About this file
This is a Past Performance Questionnaire (PPQ) template for evaluating contractors' prior work performance on federal contracts. The questionnaire is specifically for solicitation #36C25725R0010 issued by the VA Network Contracting Office 17 for the Bonham Energy System Design project.
The PPQ contains 20 evaluation criteria covering areas such as project management, technical compliance, quality control, schedule adherence, cost control, and overall performance. Evaluators rate performance using a 5-point scale: Exceptional (E), Satisfactory (S), Marginal (M), Unsatisfactory (U), or Neutral (N). The form requires detailed information about both the contractor and the respondent, including contract numbers, project descriptions, and contact information. Completed questionnaires must be submitted to rhonda.richardson5@va.gov. The template serves an administrative purpose and does not itself specify any products or services to be provided.
