36C25725Q0197 - NTX Reference Lab Testing Request for Information (Technical Questions)
1. Within document labeled “S02-36C25725Q0197”; under Addendum to FAR 52.212-1
Instructions to Offerors – Commercial Products and Commercial Services; it states to provide pricing using the attached price/cost bid schedule template and to complete Unit Price, OEM Name, Manufacturer Name. In the 36C25725Q0197 Line Items Price/Cost Schedule document, there are several other rows highlighted. Please clarify the following:
a. Which rows are required to be completed in response to the RFQ. The following rows are required to be completed: Column “E” if applicable, Column “O” Unit Price, Column “AN” Manufacturer Part Number (MPN), Column “BZ” Item Part Number and Column “CH” OEM Name.
b. We are required to enter our test order code within the document, should that information be included under Manufacturer Name? No. If no, please clarify what information is needed in that row and where vendors can enter their test code on the document. Please disregard Manufacturer Name (See Solicitation Amendment).
Please enter your test order code within Column “AN” Manufacturer Part Number
(MPN).
See attached CPT Excel spreadsheet: Column E is where either CPT or CPRR and either the FSS Product number or "Open Market" if the item is open market is entered. If FSS, identify the upgrades, beyond the base deliverables, within the FSS contract. For instance, if the base standard analyzer that comes with a NAC CPT is analyzer A but your requestor requires analyzer B, which is an upgrade, insert "Analyzer B" in this comment tab. Analyzer B will be an upgrade in capability and an increase in price.
2. In S02-36C25725Q0197; price/cost schedule, a list of tests is included with minimum and maximum turnaround times indicated. Within the Performance Work Statement; Section B;
item 3 Turnaround times, it states test results shall be received within 24 hours. Please clarify which turnaround times vendors should be referring to in response to this RFQ.
The TATs listed under the price/schedule are what VANTHCS deems appropriate for testing/resulting to occur. Section B states that if TAT exceeds 24hours then a list is required stating what the turn-around time is for each test listed in the PWS.
3. Within the Quality Assurance Surveillance Plan (QASP), for the Minimal unsuccessful performance due to instrument or reagent issues, the indicator mentions “peer group comparison”. Please clarify what this means in this context.
Peer group comparison information is found utilizing the participant summary for each proficiency testing survey report.
4. Would you like us to list our organization's test code in column BZ under “ITEM PART NUMBER”? No. Please enter your test code in Column “AN” Manufacturer Part Number
(MPN).
5. Would you like us to list our organization's test name (item Descp) in column BY under “ITEM
PART NAME”? No. Please use Column “C” Description and/or Column “E” Comment to enter your organization's test name.
6. If no to either of the above questions, what columns would you like us to use to list our organization's test codes/names? Please use Column “AN” Manufacturer Part Number (MPN) to enter your test codes and Column “C” Description and/or Column “E” Comment to enter your organization's test names.
7. Page 101 of the request specifies we complete the columns containing the Unit Price, OEM Name, and Manufacturer, can you please specify the difference between OEM Name and Manufacturer Name, and what information you need listed in these columns? Please disregard Manufacturer Name (See Solicitation Amendment). No difference between OEM Name and Manufacturer Name. Manufacturer Name is not required per Solicitation Amendment.
8. Would you like our organization to list our address in column S “ADDRESS 1” with the corresponding, City, State, zip, and country in columns VWXYZ? No.
9. Vendors shall complete the following tabs/fields: Unit Price, OEM Name, and Manufacturer Name. I am not sure we would put anything in those columns as that would not apply to us. I see Item Part Name and Item Part Number are highlighted in yellow Column BZ and CA would this be where we would put our test name and test number? Please disregard Manufacturer Name (See Solicitation Amendment). Please use Column “AN” Manufacturer Part Number (MPN) to enter your test codes/number. Please use Column “C” Description and/or Column “E” Comment to enter your organization's test name.
