About this file
This is an Amendment to a Solicitation/Modification of Contract (SF 30) for Intraoperative Neuromonitoring (IONM) Services at South Texas Veterans Health Care System (STVHCS). The amendment extends the response deadline to September 4, 2026, at 2:00 PM CDT, adds a Past Performance Survey (Attachment D.8) as a required submission, and provides clarifying answers to twelve contractor questions regarding the RFQ.
The solicitation is a 100% Service-Disabled Veteran-Owned Small Business (SDVOSB) set-aside issued by the Department of Veterans Affairs Veterans Health Administration, VHA RPO West. The contract requires an on-site IONM Technologist and remote oversight by a Board-Certified Neurophysiologist to support an estimated 245 cases annually, with pricing submitted as an all-inclusive rate per case (unit of issue: EA). Historical case data shows 198 cases in FY2024 and 246 cases in FY2025. The incumbent contractor is Risen Video Production, LLC, under contract 36C25722D0026, expiring January 16, 2027. Key requirements include 24/7/365 availability with a 60-minute emergency response capability, though historical data indicates zero cases occurred outside standard weekday hours (06:30-17:00, Monday-Friday) in the past 12 months. Prospective offerors must be verified as SDVOSB by the Small Business Administration at both proposal submission and award. Completed Past Performance Surveys must be submitted to the Contract Specialist, Carolyn Reneau, at Carolyn.Reneau@va.gov by the solicitation closing date.

Questions and Answers – 36C25726Q0274 Neuromonitoring Services – South Texas VA Question 1: LIN 0001 estimates 240 IONM cases per year. Can STVHCS provide the actual historical IONM case counts for FY2024, FY2025, and FY2026 to date?
Answer 1: Actual cases performed in 2024 – 198; 2025 – 246; Jan 2026 – June 2026 - 87 Question 2: Are IONM services currently being provided at STVHCS? If so, is it under a contract or purchase order, and can the Government provide the contract number and expiration date?
Answer 2: Current contract is 36C25722D0026 and is awarded to Risen Video Production, LLC. Contract end date is January 16, 2027.
Question 3: Can STVHCS provide an approximate case mix for the 245 annual cases by service and procedure type, e.g., instrumented spine (ACDF, PLIF/TLIF, posterior cervical), non-instrumented decompression, cranial/tumor, carotid endarterectomy, ENT/thyroid, peripheral nerve?
Answer 3: STVHCS cannot guarantee an approximate case mix for the 245 annual cases, as this may fluctuate due to factors such as surgical instrumentation, patient health, and case cancellations. The current caseload is trending toward the projected 245 annual cases, particularly with the addition of new clinical staff. Several specialties, including Neurology, Orthopedics, Vascular, and ENT, currently request IONM services; as of now, no other services have submitted requests.
Question 4: How many surgeons currently request IONM, and by specialty? Are any additions or departures anticipated during the period of performance that would change the 245-case estimate?
Answer 4: Multiple surgeons across specialties—including Neurology, Orthopedics, Vascular, and ENT—currently request IONM. At this time, no specific changes in requesting providers are anticipated that would materially impact the estimated 245 annual cases. However, caseload may vary with staffing changes and new staff onboarding.
Question 5: What is the average and maximum case length for IONM cases?
Answer 5: Case length for IONM varies by patient needs and procedure. Both the average and maximum case duration are variable and determined on a case-by-case basis, based on what is required to safely and effectively complete each procedure.
Question 6: How frequently do two or more IONM cases run concurrently, and what is the maximum number of simultaneous IONM cases STVHCS expects the Contractor to cover? Are IONM cases concentrated on specific block days?
Answer 6: It is not frequent, but two or more IONM cases can be scheduled concurrently by different specialties on the same day. With proper notification, the Contractor is expected to provide coverage for the scheduled and agreed-upon cases. There are no specific block days designated for IONM cases.
Question 7: PWS §3.1 requires 24/7/365 availability with a 60-minute emergency response (§3.1.1). Over the last 12 months, how many IONM cases occurred outside 06:30–17:00 Monday–Friday, including nights, weekends, and holidays?
Answer 7: In the past 12 months, there have been zero IONM cases conducted outside of standard weekday hours (06:30–17:00, Monday–Friday). Across the entire contract to date, there have been a total of four cases requiring coverage outside these hours.
Question 8: Unit of Issue is “EA.” Is EA = to a case or a unit of time, e.g., an hour?
Answer 8: “EA” refers to each case. Price is to be submitted as an all-inclusive rate per case.
“Intraoperative Neuromonitoring (IONM) - Service to include an on-site IONM Technologist and remote oversight by a Board-Certified Neurophysiologist. Services shall include all resources, labor, travel, transportation, parts, materials, equipment, and IT connectivity necessary to meet the PWS requirements. “ Question 9: For Factor 2 – Past Performance: will a Past Performance Questionnaire template be provided?
Answer 9: Yes. See new Attachment D.8 – Past Performance Survey, Solicitation 36C25726Q0274. Please forward this survey to any references you would like the Government to consider. Completed surveys must be submitted to the Contract Specialist at Carolyn.Reneau@va.gov no later than the solicitation closing date.
Question 10: Please advise if you may potentially allow companies other than (SDVOSB) to compete.
Answer 10: This is a 100% SDVOSB set-aside. All offerors must be verified as an SDVOSB at time of proposal and time of award by the Small Business Administration (SBA) as an SDVOSB entity.
Question 11: We would like to know if this is a brand new contract OR if there is (was) an incumbent performing these services.
Answer 11: See Answer #2 Question 12: If not brand new, could you please provide the current / previous contract number?
Answer 12: See Answer #2.
