Table 1 contains a summary of suspect materials collected for the Hospital Administration Building

No. 1 that did not contain asbestos.

ACM, as defined by the EPA and the State of Texas, are materials with an asbestos concentration of greater than 1% (>1%). A complete listing of suspect materials, sampled materials, and the analytical results can be found in the following text portions of this report and in the sample results from the laboratory. Pictures of suspect asbestos material sampled are provided in Appendix C. Floor plan diagrams depicting the locations of the samples are provided in Appendix B.

Laboratory analytical data sheets have been reviewed for potential asbestos content within individual layers of a sample for each material. Analysis of an individual layer of a material may exceed 1% while the composite analysis of the material as a whole is below 1%. Laboratory analytical data sheets are provided in Appendix A.

INTRODUCTION

On October 23, 24, 25, 26 and 28th, 2018, ATEI conducted an AHERA survey at the Amarillo VA

Medical Center, Hospital Administration Building No. 1. This facility is located at 6010 West Amarillo

Blvd., Amarillo, Texas. The asbestos survey encompassed all accessible areas of the Hospital

Administration Building No. 1. The asbestos assessment included collecting bulk samples of suspected

ACMs on the inside and outside of the building, and confirming the asbestos content of the materials in

Alpha Terra Engineering, Inc. 2 S180522-263 each homogeneous area. It is important to note that not all areas were sampled during this inspection as some areas were not accessible to the inspector, such as wall interiors. Additionally, some items could not be sampled because it would be destructive to the integrity of component, or the component’s rating.

All samples were collected from discreet locations, avoiding visible damage to finishes and surfaces.

The AHERA accredited and Texas State licensed inspector representing ATEI, Inc. who performed the asbestos survey was Alex Jackson (Texas Asbestos Inspection License #60-2759). His Texas Asbestos licenses were issued by the Texas Department of State Health Services, Austin, Texas. Copies of valid state licenses are presented in Appendix D.

PROPERTY DESCRIPTION

Hospital Administration Building No. 1 located at the referenced site includes:

Hospital Administration Building No. 1, built in 1939, is located directly in the front of 6010 West

Amarillo Blvd. Thomas E. Creek property. Building No. 1 is a five story building, and serves as the VA

Medical Center’s Administrative Offices, Museum, and mixed Patient Care and Doctors’ offices. The building consists of a concrete slab on grade foundation with brick/CMU and mortar construction. The roof system consists of Spanish clay tile style roofing with moisture barrier, and a newly installed PVC membrane roofing system on the flat areas of the roof. The flooring consists of vinyl sheet linoleum, vinyl floor tile and mastic, carpeting in the office and administrative areas, and ceramic tile in some of the restroom areas. Ceilings consist of drop ceiling tile systems throughout. Interior walls are constructed of a mixture of sheetrock and joint compound, and plaster and lathe. The main hallway structures consist of mostly plaster and lathe, while the branching office areas consist of sheetrock and joint compound on metal studs.

ASBESTOS SAMPLING AND ANALYTICAL PROCEDURES

Sampling Procedures

Representative bulk samples of suspect asbestos-containing building materials were randomly collected from each of the homogeneous areas that were considered to have the potential to contain ACMs.

Sampling equipment was decontaminated before and after sample collection to avoid cross-contamination of samples. For the purpose of this survey, areas were inspected and sampled as separate functional areas with separate and distinct homogenous materials. Homogenous material determination was based on the following criteria:

Similar physical characteristics (same color and texture, etc.)

Application (sprayed-on, troweled-on, assembly into a system etc.)

Material function (thermal insulation, floor tile, wallboard system etc.)

The survey is designed to be used as a basis for identifying and quantifying ACMs prior to the removal of

ACMs from the building and prior to any demolition or a major refurbishment. Therefore, the survey does assess the condition of the asbestos, and will note areas of damage or identify locations where additional asbestos debris may be expected to be present. The scope of the survey was confined to areas which were accessible to the inspectors on the date of the survey. The inspectors were provided access to all areas of the building where suspect ACM had the potential to be found, however, the inspectors collected bulk samples only in readily accessible areas. It was not possible to collect samples and inspect all spaces within the building, such as wall interiors.

The bulk samples were only collected during the dates when the inspectors were on-site performing the survey. Evaluation of the condition of the suspect ACMs were performed by the accredited inspector during the inspection.

Polarized Light Microscopy (PLM) Analysis Methodology

Laboratory analytical services were provided by Crisp Analytical, L.L.C., located in Carrollton, Texas, a

National Voluntary Laboratory Accreditation Program (NVLAP) certified laboratory (NVLAP code #

200349-0).

PLM samples were analyzed utilizing the Environmental Protection Agency’s Test Methods: Methods for the determination of Asbestos in Bulk Building Materials (EPA 600/R-93/116, July 1993) and the

McCrone Research Institute’s The Asbestos Particle Atlas as method references. Additional treatment and tests may be required to accurately define composition (i.e. ashing, extraction, acetone treatment, and

TEM).

Analysis was performed by using the bulk sample for visual observation and slide preparation(s) for microscopic examination and identification. The laboratory analyzed the bulk samples for asbestos

(chrysotile, amosite, crocidolite, anthophyllite, and actinolite/tremolite), fibrous non-asbestos constituents

(mineral wool, cellulose, etc.), and non-fibrous constituents. Using a stereoscope, the microscopist visually estimated relative amounts of each constituent by determining the volume of each constituent in proportion to the total volume of the sample. Samples reported to contain less than 1% asbestos were point counted in order to quantify trace asbestos for compliance with NESHAP techniques.

*Laboratory analytical data sheets were reviewed for potential asbestos content within individual layers of a sample for each material. Analysis of an individual layer of a material may exceed 1% while the composite analysis of the material as a whole is below 1%. Laboratory analytical data sheets are presented within Appendix A.

CONCLUSIONS AND RECOMMENDATIONS

Results of analysis confirmed asbestos was detected in 12 of the 309 bulk samples collected at the

Hospital Administration Building No. 1. The U.S. Environmental Protection Agency (EPA) regulations do not require removal of asbestos containing materials that are in good conditions. However, personnel who may be involved with building renovations will need to be advised of the presence of asbestos and appropriate measures may be warranted in order to assure the identified asbestos-containing materials are not disturbed during renovation activities. If the asbestos-containing materials left in place are disturbed during renovation activities, the materials must be handled and disposed of in accordance with applicable

State and Federal regulations.

AIHA LAP, LLC Laboratory #102929

A "trace asbestos" will be reported if the analyst observes far less than 1% asbestos. CA Labs defines "trace asbestos" as a few fibers detected by the analyst in several preparations and will indicate as such under these circumstances.

Since allowable variation in quantification of samples close to 1% is high, <1% may be reported. Such results are ideal for point counting, and the technique is mandatory for friable samples (NESHAP, Nov. 1990 and clarification letter 8 May 1991) under 1% percent asbestos or "trace asbestos".

In order to make all initial PLM reports issued from CA Labs NESHAP compliant, all <1% asbestos results (except floor tiles) will be point counted at no additional charge.

CA Labs is accredited by the National Voluntary Accreditation Program (NVLAP) for selected test methods for airborne fiber analysis (TEM), and for bulk asbestos fiber analysis (PLM). CA Labs is also accredited by AIHA LAP, LLC. in the PLM asbestos field of testing for Industrial Hygiene. All analysts have completed college courses or hold a degree in a natural science (geology, biology, or environmental science). Recognition by a state professional board in one these disciplines is preferred, but not required. Extensive in-house training programs are used to augment the educational background of the analyst. The Laboratory Director and Quality Manager have received supplemental McCrone Research training for asbestos identification. Analysis performed at Crisp Analytical Labs, LLC 1929 Old Denton Road Carrollton, TX 75006

Summary of polarized light microscopy (PLM / Stereomicroscopy bulk asbestos analysis) using the methods described in 40CFR Part 763

Appendix E to Subpart E (Interim and EPA 600 / R-93 / 116 (Improved). The sample is first viewed with the aid of a stereomicroscope. Numerous liquid slide preparations are created for analysis under the polarized microscope where identifications and quantifications are preformed. Calibrated liquid refractive oils are used as liquid mouting medium. These oils are used for identification (dispersion staining). A calibrated visual estimation is reported, should any asbestiform mineral be present. Other techniques such as acid washing are used in conjugation with refractive oils for detection of smaller quantities of asbestos. All asbestos percentages are based on calibrated visual estimation traceable to NIST standards for regulated asbestos. Traceability to measurement and calibration is achieved by using known amounts and types of asbestos from standards where analyst and laboratory accuracy are measured. As little as 0.001% asbestos can be detected in favorable samples, while detection in unfavorable samples may approach the detection limit of 0.50% (well above the laboratory definition of trace).

Vermiculite containing samples may contain trace amounts of actinolite/tremolite. When not detected by PLM, these samples should be analyzed using TEM methods and / or water separation techniques. Suspected actinolite/vermiculite presence will be indicated through the sample comment section of this report.

Fibrous talc containing samples may contain a regulated asbestos fiber known as anthophyllite. Under certain conditions the same fiber may actually contain both talc and anthophyllite (a phenomenon called intergrowth). Again, TEM detection methods are recommended. CA Labs PLM report comments will denote suspected amounts of asbestiform anthophyllite with talc, where further analysis is recommended.

Some samples (floor tiles, surfacings, etc.) may contain fibers too small to be delectable by PLM analysis and should be analyzed by TEM bulk protocols.
