Equipment must include 4K surgical imaging platforms, various scopes and instruments, video routing systems, mobile carts, monitors, light sources, insufflators, and related accessories as detailed in Attachment A. The contractor must provide on-site initial training and follow-up training within 14 days of request, preventive maintenance per manufacturer specifications, corrective maintenance with 4-hour response during business hours and 24-hour response after hours, loaner equipment if repairs exceed 48 hours (with $200/day liquidated damages for non-compliance), and integration with VA clinical documentation systems including Vista/CPRS workflows and DICOM support. All equipment must be new, FDA-cleared, and compliant with VA, NFPA 99, and IEC 60601-1 standards, with contractor maintaining ≥98% uptime and providing manufacturer-certified technicians. The contractor must be an authorized reseller or manufacturer with hospital-grade endoscopy support experience and appropriate insurance coverage.

5) Does the "Mediastinoscopy equipment” include camera/scope unit as well as instruments? Mediastinoscopy equipment often refers to the scope and instruments but may include the camera/video system depending on the manufacturer/supplier and the specifics of the quote or contract. Just the Scope.
STATEMENT OF WORK (SOW)
Endoscopy Equipment Lease G.V. (Sonny) Montgomery VA Medical Center Jackson, Mississippi
1. Purpose
The purpose of this requirement is to lease new, state-of-the-art Endoscopy equipment for the Operating Room (OR), ENT Clinic, and Urology Clinic at the G.V. (Sonny) Montgomery VA Medical Center (GVSMVAMC). The lease includes delivery, installation, integration, training, preventive maintenance, corrective maintenance, and full-service support.
2. Background
The medical center’s existing ENT scopes, Urology scopes, and OR endoscopy/visualization equipment require replacement due to age, reduced imaging quality, and lack of compatibility with current VA clinical documentation systems. New equipment is necessary to ensure safe, efficient, and modern surgical capability.
3. Scope of Work
The Contractor shall furnish new endoscopy systems covering the Operating Room, ENT Clinic, and Urology Clinic. Scope includes delivery, installation, integration, staff training, preventive and corrective maintenance, loaner equipment, manufacturer-certified service support, and all accessories, cabling, and routing components. Equipment will be inspected and accepted by Biomedical Engineering (HTM).
4. Equipment Requirements by Clinical Area
The Contractor shall provide all equipment identified for OR, ENT, and Urology in the quantities and categories specified in Attachment A – Salient Characteristics.
5. Contractor Responsibilities
5.1 General Responsibilities
The Contractor shall provide all labor, equipment, parts, materials, tools, and supervision required to fulfill this lease requirement. All equipment must be new, FDA-cleared, medical-grade, and compliant with VA and NFPA safety standards. Maintenance must follow manufacturer recommendations, and all replacement parts shall be OEM or OEM-equivalent. All technicians must be manufacturer-certified.
6. Installation Requirements
The Contractor shall coordinate installation with the VA, deliver, unpack, assemble, install, and configure all equipment. Installation documentation, schematics, and compliance certificates must be provided. All systems must integrate with existing VA routing equipment, imaging systems, capture devices, and video-to-chart workflows. DICOM support and compatibility with Vista/CPRS workflows are required.
7. Training Requirements
The Contractor shall provide on-site initial staff training, follow-up training within 14 days of VA request, and training on equipment operation, cleaning, safety, and troubleshooting.
8. Service & Maintenance Requirements
8.1 Preventive Maintenance
Preventive maintenance shall follow OEM specifications and shall be coordinated with Biomedical Engineering.
8.2 Corrective Maintenance
The Contractor shall provide all repairs, labor, parts, and travel at no additional cost. Response time shall be 4 hours during business hours and 24 hours after hours or during emergencies.
OEM-equivalent parts must be used for all repairs.
8.3 Loaner Equipment
If equipment is inoperable for more than 48 hours, the Contractor shall provide equivalent loaner equipment at no additional charge. Liquidated damages of $200 per day may be applied if loaners are not provided within the required timeframe. All removed equipment must be returned or replaced within 30 days, or the Government may terminate the contract for default.
9. Integration & IT Requirements
The Contractor shall support DICOM image and video export, provide compatibility with Vista/CPRS workflows, and supply integration specifications for future EMR transitions.
Systems must integrate with existing routing infrastructure and video documentation workflows.
VA IT staff will support internal network tasks.
10. Performance Requirements
All equipment must meet VA, FDA, ISO, IEC 60601-1, and NFPA 99 standards, pass Biomedical Engineering inspection, maintain ≥98% uptime, and integrate seamlessly with OR routing and documentation systems.
11. Period of Performance
Base Year: July 1, 2026 – June 30, 2027 Option Year 1: July 1, 2027 – June 30, 2028 Option Year 2: July 1, 2028 – June 30, 2029
12. Deliverables
The Contractor shall provide installation documentation, training documentation, service logs upon request, and any required reporting for covered articles subject to FASCSA orders.
13. Contractor Qualifications
The Contractor shall be an authorized reseller or manufacturer of medical-grade endoscopy equipment and must have hospital-grade endoscopy support experience. Technicians must be licensed or certified as required and must be manufacturer-trained. Contractor shall maintain liability, vehicle, and workers’ compensation insurance.

15. Evaluation Criteria
Award will be made using Lowest Price Technically Acceptable (LPTA). Quotes will be evaluated for technical compliance with the SOW and Attachment A, total evaluated price, past performance, and the ability to meet required timelines.
16. Special Contract Considerations
Work outside the approved scope shall not be performed without written approval from the Contracting Officer. Changes will be issued via SF-30 in accordance with FAR 52.243-1.
Non-compliance may result in withheld payment or termination for default.
