customer to the contractor when there is a need. All 20 spares are considered part of the monthly cost regardless of use.

1 Q: PVAHCS -The descriptive paragraph talks about a not to exceed quantity of 277 containers, yet the table lists out ~391 containers across the various locations. Again, this does not make any sense and the two total quantities need to be reconciled before an offer can be made.
A: Quantities have been revised in the table to reflect 390 total containers, the not to exceed should be 390, of these 6 small size containers are considered spare and will be located at the Phoenix Main Campus and are considered part of the monthly cost regardless of use.
2. Question:
Q&A #8 – The response that “PVAHCS uses only nylon bags and cardboard containers roughly half of each” doesn’t make sense. If this is true, why are there container sizes listed for Phoenix? Also, since bags and boxes are not secure, how can this location be compliant with VA and HIPPA rules regarding the handling and storage of sensitive information?
A: Bags and Boxes are secured. The Nylon bags are inside the secured bin, the bag is removed and emptied and returned to the secured bin. Bins have a locking system in front and cannot be accessed without a special key. The same would apply to the cardboard containers these are secured and placed inside of a locked container. Executive bins have nylon bags in them, all small containers have card board box in them.
NOTE: It is possible that this site is looking at the bins and identifying the contents that the current containers have which are; bags and boxes they don’t appear to be separate items but part of the existing containers (Executive and Small) these containers are not different from any other location therefore this question may no longer apply. The bag and box are part of the container.
3. Question:
Q&A #9 – The Phoenix and Tucson counts in the table are now greater than the not to exceed number so this doesn’t make sense. If the counts in the table get revised, then these two locations will need to re-answer this question in a manner similar to the Prescott response, which makes complete sense.
A: SAVAHCS Not to exceed a total number of 270 bins with the following anticipated breakdown for pricing purposes: 6 – Large these are considered spare containers are not held on site, the customer will contact the contractor and ask for them as needed, these containers are in the total quantity for the monthly cost.
SAVAHCS -Tucson Total Main Campus & CBOCs/RHCCCs combined –17 executive, 14 small, 10 wheeled 95-gallon totes, 229 executive, for a total of 270 containers for sensitive documents.
· Large: approximately 35”h x 15”d x 21”w with a removeable bag or bin inside.
· Small: approximately 20”h x 15”d x 20”w. with a removeable bag or bin inside.
· Executive containers = approximately 36”h x 15”d x 21”w with a removeable bag or bin inside.
· Wheeled 95-gallon totes.
4. Question:
Q&A #10 – None of the locations actually responded to the question, which is about pricing. Phoenix and Tucson responses just don’t make sense. Prescott just repeated the answer to question #9, which is about bin type where question #10 is about how to price the bins that aren’t yet assigned to a location. We are assuming that we should price them for weekly servicing but need confirmation.
A: The Phrase “Not to exceed” is used as a number of containers that the sites will not exceed as an amount to be used to reference the list of locations, quantities and sizes provided in each table. There are additional amounts of containers that are considered “spares” these are used to replace either broken bins or due to an increased need. Because it is not possible to know when these two episodes will occur each site either asks for the spare bins to be on location at the main facility or will call and ask them to be delivered the request for a delivery allows for the bin sizes listed to always be available and delivered in a 24-hour period if they are not located on site. This will prevent delays due to funding, or contract modifications. Each location will be paying for those “spares” regardless of their use to ensure their availability at all times. The bins are not to exceed that amount listed in each table as “total” if at anytime the total amount of bin changes this would require additional funding and a contract modification.
SAVAHCS will have an additional 6 large bins for the main facility only. The 6 large bins are considered part of the monthly cost but are not held on site until the need is requested.
5. Question:
Q&A #11 – Phoenix talks about five 64-gallon containers but none are listed in the table.
A: This was an error and is removed as a requirement size, the facility no longer uses this size.
6. Question:
Q&A #12 – Tucson did not answer the question, what is posted seems to be the answer for #14.
(Original question -12 Q: P. 6, 4., Table. Please confirm that for recurring services the quantity of bins serviced weekly will be fixed at a set quantity per location per this table, and invoicing shall be “by the bin” for recurring services. If this is not an accurate assumption, please explain how the Agency anticipates being invoiced, specifically if the invoicing is to be of an indefinite quantity even though a firm fixed price contract is to be awarded.)
A: SAVAHCS: bins are anticipated to be a fixed quantity as of award, amounts have been revised and are quantities that are billed monthly.
*NOTE: All locations have a revised table listing the total quantity and size and what is considered billable per month on the schedule.
7. Question:
Q&A #18 – If Phoenix is using any large rolling containers, which from earlier answers it appears that they are, this question will need to be answered since a dead-bolt lock cannot be installed on 64- or 95-gallon containers.
A: Phoenix is using Seven 95-gallon containers, these containers have a secure lock on the top and not accessible without a special key.
