About this file

This document is an amendment to a solicitation for onsite dermatologist services. The amendment provides answers to questions from potential offerors. The solicitation is for fee basis dermatology providers to staff dermatology clinics at a Department of Veterans Affairs medical center. Clinics are held daily except Tuesdays and Fridays, seeing an average of 30-40 patients per morning clinic staffed by 3-4 residents. Afternoon clinics see up to 20 new patients each and are staffed by 2 residents. The incumbent has 6 fee basis dermatologists covering various clinic times. Support staff includes an APN, RN, LPN and schedulers. Residents rotate monthly with 3-4 in morning clinics and 2 in afternoon clinics. Dermatology does not perform inpatient admissions. Common cases include skin biopsies, excisions and wide local incisions, with Mohs procedures outsourced. The amendment must be signed and returned with proposal submissions. The hour and date for receipt of offers remains unchanged.

Amendment: 0001 Questions and Answers Solicitation: 36C25623Q1320 Dermatology Services

Question: On average, how many patients per day will the Dermatologist see?

Answer: We have clinic every day except for Tuesday and Friday afternoons. We typically see 30-40 patients in the AM clinics between 3-4 residents (8-12 patients each based on PGY level). Wednesday is the surgery/procedure morning, so that morning clinic has 5-10 total patients in it (currently staffed by a General Surgeon). Afternoon clinics are the new patients and the cap is 20 per ½ day. There are 2 residents in each PM clinic. The CCC clinic is on Wednesday afternoons and it’s typically 5-6 patients (a separate upper level resident does this clinic). Thursday afternoons is the Attending clinic where a resident (separate from the consult clinic ones) rounds with Dr. Wong on his research patients.

Question: What are the most common cases/procedures? Skin cancers.

Answer: Shave, punch biopsies. ED&C. On Wed AM, wide local incision. We outsource for Moh’s.

Question: How many other Dermatologists on staff?

Answer: None on staff. We have 6 fee basis providers who cover various times in clinic.

Question: What other support staff is available?

Answer: APN in clinic, full clinic staff-RN, LPN, schedulers

Question: How many residents will the Dermatologist oversee at one time?

Answer: 4 residents rotate on a monthly basis. There are 3-4 residents each morning and 2 residents each afternoon (except Tue and Fri). On Wed afternoon, CCC clinic gets one additional resident. On Thursday afternoon, Dr. Wong gets one additional resident.

Question: How many inpatient admissions does the Dermatologist perform on average?

Answer: Dermatology does not admit. They are an inpatient consult service. If a dermatology patient needs admission, medicine would admit the patient and consult dermatology.
