AHERA ASBESTOS INSPECTION

SECTION I - GENERAL INFORMATION

INTRODUCTION

Micro Air of Texas, Inc. (Micro Air) was contracted by the Veterans Administration North Texas Health Care System (VANTHCS) located in Dallas, Texas to perform an asbestos inspection and generate a management plan in accordance with guidelines established in 40 CFR 763, the Asbestos Hazard Emergency Response Act of 1987 (AHERA), for the Veterans Administration Medical Center (VAMC) located in Dallas, Texas. This facility consists of numerous buildings constructed between 1940 and 2009. This particular report pertains to Building 2, which houses the Main Hospital.

Micro Air performed a comprehensive asbestos survey of Building 2 on various dates from September through December, 2010. After the comprehensive asbestos survey results were compiled, a Management Planner accredited by an EPA-approved training provider and licensed as an Individual Asbestos Consultant in the state of Texas, reviewed the analytical results of the inspection and current physical condition assessments. The management planner then determined the potential hazard posed by the ACM and evaluated and selected control / response options based on all compiled information. This information was then composed and written into a final report.

During the 2014 3-Year Asbestos Assessment and Re-inspection, Micro Air noted that Building 2 had been remodeled since the original asbestos inspection on this facility was conducted in 2010. Between 2010 and 2014, the interior of multiple areas in Building 2 were demolished and new building materials were re-installed. Micro Air conducted an asbestos inspection on the interior of Building 2 in order to update the Management Plan. A total of 163 bulk building material samples were collected between June and December, 2014, and the results were included in this management plan.

The federal National Emission Standard for Asbestos (NESHAP) and the Texas Asbestos Health Protection Act (TAHPA) both require that prior to any demolition or renovation activity in a building, a thorough inspection of the affected facility or part of the facility where the demolition or renovation operation will occur, must be performed for the presence of asbestos, including Category I and Category II non-friable asbestos-containing material (ACM).

Additionally, TAHPA requires that a minimum of three samples be collected from each homogeneous sampling area (material type) identified in the building. The work area and immediately surrounding areas which could foreseeably be disturbed by the actions necessary to perform the project must be inspected and sampled as applicable prior to renovations or demolition. While the VANTHCS is not subject to the TAHPA regulation, due to its exclusion as a federal facility, Micro Air collected bulk samples in accordance with TAHPA and AHERA protocol at the request of the VANTHCS engineering personnel.

M ICRO AIR OF TEXAS, INC. VAMC; Dallas, Texas - Building 2 2

REQUIRED INFORMATION

In accordance with AHERA protocol, the VAMC located in Dallas, Texas is required to name a person who shall be responsible for implementation of the management plan (Designated Person - see Section V). Per the Occupational Safety and Health Administration (OSHA) 1926.1101, construction industry standard, maintenance personnel should be made aware of the types and locations of all facility ACM. These employees are required to have adequate asbestos awareness training (minimum 2 hours), and all maintenance personnel who may come in contact with or disturb ACM are required to receive a minimum of 16 hours of asbestos training in asbestos hazards and recognition of ACM. If the owner wishes to perform ACM activities with its own personnel, the owner is responsible for obtaining the required EPA training in accordance with the Model Accreditation Plan (MAP), which is located in Appendix C, Subpart E of 40 CFR, 763 (AHERA). Additionally, all OSHA requirements must be met, as well (i.e., asbestos physical, etc.). Otherwise, it is recommended that all ACM activities be performed by a Texas licensed Asbestos Contractor, and project oversight be conducted by a Texas licensed Asbestos Consultant Agency.

In accordance with the VA directive to comply with AHERA, the entire facility must be re-inspected and a reassessment of ACM conducted by an accredited inspector at least once every three years, and the management plan should be updated by a licensed Asbestos Inspector / Management Planner or Individual Asbestos Consultant. Additionally, visual inspections of the ACM must also be made by the Designated Person for the facility at least once every six months to determine whether any areas of the material have changed in condition, or if there are any other factors which may necessitate abatement actions.

FACILITY DESCRIPTION

Building 2 is approximately 441,649 square feet in size, and it serves as the Main Hospital for the Dallas VA. The construction date of this building is believed to be 1955. The building consists of a sub-basement, basement, and nine floors where Main Hospital activities are conducted, and the 10 and 11 floors, which are mechanical areas. The general exteriorth th construction of the building is a concrete slab foundation with red exterior brick and a flat tar and gravel roof.

The sub-basement consists of dirt floors and concrete walls and ceilings. Pipe insulation was abated from the eastern portion of the sub-basement in the early 1990s; removal of asbestos-contaminated soil was also completed at this time. Other areas of the abatement may have been abated prior to the 1990s; however, the VA has no documentation stating this; therefore, all other areas of the basement are assumed to have asbestos-containing pipe insulation and possibly, asbestos-contaminated soil.

The basement and 1 through 9 floors are typically finished with a variety of flooring materialsst th including vinyl floor tile, sheet vinyl flooring, ceramic tile, rubber flooring material, carpet and concrete. Floor tile and black floor tile mastic are present beneath the carpet in some locations. Walls are generally sheetrock or plaster. Ceilings include several types of lay-in

M ICRO AIR OF TEXAS, INC. VAMC; Dallas, Texas - Building 2 3 ceiling tile, sheetrock, plaster and concrete. Abatement of pipe insulation and floor tile and mastic was completed in the following areas: portions of the basement in 2001 through 2005 and 2007, portions of the 1 Floor in 2000 through 2004 and 2009, portions of the 2 Floorst nd in 2000 through 2003, portions of the 3 Floor in 1997, 1998, 2003 and 2005, portions of therd

4 Floor in 1999 through 2004, one area on the 5 floor in 2007, portions of the 6 Floor inth th th

1996 through 1998, portions of the 7 Floor in 2005, 2009 and 2010, portions of the 8 Floorth th in 2005 and 2006, and one portion of the 9 Floor in 2007. th

Asbestos-containing pipe and fitting insulation (steam, hot water, chilled water, etc.) has been abated from numerous areas inside Building 2. Additionally, HVAC duct insulation with black mastic and floor tile and black mastic have also been abated from various areas inside the building. See Section IX for more specific information regarding areas that have been abated.

HVAC air handlers were observed to have rubber vibration dampeners and friable asbestos-containing cloth vibration dampeners. HVAC ducts with insulation and black mastic were observed throughout the building. Remnant sections of original asbestos-containing steam pipe insulation were noted above the ceiling in numerous areas. Access to areas located above the plaster ceilings was limited; therefore, it is assumed that pipes exist above the solid ceilings and in wall chases. Recessed wall radiators are located primarily on the 7 , 8 , andth th

9 floors of the building, and they are assumed to have asbestos-containing insulation locatedth behind the radiator in the recessed wall cavity. It also appears that some of the recessed wall radiators may have been removed from some of the rooms on the 8 floor, as there is a metalth plate over the opening on the wall where the radiator(s) once existed.

2014 3-Year Asbestos Reassessment and Update

The interior of Building 2 was remodeled between 2010 and 2014. Based on information provided by the VAMC, the interior building materials of Building 2 were removed, and the interior of this building was remodeled at the following locations: Basement Suite BB02, 1st

Floor Suite 1C-421, 2 Floor Geropsych, 7 Floor Main Elevator Lobby, 7 Floor A-Wing andnd th th

C-Wing, and 9 Floor Elevator Lobby. th

SECTION II- ASBESTOS LABORATORY ANALYTICAL REPORT

METHODOLOGY

Micro Air of Texas, Inc. performed a comprehensive asbestos survey of Building 2 on several dates from September through December 2010 in accordance with AHERA and TAHPA sampling protocol and assessment factors. The inspection was performed by Melissa Kiser, an accredited asbestos inspector and management planner who is licensed as an Individual Asbestos Consultant by the state of Texas (license #10-5436). Ms. Kiser was assisted by Jennifer McMillin, David Easley, and Elizabeth Bargar, accredited asbestos inspectors who are licensed as Building Inspectors. Micro Air of Texas, Inc. is licensed as an Asbestos Consulting Agency (#10-0110). All samples collected during this inspection were analyzed by J Resources, Inc., whose laboratory is accredited by the National Institute of Standards3 and Technology (NIST) under the National Voluntary Laboratory Accreditation Program (NVLAP) program (accreditation #200525-0) and licensed by the state of Texas (license #30- 0273). Laboratory analysis of collected samples was performed in accordance with EPA and NVLAP prescribed methodologies for Polarized Light Microscopy of bulk material samples for the presence of asbestos. The results of laboratory analysis of collected bulk material samples are included in this section for reference.

A total of 807 bulk material samples were collected during this comprehensive survey, and 777 of the collected samples were analyzed by PLM. Federal regulations require that each identifiable layer of an individual bulk building material sample be analyzed separately for asbestos content. In addition, federal regulations state that if any one sample from a homogeneous area is found to be asbestos-containing, then the entire homogeneous area is considered asbestos-containing, regardless of the results from the remaining samples from that homogeneous area.

Between June and December , 2014, Micro Air of Texas conducted additional asbestos surveys in various locations of Building 2 that were renovated (see page 3 above for locations). The inspections were performed by Jennifer McMillin. A total of 163 bulk material samples were collected and analyzed by PLM during the asbestos surveys. All of the samples were analyzed by J Resources, Inc.3

CLASSIFICATION

All materials which were sampled as being suspect for containing asbestos or were assumed to be asbestos-containing were grouped by sample type into Homogeneous Sampling Areas for purposes of assessment and quantification. Homogeneous materials are those which have similar appearance, texture, and function. If the inspector believes these materials to be the same, even if they are located in separate areas of a building, they may be grouped into the same sampling set. For purposes of this inspection report, the Homogeneous Area is designated as : HA. Each Homogeneous Area is given a number to distinguish it from other Homogeneous Areas (Example: HA-01A).

M ICRO AIR OF TEXAS, INC. VAMC; Dallas, Texas - Building 2 2

A CAD drawing(s) demonstrating the location of each collected bulk building material sample has been included in this section, as well as a CAD drawing(s) depicting the location of asbestos-containing homogeneous areas. CAD drawings showing the approximate locations of areas that have been abated in the sub-basement, basement, and the 1 through 9 floorsst th can be found in Section IX of this Management Plan.

LIMITATIONS

Micro Air’s scope of services was limited primarily to accessible interior building materials.

While Micro Air made every effort to perform a thorough survey, the possibility exists that materials containing asbestos were not identified, inaccessible, or different from those identified at other locations. The findings and opinions presented are indicative of conditions on the date of our site inspection and should not be relied upon to represent conditions at substantially later dates, due to possible changes in the condition of building materials, renovations, and repairs. Additionally, most of the buildings located at this VA facility have steam pipes running underground to supply heat to each building. This material was inaccessible at the time of this inspection. All pipes located in wall chases, underground, in the crawlspace of a building, etc., is assumed to be ACM until tested and proven otherwise.

Micro Air was informed by the VAMC locksmith that there are several wooden doors located in Building 2 that have insulation inside the door. Micro Air did not sample any wooden doors during this survey; however, all wooden doors located inside this building should be considered suspect for containing asbestos-containing interior insulation. All doors should be sampled for ACM prior to being renovated or demolished.

SECTION III

M ICRO AIR OF TEXAS, INC. VAMC; Dallas, Texas - Building 2 1

SECTION III - ACM Material Assessment and Quantification

Each area where ACM was identified in the asbestos management plan was visually inspected and assessed as to physical condition and potential for airborne asbestos fiber release.

Included in the assessment factors are such items as friability, accessibility, amount of current damage, amount of airflow, vibration, or abrasion to which the material is subject, potential for water damage, type and number of personnel normally located within each homogeneous sampling area, proximity to normal maintenance activities and potential for disturbance of such materials under normal operating conditions, potential for deterioration, and overall health risk of the material.

Final hazard assessment ratings are based upon the system designed in the EPA AHERA protocol for asbestos building inspectors. The material assessment categories in order of decreasing critical importance are as follows:

1. Damaged or Significantly Damaged TSI ACM

2. Damaged Friable Surfacing ACM

3. Significantly Damaged Friable Surfacing ACM

4. Damaged or Significantly Damaged Friable Miscellaneous ACM

5. ACBM with the Potential for Damage

6. ACBM with the Potential for Significant Damage

7. Any Remaining Friable ACBM or Friable Suspected ACBM

These assessment categories were used to determine appropriate response actions for all ACM, which are included in Section II of this report.

M ICRO AIR OF TEXAS, INC. VAMC; Dallas, Texas - Building 2 2

The following accredited Inspectors / Licensed Individual Asbestos Consultant has assessed the ACM located within this facility in accordance with the above AHERA requirements.
