About this file
This is a Performance Relevancy Survey form (Exhibit A) for a federal contract solicitation related to replacing campus steam and condensate lines (Project No. 564-26-103; Contract No. 36C25626R0044).
Part 1 requires offerors to submit no more than three past performance projects completed within five years of the solicitation issue date. For each project, offerors must provide the project/contract title, contract number, project location, original and current contract values, start and end dates, whether the project was completed on time, any liquidated damages assessed, and CPARS availability. Offerors must describe the work performed and classify each project as "Very Relevant," "Relevant," or "Somewhat Relevant" based on alignment with the solicitation's scope, magnitude, and complexity requirements. Offerors must also provide the point of contact information for their reference on each project, including name, title, agency, email, and phone number, with explicit responsibility for follow-up on questionnaire submissions prior to solicitation close.
Part 2 requests additional non-project-specific information to be provided only once in the submission package. This includes information on liquidated damages assessments within the past five years, problems encountered on relevant projects with corrective actions taken, explanations of any marginal CPARS ratings, professional or industry awards with supporting certificates, unique skills and accomplishments with verification, and technical areas uniquely relevant to the solicitation describing specific work elements performed by the firm and major subcontractors, including identification of subcontractors expected to be used on the current project.
