About this file
This document contains questions and answers regarding a solicitation for Community Based Outpatient Clinic services. The solicitation is for contracted primary care services for a new CBOC located in Killeen, Texas, to be issued by the Department of Veterans Affairs for the Central Texas Veterans Health Care System. The solicitation will be set aside for Service-Disabled Veteran Owned Small Businesses. The questions and answers clarify that the contractor cannot bill during the initial 90-day buildout period, but can begin billing on day 91 once inheriting patients from the VA. Estimated enrollment for the first year of the contract is around 4,700 patients, lower than the 6,000 patients per month outlined in Schedule B, therefore reducing the risk for the contractor.

Question and Answer Clarification
1. We would like for the Government to clarify the following from amendment 4 for the Copperas Cove and Killeen CBOCs. We believe the Government stating they are not “rolling panels of patients” to the new clinic is in conflict with the Government’s intent to “assign” approximately 1200 patients the first month and let the contractor inherit the vesting visit. For these 1200 assigned patients, please confirm the contractor will inherit those patient’s last vesting visit and the contractor can begin billing for them in month one of the contract.
Answer (Contracting): Build-out occurs during the first 90 days. The contractor cannot bill for patients in month one since build-out will be ongoing during that time.
Answer (Facility): VA do not intend to assign any Veterans, as this will be in conflict with Veterans choice to select their treating facility for care. For Veterans who elect to change their site of care to the contract clinic, the Contractor will inherit those Veterans.
Clarification Answer:
Question 1 – The first 90 days are for building the facility no PMPM since they do not see patients.
Question 1 – “YES” since the contractor is picking up the vesting it can start billing for patients on the 91st day.
2. Subject solicitation for Community Based Outpatient Clinic services was issued with anticipated enrollment of 6,000 vested patients per month. The contract requires build-out of a facility which will take approximately three (3) months. The Governments response to question 1.c) states the contractor must fully staff the CBOC based on the line items in the solicitation. Response to question 1.c)ii the Government estimates 1200 PMPM will be assigned first month with an estimate of 436 PMPM subsequently added each month thereafter for the first year. Given the Governments estimate and the enrollment period of nine (9) months during the first contract year actual enrollment may reach 4,688 at the end of the year. Average of enrollment for the build-up period, based on the above, will be approximately
2,944. Schedule B enrollment of 6,000 may not be achievable during the base year leaving the contractor at risk for recovery of build-out, equipment, furniture, fixtures, and full PACT staffing costs. The contractor will be required to divide total costs by 2,944 to accurately determine the
PMPM price, then multiply by 6,000 enrollees to comply with schedule B. This artificially inflates the price. Would the Government consider reducing the base year enrollment to better reflect their estimated enrollment build-up average of 2,944 PMPM. Doing so would decrease risk to the contractor and reduce price to the Government.
Answer (Facility): We estimate 1200 patients the first month; with an estimate of 436 each subsequent month for the first year. Estimated enrollment at the end of the first year is 6,000. This is not enrollment of
6,000 patients per month.
Clarification Answer:
Question 2 – Enrolment estimate of 4,700 for the first year.
