The AHCP software is a discharge planning solution that integrates with the VA's VistA electronic health record system and CPRS (Computerized Patient Record System) to create health literacy-appropriate discharge instructions and care plans for veteran patients. Key deliverables include discharge planning software with customizable patient discharge instruction booklets, reporting functionality for quality improvement tracking, administrative user management capabilities, and branded site-specific configurations including congestive heart failure discharge instructions. The vendor must install and configure the software on designated VA servers, customize it for the G.V. Montgomery VAMC environment, and provide comprehensive technical and operational training to all staff members at the facility during normal business hours, Monday through Friday, 8:00 AM to 4:30 PM. This is an INTENT TO SOLE SOURCE NOTICE only.

The Department of Veteran Affairs, Network Contracting Office (NCO) 16, intends to issue a sole-source purchase order under the authority of 41 U.S.C 253(c) (1) - FAR 6.302-1 (only one responsible source and no other supplies or services will satisfy agency requirements), to with Engineered Care, INC., located at 44 Montgomery Street, Suite #3500, San Fransico, CA., 94104. Supply delivery will be 30 Days ARO under NAICS 541511. The Vendor shall provide the below list of materials to G.V. (Sonny) Montgomery Veterans Healthcare System located at 1500 East Woodrow Wilson Avenue, Jackson, MS., 39216-5116. This action will result in a firm-fixed-price supply purchase order. The period of performance shall be 30 days ARO with option years.

This notice of intent is not a request for quotations; interested parties may express their interest by providing a capabilities statement not later than 16 June 2026 at 10:00 AM CST, to kolbi.barton@va.gov. When responding to this announcement, respondents should refer to number 36C25626Q0873, Notice of Intent in the subject line. The capabilities statement must provide clear and unambiguous evidence to substantiate the capability of the party to provide the required supplies without deviation. Responding as an interested party under the www.beta.sam.gov website does not constitute convincing evidence of capabilities. A determination not to compete this proposed contract upon responses to this notice is solely within the discretion of the Government. Verbal and facsimile responses are not acceptable and will not be considered. All interested parties are reminded to be registered with System for Award Management (SAM) at https://sam.gov/SAM/ in order to be eligible for award of Government contracts. No telephone calls will be accepted. If after June 16th at 10:00 am, no viable responses have been received in response to this announcement, NCO 16 shall negotiate solely with Engineered Care, INC., located at 44 Montgomery Street, Suite #3500, San Fransico, CA., 94104.

The Government anticipates the award of a firm-fixed-price purchase order. A determination by the Government whether to compete the proposed requirement shall be based upon responses received and is solely within the discretion of the government. Information received shall be considered solely for the purpose of determining whether to conduct a competition.

REQUIRED SUPPLIES:

The license grant includes, at no additional charge, the right to make and use a reasonable number of copies of the Software for testing, back-up, archival and disaster recovery purposes, and of the Documentation for internal training, support and deployment.

The successful vendor will provide the following solution:

· Creation of After Hospital Care Plans with health literacy appropriate patient discharge instructions for patients;

· Report functionality to enable continual quality improvement and performance tracking;

· Administrative interface to perform user management, application configuration and auditing;

· Integrate the AHCP software with the hospital based VistA software used for evidence-based, VistA-Integrated Discharge Instructions. Care Transitions Portal replaced the site`s existing CPRS discharge note with a patient-friendly discharge instruction booklet that: * Has been shown to reduce unnecessary readmissions * Is customized for the VA environment to help educate veterans about their care and additional VA programs * Improves the discharge process at a VA site while providing leadership opportunities for clinical staff. This is a new software that will interface with CPRS to improve the discharge process. It allows the care givers to have one source to print Care Plans, discharge medication list, the patients schedule appointments, etc. End-users will access via CPRS Tools. Engineered Care grants to the Customer a license for the Customer and its Users to use the Product, during the Term. Term of license is 5 years.

· Allows care givers to have one source to print Care Plans, the patients schedule appointment, etc., through access via CPRS Tools.

· Clear discharge instructions compiled in a booklet to keep instructions in one place and be easier for veteran patients to review and understand and follow their Care Plans, in one easy booklet.

· Technology that is table integrated application intended to improve the patient discharge process by providing education regarding patient diagnosis, documentation follow-up appointment and discharge services, confirms the medication plan, reviews steps on what to do if a problem arises, assesses the patient’s understanding of the plan, gives the patient a written discharge plan to take home review and follow.

3. SPECIFICATIONS

The successful vendor will work with designated clinical and IT representatives of the GVSM VAMC to deploy the VistA integrated AHCP. This work will include installation, configuration and customization as required.

3.1 Salient Characteristics/Deliverables:

The proposed software will include the following features:

· Discharge planning software for providers

· Creation of health literacy appropriate patient discharge instructions configured for optimal access by GVSM VAMC staff

· Reporting functionality to enable continual quality improvement and performance tracking

· IT Professional interface to perform user management, application configuration and auditing

· User accounts will be created for all users

· Customized to match GVSM VAMC branding requirements including logo placement

· Addition of GVSM VAMC is related Congestive Heart Failure (CHF) discharge instructions Vendor will create and print a Care Plan booklet for every veteran discharged from the GVSM VAMC site. The Care Plan will include the following data to the extent that they are available within the system:

· Branded Site Logo

· Patient Aligned Care Team details

· Patient Demographics

· Medications

· Appointment Information

· Instructions on care and management of primary discharge diagnosis

· Diet and Activity Instructions

· Additional Instructions

· My HealtheVet information

3.2 AHCP Software Installation and Configuration

AHCP Software setup. Install the software on the designated server system and create initial user accounts for testing.

3.3 Implementation and Customization of After Hospital Care PlanVendor will customize the After Hospital Care Plan for the GVSM VAMC concerning specific medicines and procedures at GVSM VAMC. The AHCP booklet will be customized to address/improve care delivery.

4. PERFORMANCE MONITORING

The successful vendor will provide periodic performance reporting during the course of AHCP software installation and deployment. Jackson VAMC will have real-time access to the AHCP software during this time and may conduct periodic product sampling to verify compliance with requirements.

5. SECURITY REQUIREMENTS

Software shall operate within the VA internal/intranet side of the web system (VAWW). VA sensitive information will reside within the vendor database.

C&A requirements do not apply, and a Security Accreditation Package is not required. ITARS #598157 has been approved by OI&T and permission has been granted to purchase (see supporting email). Care Transitions Portal uses an MDWS interface to VISTA and uses Remote Procedure Calls already defined in VISTA (RPC Broker calls). At least 3 other technologies were reviewed. This one has the most complete discharge instructions that meet the needs of our veterans. There is no existing technology for a discharge planning booklet in the VA. It is the only standard that integrates with the CPRS Chart.

6. TRAINING

Contractor shall provide comprehensive training on the use of the AHCP software being purchased for all staff members that are to use this software. At a minimum, training shall include, but not be limited to technical and operational training. Instruction/user guides shall be provided for both training and technical and operational usage.

Training shall occur at the G.V. “Sonny” Montgomery VA Medical Center, 1500 E. Woodrow Wilson Dr. Jackson, MS. 39216, via on-line, instructor driven classes during normal business hours, 8:00am-4:30pm, Monday through Friday. Training dates and times shall be prescheduled and preapproved by the contracting officer’s representative prior to initiation.
