(Sonny) Montgomery Veterans Affairs Medical Center (GVSMVAMC) and courier services between the G.V. (Sonny) Montgomery Veterans Affairs Medical Center and MS Baptist Health Systems Laboratory, located at 1225 N. State St, Jackson, MS 39202; G.V (Sony) Montgomery Veterans Affairs Medical Center and MS State Health Department, located at 570 E Woodrow Wilson Avenue, Jackson, MS 39216; G.V. (Sonny) Montgomery Veterans Affairs Medical Center and Harbor House Chemical Dependency Center, located at 5354 I-55 South Frontage Rd, Jackson, MS 39212; and between G.V. (Sonny) Montgomery Veterans Affairs Medical Center and the University of MS Medical Center, located at 2500 N. State St, Jackson, MS 39216. Services shall be provided daily as needed and/or after hours, to include weekends, as described below. Items to be transported include patient records, forms, STAT laboratory specimens and laboratory supplies that can fit within a half-ton panel van or car trunk as required. This service will be of one type: “As-needed trips” and/or after hours, to include weekends, as described below. Services to are to be provided for the Base period with four (4) 12-month option periods depending upon a continued need and availability of funds. Requested services are needed because the G.V. (Sonny) Montgomery Veterans Affairs Medical Center lacks the staff and transportation required to perform this type of service after-hours.

2. The vendor will be contacted via telephone when services are required. If services are to be provided, notification will normally be made within 15 minutes of the required arrival time on the same day that service is requested. The 15-minute expected arrival time is not meant to put a driver in any situation that may cause the unsafe operation of a vehicle.

3. The GVSMVAMC will label, package and send specimens (room temperature, refrigerated, or frozen) based on the testing laboratory’s requirements.

Specimens will be shipped from following location:

G.V. (Sonny) Montgomery VA Medical Center 1500 East Woodrow Wilson Drive Jackson, MS 39216 Specimen Mail Out (current Location is basement, room: F-28) OR Administrative Officer of the DAY (AOD) desk located in the Emergency Department (ED)

4. Performance Requirement shall be as follows:

a) As needed trips and/or after-hours trips between GVSMVAMC and other medical centers and facilities throughout the Jackson Metropolitan area, to include weekends. The courier service will be on an as needed basis and/or after-hours and will be coordinated through Logistics Service. Should this service be needed, the courier will be notified by the Contracting Officer Representative (COR), or other specified Logistics Service employees designated to authorize trips. Both the contractor and COR are to keep an accurate total count of all stops made during this contract so invoices can be correctly certified. Unscheduled trips that are for the delivery of lab specimens in direct support of patient care, along with trips to and from other labs and medical centers need not be pre-approved. All other non-scheduled trips need pre-approval as explained above. In the event the courier service is made after normal business hours, the service authorizing the run is responsible for informing Logistics Service the next business day.

b) Service includes transporting those items listed in paragraph 1 above between the points described in paragraph 1 above. There are two designated pick-up points at the GVSMVAMC. Flexibility in actual arrival and departure times must be given to the Contractor who must face the many hazards and differing road conditions that exist on the roadways. Likewise, because there may be times the pick-up points are unable to have the transportable goods ready at the designated time, the Contractor must exercise flexibility in their departures, which may put the courier behind schedule.

e) The Contractor shall pick-up patient records, forms, STAT laboratory specimens and laboratory supplies (weighing under 20 pounds) that can fit within a half-ton panel van or car trunk as required by GVSMVAMC and delivered to the specified destination.

Contract GRAND TOTAL

For the purposes of the award of this Contract, the Government intends to evaluate the option to extend services under FAR 52.217-8 as follows:

The evaluation will consider the possibility that the option can be exercised at any time and can be exercised in increments of one to six months, but not for more than a total of six months during the life of the contract. The evaluation will assume that the prices for any option exercised under FAR 52.217-8 will be at the same rates as those in effect under the contract at the time the option is exercised. The evaluation will therefore assume that the addition of the price or prices of any possible extension or extensions under FAR 52.217-8 to the total price of all proposals relative to each other and will not affect the ranking of proposals based on price, unless, after reviewing the proposals, the Government determines that there is a basis for finding otherwise. This evaluation will not obligate the Government to exercise any option under FAR 52.217-8.
