The purpose of this amendment is to update the period of performance on the Statement of Work (SOW).
All changes in this amendment are hereby incorporated into the solicitation.
All other terms and conditions remain the same.
Statement of Work (SOW)
SATELLITE TELEVISION SUBSCRIPTION SERVICE
BACKGROUND
Central Texas Veterans Healthcare System (CTVHCS) Community Based Outpatient Clinics (CBOCs) currently receive Television Subscription Service (equipment, software, channels, programming, labor and technical support).
Satellite Television Subscription Service is provided for all stakeholders (patients, visitors, staff, etc.) at the CTVHCS CBOCs.
SCOPE OF WORK
Contractor shall provide Labor, Parts, Materials, Equipment, Hardware, Software, Programming and Technical Support to ensure Satellite Television Subscription Service connectivity to all televisions (TVs) within the CTVHCS CBOCs necessary for Commercial Basic service, including local channels.
• Palestine - 2 TV outlets
• Bryan College Station - 2 TV outlets
• Brownwood - 2 TV outlets
Contractor shall provide standard channels including local channels.
Contractor shall ensure their equipment, channels, hardware and software is compatible with all TVs within CTVHCS CBOCs in Palestine, Brownwood, and College Station.
Contractor shall provide their own internet connection during the installation process of all software and channel programming.
The CTVHCS CBOCs shall not furnish parts and or test equipment for the duration of the contract. It is the responsibility of Contractor to provide the essential equipment and supplies necessary to complete all work relevant to the contract.
Government property shall not be removed from the agency without a signed Property Pass provided via the COR.
Contractor shall always be required to wear Personal Protective Equipment (PPE) during the performance period.
Contractor shall thoroughly clean the direct and indirect areas where work was performed.
Upon completion, Contractor shall contact the Contract Officer’s Representative (COR) to inform of the completion of service.
The contractor will be required to ensure during the entire contract that the services are operational and if issues arise the vendor is required to have a service technician dispatched to troubleshoot and correct the issue.
PERFORMANCE PERIOD
The Period of Performance (POP) shall be a Base Year Period of (1) year with the Governments option to extend the contract for four (4) 12-month option periods.

All work shall be scheduled during normal work hours (7:00 AM to 4:00 PM) Monday – Friday, Central Standard Time (CST), excluding Federal Holidays.
Work shall not impact patient care services.

The President of the United States of America may declare additional days as a Federal holiday, and if so, this holiday shall automatically be added to the list above. When a holiday falls on a Sunday, the following Monday shall be observed as a Federal holiday. When a holiday falls on a Saturday, the preceding Friday shall be observed as a Federal Holiday.
NOTE* Any change in the work schedule must be approved in writing by the VHA Representative. Contractor’s personnel shall be on site and ready for work at the specified time on each day that Contractor is scheduled to perform work.
PERFORMANCE REQUIREMENTS
Contractor shall provide all Labor, Parts, Materials, Equipment, and Supervision required to perform all work.
Contractor shall provide specification and warranty documentation related to all equipment installed and placed at the CTVHCS CBOC.
Contractor shall check-in with COR Representative upon arrival at CTVHCS
CBOC.
Contractor is responsible for repairing damages incurred during performance of this scope of work to include removal/installation or moving materials and tools in and out of the work area.
Contractor must attain the correct type of permit required by VHA Safety Office.
Work cannot commence until the appropriate work permit has been signed off by all parties. The following permits shall apply depending on the scope of work:
Hot Work Permit Above the Ceiling and Penetration Permit Contractor shall thoroughly clean the direct and indirect areas where work was performed.
Upon completion Contractor shall contact the COR upon completion of service. A Final Inspection of services shall be documented.
INVOICE AND PAYMENT
a. Payment to be made monthly, following satisfactory performance by Contractor’s personnel and upon receipt of a properly prepared invoice.
b. Contractor shall be required to invoice through the Tungsten System at http://www.tungsten-network.com/us/en/veterans-affairs/. Additional information regarding Tungsten shall be provided upon award.
c. The contractor shall have current registration in the System for Award Management (SAM) for this solicitation; URL: www.sam.gov.
d. The invoice MUST be itemized to include the following information. Any information listed below, and not provided on an invoice, will render that invoice incomplete and the invoice will be returned for immediate correction.
i. Time period being invoiced for.
ii. Description of services performed.
iii. Unit Cost billed.
iv. Extended amount due.
v. Invoice number, date, and;
vi. Contract and obligation numbers.
