B.3 STATEMENT OF WORK

Scope of Work Expansion of Security Cameras

Building 163 Temple VA Project

Requirements:

The contractor shall be responsible for providing labor and materials for the services required to install security cameras in specified areas of the hospital per the drawings for Building 163. This is an expansion of the existing security system. Work performed will be done at the Central Texas Veterans Health Care Facility in Temple at 1901 Veterans Memorial Drive, Temple, Texas 76504. After all work is completed, the contractor will have a walkthrough with the Project Manager (PM) or VA COR to ensure all work is completed, provide test reports and ready to receive final payment.

The Contractor shall provide and install any required materials, equipment, labor, tools supervision, skilled personnel, and trades for the completion of this project. The Government will not provide any materials, equipment, labor, or trades for this project. It is the sole responsibility of the Contractor to meet the requirements of this project without any assistance from the Government. The Government will provide access and information only for this project. All equipment must be functional tested and verified/witness by COR/VA Team.

The Contractor shall contact the VA COR 10 days ahead of starting work to allow for coordination.

The Contractor shall protect all government property from damage to include floors, ceiling tiles, fire suppression equipment, walls, wiring and existing items above the ceiling. Contractor shall replace any damaged item with new or better equipment. All wall penetrations must be preapproved by the VAPM.

The Contractor shall provide a work schedule to include a detailed list of work to be done for each requirement and the sequence in which it will be accomplished. The schedule shall be in the form of a progress chart of suitable scale to indicate the work scheduled for completion by any given date during the work. The work plan shall indicate time frames and programmed power shut-off, if applicable.

Contractor shall highlight on the schedule any off-hours and overtime work requirements. Any overtime or outside of normal scheduled time must be preapprove by the VAPM two days in advance. Work arounds in patient care areas will have to be considered during the Day and adaptations will have to be made on a day-to-day basis due to patient care issues which will be may be emergent. The Contractor shall notify the VAPM upon entry and exit to the VA property.

A pre site visit will be coordinated for Contractors bidding and all must be present to verify measurements, material requirement per the SOW and investigate site conditions prior to submitting proposal. All work must comply with federal, state, and local regulation All necessary equipment required to meet the intent of these specifications, whether or not enumerated within these specifications, shall be supplied, and installed to provide complete and operating security cameras.

Contractor and all associated subcontractor employees shall comply with applicable facility and local security policies and procedures. Contractor shall provide names of Contractor and Subcontractor employees to the COR or Government representative. VA will issue contractor badge/pass to contractor and subcontractor employees, as applicable. Employee list shall include name, title, qualifications and responsibilities like hazardous materials, PPE, fire safety penetrations, material disposal, lock-out/tag-out, etc.

The Contractor shall provide one (1) year performance agreement labor and material in addition to the manufacturer’s warranty after final acceptance. Contractor shall provide a warranty management plan copy. The plan shall include information relevant to the warranty of materials and equipment incorporated into the project, including the starting date of warranty. The plan shall be in narrative form and contain enough detail to render it suitable for use by future maintenance and repair personnel, whether tradesmen, or of engineering background, not necessarily familiar with this contract. The plan should also include drawings with a system block diagram of the electrical assembly of the complete system. The plan shall be assembled in a binder and shall be turned over to the Government at the final walkthrough. The warranty period shall begin on the date of project acceptance and shall continue for the full product warranty period.

“VHA Supplemental Contract Requirements for Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors

1. Contractor employees who work in or travel to VHA locations must comply with the following:

a. Documentation requirements:

1) If fully vaccinated, shall show proof of vaccination

i. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination

2) If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor’s order or an FDA approved over-the-counter test.

3) Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel.

4) Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.

2. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the following website: COVID-19 Screening Tool. Regularly check the website for updates.

a. Contractor employees who work away from VA locations, but who will have direct patient contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.

3. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.”

All work shall be done according to the Infection Control Risk Assessment (ICRA) specified for the respective areas where work is performed.

Note 1: The C&A requirements do not apply, and that a Security Accreditation Package is not required for this contract.

Note 2: Per VHA Directive 1193 (paragraph 3.b) Dated August 13, 2021 Contractors shall be fully vaccinated for Covid 19 to work on Federal property. The vaccination status will be verified by VA personnel each day onsite. Screenings and temperature will be required before entry in all buildings.

Note 3: The Contractor shall schedule and provide installation in such a manner that will not impede patient care.

Note 4: The Contractor shall provide training for programming the system, troubleshooting the system, and repairing the system (inclusive of documentation, i.e. handbooks).

Note 5: UL Listed 1 ½” or larger fire rated cable way shall be used at the fire wall penetration. Also, Ul listed red fire caulk shall be used for small opening at conduit.

Note 6: The Contractor shall ensure that camera system wiring, or cables are attached to J-hook hangers and not to existing equipment/piping in the ceiling.

Note 7: The Contractor shall obtain all required permits to include infection control & above ceiling permits.

Project Requirements:

1. The Contactor shall provide and install the following requested equipment in the specified areas of the hospital below to expand the existing security system.

Indoor Dome Camera

Multi-Sensor (Quad Camera)

2. 12-24 port switch POE with at least 2 SFP ports, area 1, with wall mount cabinet Five dome cameras, one multi-flex camera.

3. 12-24 port switch POE with at least 2 SFP ports, area 1, with wall mount cabinet Five dome cameras, one multi-flex camera.

4. 12-24 port switch POE with at least 2 SFP ports, area 2, with wall mount cabinet (or just rack, due to space) Two dome cameras, one multi-flex camera, and one Bullet Camera on a outside exterior wall mount.

5. 12-24 port switch POE with at least 2 SFP ports, area 3, with wall mount.

Two dome cameras, one multi-flex camera.

6. And all other necessary parts and supplies for installing the components and cabling above as normally utilized in competitive installations and standard practice.

7. Single Mode Fiber should be Plenum Rated.

8. CAT6 minimum ethernet wiring should also be used. Rated for Plenum Use when Ran in ceilings etc.

9. There will also be some SM Fiber runs made at this time also for connectivity to the cameras.

10. One 12 strand SM fiber run will be running from bb-117 to BG-04. This 12 strand SM fiber will be running in plastic conduit and using j-hooks and cable trays where possible. The area near bb- 117 is highly congested and allowances will be given at installation time for that. Both ends of the fiber will be terminated in fiber termination boxes mounted on the wall. Terminations will be LC (SFF) connectors.

11. One 12 strand SM fiber run will be running from BG-04 up to 4g20-204-t. This 12 strand SM fiber will be running in plastic conduit if possible or may be ran without conduit if necessary due to space requirements of through-floor penetrations. These two rooms are stacked and through floor penetrations will be used. Both ends of the fiber will be terminated in fiber termination boxes mounted on the wall. Terminations will be LC (SFF) connectors. Termination boxes in this room (BG04) will be next to each other.

12. One 12 strand SM fiber connection will be from the switch mounted in 2E205 up and around the wall using j-hooks and plastic conduit for protection of fiber, to a hole in the floor to Elevator machine room 1E149 and then ran through 1C161 elevator room to hallway and then on to 1E157 (electronics com closet) and connected to the switch installed there.

13. One 12 strand SM fiber will be through the floor into the sub-basement into the bldg. through the Elevator Lobby area in the ceiling and all the way to BG-04 and terminated in a wall mount box, near the other wall mounted boxes here-in. This run is probably about 400ft or so. This fiber run will be run in plastic conduit. Utilizing j-hooks if necessary and cable trays if not.

14. One Video edge from TYCO/American Dynamics to match what we already have installed shall be used for perfect integration. One switch, a cisco standard 24-port / at least 2 SFP ports will be used at the 4g20 server rack where the other servers are installed. One cat6 wire will be ran from 4g20 to 4j34 which is under 200ft from 4g20. These will be installed in current rack already in place, which is a standard 19-inch rack.

15. Cameras will be Illustra, Each Illustra camera is NDAA compliant and exceeds all federal regulations that the VA falls under. Also, no extra licenses are required for Illustra Cameras. We currently use Illustra and have examined the Multi-sensor quad camera and require this utilization as stated in the above specifications.

Building 163 Logistic Store Area Above ED

1. Provide, install, terminate each end for 4 cameras, wires, conduits, from the 2nd floor Logistic E202 rom to the IT/electronic switch room near Ed room at 1st floor. Three (3) cameras will be on the end wall looking down the Primary inventory shelving isles. One (1) camera will be in the room near the East wall looking at the exit doorway.

2. Provide, install, terminate each end for 4 cameras outside of the room 2E202 hallway to the IT/electronic switch room near Ed room at 1st floor as indicted below:

a. Provide and install a Tyco/Illustra Flex multisensory camera (to function and connect with existing Tyco security system) with views of the 4 (four) cameras inside it pointing in the various areas, toward the stairwell door, the 2E202 door, down the hallway opposite the stairwell and the last one to be determined at installation time. The four (4) cameras (sensors) inside the Flex Multisensor are highly movable and can be positioned at installation time.

b. Down the hallway in front of the elevator shaft, provide and install a camera on the opposite wall looking at the elevator doors, as shown in the diagram.

c. Down the hallway in front of the elevator shaft, provide and install a camera on the opposite wall looking at the elevator doors, as shown in the diagram.

d. Provide and install a camera inside the second-floor fire exit stairwell on the wall opposite the door viewing the door. This area has a solid ceiling and if able to run cabling inside the ceiling is not possible then some form of metal conduit shall be used, in that area.

e. Inside the mechanical room 2E205, on the wall near the door, provide and install/hung a wall-mount cabinet with a POE switch to function with the cameras installed on the second floor. This cisco switch switch shall have a minimum of 24 POE jacks, with four

(4) SFP ports. There shall be a hole created above the switch with a sleeve installed and the sleeve will be sealed with fire caulk plug when finished with installation.

Building 163 Outside of Emergency Dept

1. Provide and install a camera and wire inside the first-floor stairwell on the wall opposite the door viewing the door. This area has a solid ceiling and if able to run cabling inside the ceiling is not possible then some form of metal conduit shall be used, in that area.

2. Provide and install a Flex Multisensor camera and wires ceiling mount across from the emergency exit door, with one view pointing at the emergency door, one camera view at the stairwell exit, and one camera view down the hallway. The last camera view will be determined at installation.

3. Provide and install a camera in front of the elevator shaft. A camera shall be on the opposite wall (in ceiling) looking at the elevator doors, as shown in the diagram.

4. Provide and install a outdoor camera outside the emergency exit door and pointing at the doorway. The camera will be installed on the brick wall under the fascia. If the penetration cannot be used a penetration will be drilled through the wall. All wall penetrations will be minimal, and sealed with UL Listed approved fire caulk, preferably Hilti or equitable.

5. Provide and install four camera wires “home-run” cabled with no breaks in the wiring. The four wiring runs will be Ran to 1E157 which is an electronics communication closet. These runs are all under 300ft.

6. Provide Install a wall mount bracket with a switch installed for the cameras. This switch should have a 24 POE jacks, with four (4) SFP ports at the least.

7. These Four cameras will be “home-run” cabled with no breaks in the wiring if possible. The four wiring runs will be Ran to 1E157 which is an electronics communication closet. These runs are all under 300ft.

8. This closet (1E157) shall have a wall mount bracket with a switch installed for the cameras. This switch should have a minimum of 12 POE jacks, with two (2) SFP ports at the least.

Area Three, this area is located at the SPS back dock area.

1. A camera will be installed under the overhang outside of room BD115. The overhang area has a 2x2 metal access door located next to where the camera needs to be installed, looking at the dock itself.

2. A camera will be installed across from the camera above near BD115, but next to the opposite wall. A 2x2 metal access door will be needed to install this camera in this overhang.

3. A Flex Multisensor camera will be installed inside the exit doorway, with one camera sensor pointing at the exit doorway, one sensor pointing at the SPS double doorway, and a third sensor pointing at the hallway leading away from the area. The fourth sensor will be determined at install.

4. These three cameras will have wiring ran, with no breaks, to closet BB-117.

5. This closet (BB-117) shall have a wall mount bracket with a switch installed for the cameras.

This switch should have a minimum of 12 POE jacks, with two (2) SFP ports at the least.

1. Perform a final walkthrough with the VA COR or PM when completed.

It is understood and agreed between the Government and the Contractor that observance of such days by Government personnel shall not otherwise be a reason for an additional period of performance, or entitlement of compensation. In the event the Contractor’s personnel work during the holiday, they may be reimbursed by the Contractor, however, no form of holiday or other premium compensation will be reimbursed either as a direct or indirect cost, other than their normal compensation for the time worked.

A. Invoices and Payment

a. Payment to be made monthly, in arrears, following satisfactory performance by Contractor’s personnel and upon receipt of a properly prepared invoice.

b. Contractor shall be required to invoice through the Tungsten System at http://www.tungsten-network.com/us/en/veterans-affairs/. Additional information regarding Tungsten shall be provided upon award.

c. The contract shall have current registration in the System for Award Management (SAM) for this solicitation; URL: www.sam.gov.

d. The invoice MUST be itemized to include the following information. Any information listed below, and not provided on an invoice, will render that invoice incomplete and the invoice will be returned for immediate correction.

i. Time period being invoiced for.

ii. Description of services performed.

iii. Unit Cost billed.

iv. Extended amount due.

v. Invoice number, date, and;

vi. Contract and obligation numbers.

Other Considerations

• The contractor will be required to submit a safety plan for VA review and approval prior to the performing work.

• It is the contractor’s responsibility to provide adequate documentation verifying that any equipment and/or materials proposed will meet or exceed all federal government requirements.

• Majority of the work shall be performed during normal working hours. Normal working hours are 8:00 am – 5:00 pm, Monday – Friday. Some work will be completed during evening and weekends to avoid patient care interruptions. All premium time must be preapproved with the VAPM two days ahead.

• The contractor’s supervisor shall always be available to address any needs the VA COR may have during the period of performance. All employees or sub-contractors shall be accountable solely to the Contractor, not the Government.

• The contractor may be required to coordinate work activities and shutdowns with VA COR and Project Manager.

• The contractor shall thoroughly clean inside and outside of project site daily with contractor’s own equipment.

• The Government will not provide facilities or staging area for the Contractor’s use to support this contract. The contractor is responsible for all shipping, receiving, storage and security of any materials or equipment for the duration of this project.

• The Government will not provide materials for this project.

• Contractor personnel’s conduct shall not reflect discredit upon the Government. The Contractor shall ensure that personnel present a professional appearance. The Contractor’s employees shall observe and comply with all local policies and procedures concerning fire, safety, environmental protection, sanitation, security, and possession of firearms or other lethal or illegal weapons or substance. The Contractor is responsible for ensuring that any Contractor employees providing services under this contract conduct themselves and perform services in a professional, safe, and responsible manner. The Contractor shall remove from the job site any employee for reasons of misconduct or security. The Contractor shall ensure no contractor employees conduct political related activities or events on VA Facilities.

• The Contractor shall notify the COR or Government representative of any accidents requiring medical treatment. Emergency medical treatment and services for Contractor personnel is the responsibility of the Contractor.

The project shall be in accordance with the latest Uniform Building Code (UBC), National Fire Protection Associate (NFPA) standard, Uniform Federal Accessibility Standard (UFAS), Inspectors Manual, VA Specifications and Standards, and local facility requirements. In the event there is a conflict between any of the standards/codes, the more stringent standard/code shall apply.

The contractor shall provide all applicable safety signage and barricades applicable to OSHA, VHA and DOD regulations. Contractor shall comply with VHA DIRECTIVE 2004-012 and all VA Center Policy Memorandums, including but not limited to the following:

(a) Protection of patients, visitors, and employees from traumatic injury, as well as occupational and facility–associated infections.

(b) Compliance with OSHA and State construction safety regulations. Construction contracts require that the general contractor’s on-site “competent person” have completed the OSHA 30-hour construction worker course and that all other general and sub-contractor’s workers have completed the OSHA 10-hour construction worker course or other relevant competency training. The determination for training is based on the project hazards and complexity, State and Federal regulations and VA requirements. Verification of completion of training will be required prior to performing work.

(c) Compliance with EPA and State environmental regulations.

SAFETY PLAN REQUIREMENTS

Prior to performing any work, the contractor will be required to submit a safety plan in accordance with OSHA and VA Regulations.

The safety plan shall include, but not limited to the following:

A. Safety and Health Policy (i.e. company policy) B. Safety and Health Objectives (i.e. company goals, procedures, etc) C. Job Site Inspections (i.e. safety inspection/potential hazards/prevention, etc) D. Contractor Safety Administrator (i.e. competent person/safety officer) E. Personal Protective Equipment (i.e. head, hearing, and eye protection, footwear, clothing, gloves, etc.)

F. Safety Rules (i.e. rules applicable to the work/task being performed, equipment, ladders, confined spaces, open flame, etc.)

G. Job Safety Training (i.e. OSHA, safety meetings, etc)

• 30 Hour OSHA Construction Safety Course – competent person

• 10 Hour OSHA Construction Safety Course – all others

H. Safety Discipline (i.e. correction of violations, etc) I. Hot Work Permit (i.e. responsibilities, procedures, etc) J. Emergency Procedures (i.e. responsibilities, procedures, etc) K. Written Hazard Communication Program (i.e. responsibilities, procedures, list of hazards, MSDS, etc)

SAFETY INSPECTIONS

On a weekly basis, the VA will conduct a safety inspection of the work area. This inspection includes, but is not limited to the following:

A. Means of Egress

• Fire/Smoke Doors unlocked, free of obstructions

• Means of Egress (includes Exits) free of obstructions and clearly marked

B. Fire Alarm, Fire Detection, and Fire Suppression Systems.

• Fire Alarm Pull Stations are unobstructed

• Smoke Detector smoke caps in use, or removed at end of the day

• Hot Work Permits are current, in use

• Sprinkler Heads are unobstructed

C. Housekeeping and Traffic Control

• Trash and debris are removed promptly and transported outside the construction area

• Floor mats and dust tack mats are located at the entrance to construction area

• Tools and equipment are properly stored

• Signs installed to restrict patient access (doors locked as needed)

• Dust control measures are in place

• Barriers are in place and properly sealed

• Debris barriers are install at all drainage locations to prevent debris in the storm drain system.

D. Hazardous Chemicals/Conditions

• Appropriate storage of hazardous chemicals

• MSDS sheets are readily available on-site

• Lock out/tag out procedures are in place

• Hot work permit is current and posted

E. Clothing

• PPE is available and properly worn

• Clothing is relatively dust free with the construction area and patient areas

(d) Comptroller General Examination of Record. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form.
