Questions and Answers for Vascular Lab Accreditation RFP 36C25722R0061 002 September 9, 2022

1. Question: Is a new requirement or a follow-on to an existing contract?

Answer: New requirement

2. Question: Is this a brand-new contract OR is there is (was) an incumbent performing these services?

Answer: New requirement

3. Is this requirement to establish a new non-invasive vascular laboratory?

or restore services that are not currently offered for lack of personnel and/or accreditation?

Answer: The Central Texas Veterans Health Care System (CTVHCS); requires the contractor to establish and maintain an IAC accredited noninvasive vascular laboratory to support patient care within the Central Texas Veterans Health Care System (CTVHCS).

4. Are vascular ultrasound imaging exams being performed at the facility currently?

Answer: No

5. Per the CLIN Structure - it does not address both Key Personnel positions as listed in sub-factor (b) - Staffing. Please add a CLIN/subCLIN to account for the Key Personnel - Vascular Laboratory Program Supervisor's time per the Evaluation Criteria.

Answer: Solicitation updated to incorporate both key personnel.

6. Please confirm the Key Personnel requirements. Under the Evaluation Criteria Factor 1 - Technical Proficiency and Experience Sub-factor (b) – Staffing

• Vascular technologist(s) to provide at least 1.0 full-time equivalent staffing on-site at the Olin E. Teague Veterans Center: 1-2 key personnel.

• Vascular laboratory program supervisor: 1-2 key personnel employed by the contractor to manage administrative, regulatory, and training activities related to the contract.

Please advise.

Answer: Updated to reflect the following.

Sub-factor (b) – Staffing

1. List key personnel, to include a minimum number of the following:

• Certified vascular technologist(s) to provide at least 1.0 full-time equivalent staffing on-site (2080) at the Olin E. Teague Veterans Center: 1-2 key personnel

• Vascular laboratory program manager to provide at least .23 full-time equivalent: 1-2 key personnel employed by the contractor to manage administrative, regulatory, and training activities related to the contract.

7. Per PWS Section 4 Contractor Responsibilities - Education and Training - it states, "Up to 40 Category I AMA Credits related directly to Vascular Testing every three years for Vascular Laboratory Medical Director and Vascular Laboratory Medical Staff and up to

40 Society for Vascular Ultrasound (SVU)-CME credits to technologists for QI activities each year. "How many Vascular Laboratory Medical Staff are there now and anticipated in years 2-10?

Answer: There are currently no personnel staffing a vascular laboratory at CTVHCS.

Expansion of services is anticipated in the future, as the need for vascular testing is anticipated to grow beyond what one technologist can support. In the short-term, the contractor will be expected to operate in a collaborative environment, with a goal of harmonizing testing standards across various departments. For example, the vascular laboratory (VL) will establish policies and procedures for each examination offered.

Providing education to Radiology Service sonographers about the VL procedures and sharing interpretation criteria and standards is one way that intra-facility variation can be reduced, and quality/reliability of testing can be increased.

8. Is the required training to occur during normal work hours and considered billable hours?

Answer: In-person required training is expected to occur during normal work hours and would be considered billable hours. The contractor may have additional self-study requirements for staff. It is expected that the contractor may have additional offsite staff to support administrative, quality improvement, training, and education activities, thus allowing the on-site staff to prioritize the performance of vascular diagnostic examinations.

9. Per PWS Section 4 Contractor Responsibilities - Management - it states, "Use of a database to track and report QI activity results, documenting…" What database is this and who Is responsible for providing and maintaining it?

Answer: The contractor will have access to VA informatic systems, including a RIS, PACS, and HER. The contractor may use these or other systems to track and report QI activities for the vascular laboratory. At present, with no operational vascular laboratory at CTVHCS, there is no extant system being used for vascular lab QI.

10. Per PWS Section 4 Contractor Responsibilities - Staffing - it states, "Ability to train and supervise other vascular lab staff, clinical staff, or Radiology department sonographers to…" How many personnel are there now and anticipated in years 2-10?

11. Is the ultrasound equipment listed in the contract already purchased and on site?

Answer: A programmable treadmill and new duplex scanner are currently in the acquisition process.

12. How many vascular physicians will be on site and ordering the ultrasound exams?

Answer: Initially, it is anticipated that due to capacity issues, orders for vascular laboratory examinations will come from residents and staff of the Vascular Surgery Section. This includes four vascular surgeons (approximately 2.5 FTE), a physician assistant, a nurse practitioner, and two vascular surgery fellows or residents. If demand exceeds capacity, the vascular laboratory technical director and the vascular laboratory medical director will establish a system to triage requests.

13. Please provide historical data for the past 3 years to include the # of vascular ultrasounds exams performed/ordered?

Answer: As the CTVHCS vascular laboratory has been closed since 2019, no data about volume for the past three years can be provided. Based on the volume of patients seen and followed by the vascular surgery clinic, it is anticipated that demand for services will be present from the outset, with the schedule for the vascular lab defined by a triage system. Once the lab has be re-established, workflows have been refined, and an accreditation application has been prepared, it may be anticipated that future attention may turn to how to expand services to meet the needs of general users (i.e., referrals from primary care, cardiology, etc.).

14. Will the USG reconsider allowing experience of proposed key personnel/employees to demonstrate technical proficiency and experience in accordance with section 2.1 of the

PWS?

Answer: The government requires a certified vascular laboratory technologist in accordance with section 2.1 of the PWS.

15. Per the Instructions to Offerors, Please advise which Volume (I, II, or III) each of these completed attachments is to be included.

(f) The offeror shall complete and submit with offer the following attachments:

• A: Page 1

• B.2: PRICE/COST SCHEDULE

• B.3: 2. QUALIFICATIONS (Provide required documentation; also see E.8)

• D.1: QASP

• E.10: REPRESENTATIONS AND CERTIFICATIONS (www.sam.gov) (Ensure all required areas are completed) Answer: These documents are not a part of volume I, II, or III.

16. The Base Year lists the estimated quantity (EST. QTY.) as 2080 hours, the hours required for a Vascular Lab Technologist FTE, but the “Description” also lists “Education and Training”, which requires additional hours and personnel. Can you clarify the hours needed? Can this be separated out?

Answer: The price cost schedule has been updated. In accordance with FAR 52.212-1(e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions, including alternative line items (provided that the alternative line items are consistent with FAR subpart 4.10), or alternative commercial products or commercial services for satisfying the requirements of this solicitation. Each offer submitted will be evaluated separately.

17. For the Base Year, there is no mention of IAC accreditation application preparation and submission, which requires a minimum of 1040 hours for the initial application. Can this be separated out for the proposal?

Answer: In accordance with section B.3 section 1.1 The Central Texas Veterans Health Care System (CTVHCS); requires the contractor to establish and maintain an IAC accredited noninvasive vascular laboratory to support patient care within the Central Texas Veterans Health Care System (CTVHCS). Line items 0001/0002 are part of the initial accreditation. In addition, FAR 52.212-1(e) Multiple offers.

18. For the Option Year One through Ten, the estimated hours would be for maintenance of the IAC application, ongoing quality improvement, and training and education. There is no estimated hours for the “Certified Vascular Laboratory Technologist” as stated in “Item Number 0001”. Will the VA require ongoing Technologist coverage to maintain the accreditation? Can the technologist coverage be added to the proposal for the Option Years?

Answer: The price cost schedule has been updated. In accordance with FAR 52.212-1(e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions, including alternative line items (provided that the alternative line items are consistent with FAR subpart 4.10), or alternative commercial products or commercial services for satisfying the requirements of this solicitation. The Government does not guarantee that it will place any orders under this contract in excess of the guaranteed minimum guaranteed award amount.

Inclusion of FAR 52.217-8, Option to Extend Services: The evaluation will assume that the prices for any option exercised under FAR 52.217-8, will be at the same rates as those in effect under the contract at the time the option is exercised. The evaluation will therefore assume that the addition of the price or prices of any possible extension or extensions under FAR 52.217-8 to the total price for the basic requirement and the total price for the priced options has the same effect on the total price of all proposals relative to each other, and will not affect the ranking of proposals based on price, unless, after reviewing the proposals, the Government determines that there is a basis for finding otherwise. The evaluation will consider the possibility that the option can be exercised at any time and can be exercised in increments of one to six months, but not for more than a total of six months during the life of the contract. This evaluation will not obligate the Government to exercise any option under FAR 52.217-8.
