About this file

This document is a Past and Present Performance Questionnaire attachment for a VA construction contract solicitation. The questionnaire is designed to evaluate contractors' previous performance through ratings from previous clients or contract representatives. It contains 20 evaluation criteria rated on a scale of Exceptional (E), Satisfactory (S), Marginal (M), Unsatisfactory (U), or Neutral (N).

The evaluation areas include project management capabilities, compliance with laws and regulations, quality control, problem resolution, schedule management, cost control, and overall performance. The form requires detailed information about both the contractor being evaluated and the respondent providing the evaluation. Completed questionnaires must be submitted to cassandra.king@va.gov by the date specified in the solicitation. The form also includes sections for indicating whether any cure notices were issued and if the evaluator would award another contract to the contractor, along with space for explanatory remarks.
