CONTINUATION PAGE

Question 1:The FBO website has a response date of June 1, 2018, but box 8 on Standard Form 1449 had an offer due date/local time of 06-07-2018, 10:00. When are responses due by?
Answer 1:Responses are due by June 7, 2018, 10:00 am PST.
Q2:Will we be responsible for specialty systems if we recommend thermal eradication or shock oxidization?
A2:No.
Q3:What is the follow-up testing period after a result over 200 CFU/L?
A3:Refer to Attachment “C”. Paragraph on action levels describes course of action for varying levels of CFU/L results.
Q4:During collection of bacterial samples, other testing parameters are requested. Is direct reading instrumentation, coulometric evaluation or laboratory analysis preferred?
A4:Direct reading of attributes such as water temperature and pH are acceptable at the time of sample draw. Laboratory analysis is required for detailed sample testing.
Q5:Do any of the facilities to be tested use bromination or ozonation as biocidal agents?
A5:No.
Q6:Are historic chlorine, monochloramine and chlorine dioxide data and their comparative efficacy by concentration available? Appendix D does not provide the data for oxidant concentration, only biologic result.
A6:Oxidant concentrations vary due to a number of factors.
Q7:Is it required to record flushing periods in oxidant concentration measure?
A7:Flushing, thermal eradication and/or hyper-chlorination should be noted for all retest samples.
Q8:Will the facility or contractor be responsible for collecting Dental Samples?
A8:Contractor.
Q9:Will the government consider weighing the experience/past performance of the SDVOSB Primes greater than the experience/past performance of non-SDVOSB Subcontractors?
A9:Refer to 52.212-2 Evaluation – Commercial Items; and Basis for Award; on pages 36-37 regarding how evaluations will be conducted.
Q10:Can you provide a breakdown of each site's historical dental and cooling tower samples, similar to Attachment D?
A10:Cooling tower samples totaled approximately 100 for period of 10/1/2015 – 9/30/2016 and 160 for 10/1/2016 – 9/30/2017. For Dental water samples, use an approximate count of 3000 samples for the purpose of pricing.
Q11:Should a separate technical response be provided?
A11:Refer to E.1 Instructions to Offerors, pages 32-33 of the solicitation on what is required to be submitted.
Q12:Will the government require the SDVOSB Prime to have the CDC Elite Lab certification? If not, will the government consider ranking SDVOSB Primes with CDC Elite certification higher than SDVOSB Primes who have to rely on subcontractors with the CDC Elite certifications?
A12:Laboratory performing sample analysis must be CDC Elite Certified. Refer to 52.212-2 Evaluation – Commercial Items; and Basis for Award; on pages 36-37 regarding how evaluations will be conducted.
Q13:Can you clarify what is meant by Number 5 "Current Schedule" on Attachment F?
A13:Current Schedule, or period of performance, of any ongoing, or completed [within the past three years], contract that offerors are seeking to include as relevant experience.
Q14:Is NELAP an acceptable laboratory accreditation for HPC analysis?
A14:No.
Q15:The CLIN 0001 description sounds like you have about 3,500 sample points annually and therefore 25% (875+/-) will be tested quarterly; thereby, each location will be tested only once per year, not 4 times per year. Is this what you intended?
A15:No. The 3,500 samples represents the estimated total number of tests expected annually for VISN 22. Sample points are not always the same and vary from quarter to quarter.
Q16:For CLINs 0002 (and perhaps 0003, 0004) we recommend splitting the CLIN into A and B and adding a per visit, per site trip charge. This is counter-intuitive but it will save you money. Bidders do not know if they are sending a tech to collect 1 follow-up sample or 200 samples. Sending a tech will cost a few hundred dollars so they will assume the worst case (only a few samples) and thus your unit cost will be higher than necessary.
A16:CLINs 0002, 0003 and 0004 shall remain as noted in the solicitation. Coordination of samples for potable water and Dental Clinics can be coordinated to coincide for maximum efficiency. Follow-up sampling can be performed by local VA staff and shipped to laboratories for analysis.
Q17:For CLINs 0003, 0004, and 0005 verify you only want containers and not a tech to come to the site.
A17:Correct.
Q18:For CLIN 0004 you want ‘analysis,’ does this mean samples will be mailed in by the customer or a tech needs to come to the site to take samples?
A18:Samples are sent to the laboratory by VA.
Q19:Attachment A shows Est Annual QTY for Quarterly collection, can the government confirm that the amounts shown under Est Annual QTY are the annual quantities and not quarterly amounts?
A19:Quantities noted in Attachment A of the Solicitation represent the estimated total, annual numbers, collectively, for all VISN 22 facilities.
Q20:RFP Section B.3 Price/Cost Schedule shows Unit as each and Unit Price. Can the government confirm that Offerors should provide a per sample price?
A20:Offerors should provide a unit price based on the estimated total annual number of samples, containers, analysis, etc.
Q21:RFP Section C.4 52.216-22 shows this is an Indefinite Quantity contract. Can the government provide historical quarterly quantities collected from each location?
A21:Historical annual testing is provided, by facility, in Attachment D of the Solicitation.
Q22:Attachment E – Do all locations have Dental Clinics? If not, which locations will require this service? Can the government provide historical quarterly dental water line quantities collected from each location?
A22:Use the estimated number of dental water samples noted in the Solicitation for pricing purposes. All facilities have Dental Clinics on the main campus. The following facilities also have Dental Services in the remote locations noted below:

Albuquerque Northwest Metro CBOC 1760 Grand Blvd SE Rio Rancho, NM 87124

San Diego Oceanside CBOC 1300 Rancho del Oro Drive Oceanside, CA 92056

Q23:RFP Page 7 state “The contractor shall collect the dental waterline samples at the VA Medical Center’s Dental Offices throughout VISN 22.” On Page 6 it states “the contractor will furnish sterile bottles…” Attachment A CLIN 0003 shows 300 Sample collection and samples analyzed and CLIN 0004 shows 60 sample containers, shipping containers, and analysis. Can the government clarify if the dental water line samples on CLIN 0003 and CLIN 0004 will be collected by the VA or if the contractor will be collecting the samples? If the VA is not collecting the samples can the government clarify that the Contractor is responsible for collecting the 300 samples on CLIN 0003 AND the 60 samples on CLIN 0004?
A23:Samples in CLIN 0003 can be coordinated by offeror to coincide with potable water sample collection. CLIN 0004 is for containers and analysis with VA personnel drawing samples.
Q24:RFP Page 6 Section 2 refers to “promptly transported to the facilities included in Attachment F.” An Attachment F was not provided. Can the government provide Attachment F? Or can the government confirm if the facility list referenced is shown in Attachment E?
A24:This should read Attachment E. Attachment F is for offerors to list relevant experience.

Applies to Questions 25 through 28: On page 4 of 51 of the solicitation it is stated (in italics) that: “Outlets” are the points in the water distribution system where the end user comes in contact with the water (e.g., faucet, showers, and ice machines); no less than 35% of total samples shall be drawn prior to the thermostatic mixing valve (e.g. hot/cold water supply lines) closest to the outlet.

Q25:Can the VA provide a diagram indicating where in the water lines the sampling is to occur when performed prior to the thermostatic mixing valve?
A25:Sampling at ports upstream of the mixing valve body.
Q26:Will the contractor be required to separate pipes or perform plumbing in order to obtain a water sample prior to the thermostatic mixing valve?
A26:No.
Q27:Can the VA provide a description of specific tasks that the contractor will be required in order to obtain a water sample prior to the thermostatic mixing valve?
A27:Sampling at ports upstream of the mixing valve body.
Q28:We are primarily concerned that the time required to obtain these samples will be much longer than that required to obtain sample from an outlet. If the contractor is not required to perform plumbing tasks, approximately how much time is required to obtain a water sample prior to the thermostatic mixing valve will the VA provide access in a timely and efficient manner?
A28:VA will provide necessary access to ensure timely collection of water samples.
Q29:Attachment A Item 0003 states that “Supply of water sampling containers, information on proper collection of samples for total colony forming units of heterotrophic water bacteria per milliliter of water count, and analysis of water samples for the VA Medical Center Dental Clinic to ensure compliance with Safe Drinking Water Act”. In contrast, Section 2 (a) paragraph 4 under Statement of Work States that “The contractor shall collect the dental waterline samples at the VA Medical Center’s Dental Offices throughout VISN 22; the contractor will coordinate sampling times with the Point of Contract at each of the facilities and shall notify the Point of Contact upon arrival. Each dental waterline will be tested separately. All hand pieces, air-water syringe tips, and any other equipment reprocessed by SPS before collection samples from the waterlines. The air-water syringe itself, if not changed between patients, shall remain on the line while collecting the sample”. For Item 0003 is the Contractor responsible for both the supply of water bottles and the collection of the samples or just the supply of water bottles?
A29:Contractor to supply water bottles as noted within Attachment A, Item 0003.
Q30:Statement of Work Section 1 (a) 4th paragraph, page 4 of 51 states the “Water samples must be taken from outlets on the hot water distribution system and outlets on the cold-water distribution system from each building for each quarterly testing cycle. NOTE: “Outlets” are the points in the water distribution system where the end user comes in contact with the water (e.g., faucet, showers, and ice machines); no less than 35% of total samples shall be drawn prior to the thermostatic mixing valve (e.g. hot/cold water supply lines) closest to the outlet. All water samples will be taken at first “draw” to catch the water that comes out immediately from the outlet and sample quantity will be no less than 250 ml”. How will contractor access sampling points Prior to the thermostatic mixing values?
A30:Thermostatic mixing valves have testing ports upstream of the mixing body.
Q31:Contractor is required to test all water collected for LD for pH, temperature and biocides at the source. There are several different ways to do this testing and it would be useful to know if any specific methods are required. The easiest (least equipment and least equipment costs) would be testing using pH strips, an infrared thermometer and a test kit in a box for biocides. Biocide testing may be for free chlorine or chlorine dioxide (or something else). It would be useful to know if specific test methods are required and or the types of biocides that we may encounter in order to best price the work. Please clarify.
A31:At the time of sample draw, contractor will measure and record biocide level, pH and water temperature. Actual field measurement of these items are left to the offeror as individual means and methods. Formal analysis of the sample must be performed at a CDC-Elite Laboratory.
Q32:Item 0005 “Additional supply of water sampling containers beyond the 20% non-routine for sampling as a result of a report of over 200 colony forming units of heterotrophic water bacteria per milliliter of water count or for other reasons requested by the COR and/or Site POC for any site. The cost of this sampling will be based on historical pricing set per sampling location and per samples required. Contract Period: Base POP Begin: 07-01-2018 POP End: 06-30-2019 As required and requested. Sample containers, shipping containers, and analysis”. There is no historical information regarding potential number of samples and collection frequency required under this cost item nor is it specified that Contractor will be responsible to collect the additional samples. Please clarify potential number of samples to be priced under Item 0005 and if Contractor will be responsible for the collection of additional samples.
Q32:For the purposes of pricing additional water sapling containers noted in Attachment A, Item Number 0005, offerors shall include pricing for a total of 25 containers. Similarly, offerors shall price Item 0005 assuming VA personnel will collect samples and ship to identified CDC Elite Laboratory for analysis.
Q33:Item 0003: Are the dental clinic’s involved under this work item located on the same medical campus defined in attachment 006 of RFP? If not please provide addresses of dental clinics covered under work item 0003.
A33:See answer to Question 22.
Q34:Will all work items 0001 through 0007 be awarded under one (1) contract?
A34:See FAR 52.216-18 – Ordering (page 19); FAR 52.216-19 – Order Limitations (pages 19-20); and FAR 52.216-22 – Indefinite Quantity (page 20).
Q35:Please confirm that the contractor will recover all of the dental line samples or a portion of them? The SOW language seems to suggest both.
A35:See answer to Question 23.
Q36:Can you please provide the times in which these samples must be taken by the contractor? Any requirements for after hours or before hours to pull these samples?
A36:Normal business hours, typically 8:00 am – 4:30 pm, there may be minor variations between facilities.
Q37:Our past experience would indicate that the amount of dental samples listed in the solicitation is significantly lower than what is required. Please confirm the dental samples amount?
A37:Sample quantities listed in the solicitation are estimated based on past sampling/testing.
Q38:The SOW requires 2 samples to be pulled from the cooling towers when the VA requires quarterly. Please confirm.
A38:CLIN 0006 specifies sampling/testing frequency and estimated quantity of cooling towers.
