No. Question Answer 1 The SOW references water purification systems, however, they are not included in the inventory list-should they be covered?

No. All references to water purification systems have been removed from the SOW (See Revision 1 posted with this amendment).

2 If water purification systems are on the inventory, how are filters and/or UV lamps handled? Are they consumable and billed back to the VA? Does the contract provide 1 set per year as part of the PM and if use requires more frequent changing, then extra sets are billed back?

See answer to question 1.

3 Are accessories/consumables such as batteries and mattress pads included?

No.

4 Will there be a designated workspace?

Yes, an area will be designated after award.

5 The RA Medical Pharos EX-308 Laser was purchased by Strata Skin Science on August 2021 and is no longer supported with no parts availability direct from the OEM. This laser is based on 15–20-year-old technology and should be removed from service.

This item has been deleted from the list of equipment and does not require coverage.

6 What is the process for obsolete/End of Life beds or stretchers when it comes to parts availability?

If/when services/repairs cannot be performed due to the unavailability of discontinued parts, the specific piece of equipment will be removed from service and an equitable price adjustment made to the contract.

7 Are just the lasers considered covered and not any laser delivery systems, such as, slit lamps, head pieces, hand pieces, scanning hand pieces, microscope attachments, etc.?

Yes, just the lasers themselves are to be covered, not delivery systems.

No. Question Answer 8 Is there a particular format that the past performance should be provided in? Please provide clarification as the evaluation criteria (page 23) includes past performance, but the required response sections (Special Instructions) do not include any mention of past performance.

Past performance references are not required. The Government intends to review performance records available in the Government’s Contractor Performance Assessment Report System (CPARS).

9 Are vendors permitted to provide additional response content (e.g.

technical approach)?

Yes.

10 Is there a page limit for the RFQ response?

No.

11 For high dollar repair parts within scope, what is the acceptable failure rate?

There is no “acceptable” failure rate for repairs/parts within scope. If a repair, or a replaced part fails, it will continue to be repaired/replaced until returned to a fully functional condition.
