About this file
This is a past performance survey form designed to evaluate contractors bidding on intraoperative neuromonitoring (IONM) services for the Department of Veterans Affairs. The survey requests feedback from previous clients who have used the contractor's services to support source selection decisions for solicitation 36C25726Q0274.
The survey covers neuromonitoring equipment and certified technician services to be provided at the South Texas Veterans Health Care System (STVHCS) in San Antonio, Texas. Services include intraoperative neurophysiological monitoring and deep brain stimulation procedures, requiring an onsite IONM technologist and remote oversight by a board-certified neurophysiologist. Evaluators are instructed to rate contractors on six key criteria using a five-point scale: compliance with requirements and expectations, knowledge and expertise in neuromonitoring services, personnel accessibility and timeliness of response, scheduling process efficiency, patient privacy protection, and likelihood of future contract award. The survey requests information on average monthly service volume and period of performance. Completed surveys must be submitted by August 30, 2026 at 2:00 PM CT to carolyn.reneau@va.gov. All reference information is classified as source selection sensitive, and evaluator signatures are required for survey validity.
