4.4. DELIVERABLES:
4.4.1. Set-up cell saver equipment and perform pre-operative (pre-op) activates as required by the operating room (OR) schedule.
4.4.2. Perform cell saver operations, which includes:
Step 1: Suction: The blood is sucked away from the operative site via a dual-limen tube, which mixes the blood immediately with the anticoagulant into a custom-made reservoir.
Step 2: Filtration: The contents of the reservoir are filtered to remove large closets and debris. Most collection reservoirs have filters in the 40-150 um range.
Step 3: Separation: The remaining volume is drawn into a centrifuge to be processed. Sterile isotonic saline solutions is pumped into the centrifuge bowl. The force supplies by the centrifuge holds the more dense RBCs against the outer wall of the bowl. The less dense plasma moves towards the center of the bowl where it spills over into a waste bag. Almost on-third of red cell volume may be lost in the scavenging device during processing, depending on the type of device in use. Photo-optics are used to detect the maximum cell density and then to initiate the washing of the packed suspension with a selected volume of saline.
Step 4: Disposal: Waste products, including white blood cells, platelets, plasma, anticoagulant, fat, clotting factors, and free plasma hemoglobin are collected in a bag and are disposed of as clinical waste.
Step 5: Cell salvage: Packed RBCs are collected in a separate bag. The quality of the collected red cells depends upon the volume of wash solutions used, the degree of concentration achieved and the quality of the blood before washing, the type of surgery, and the presence of residual contaminates.
Step 6: Reinfusion: The collected red cells can be reinfused immediately or for up to four (4) hours after processing if kept at room temperature. If citrate is used as an anticoagulant, rapid liver metabolism makes citrate toxicity unlikely, but in compromised liver function, correction with small doses of calcium (10 ml of 10% calcium gluconate) provides immediate and non-toxic reversal. It is recommended that the standard label should be used for the cell-salvaged blood. Labelling of autologous salvaged red cells is as important as the labelling of allogeneic (donor) units.

4.7.5. Registration with Contractor Performance Assessment Reporting System (CPARS and Employment Eligibility Verification (E-Verify)
4.7.5.1. As prescribed in Federal Acquisition Regulation (FAR) Part 42.15, the Department of Veterans Affairs (VA) evaluates Contractor past performance on all contracts that exceed the Simplified Acquisition Threshold and shares those evaluations with other Federal Government contract specialists and procurement officials. The FAR requires that the Contractor be provided an opportunity to comment on past performance evaluations prior to each report closing. To fulfill this requirement VA uses an online database, CPARS, which is maintained by the Naval Sea Logistics Center in Portsmouth, New Hampshire. CPARS has connectivity with the Past Performance Information Retrieval System (PPIRS) database, which is available to all Federal agencies. PPIRS is the system used to collect and retrieve performance assessment reports used in source selection determinations and completed CPARS report cards transferred to PPIRS. CPARS also includes access to the federal awardee performance and integrity information system (FAPIIS). FAPIIS is a web-enabled application accessed via CPARS for Contractor responsibility determination information.
4.7.5.2. Each Contractor whose contract award is estimated to exceed the Simplified Acquisition Threshold requires a CPARS evaluation. A government Focal Point will register your contract within thirty days after contract award and, at that time, you will receive an email message with a User ID (to be used when reviewing evaluations). Additional information regarding the evaluation process can be found at www.cpars.gov or if you have any questions, you may contact the Customer Support Desk @ DSN: 684-1690 or COMM: 207-438-1690.
4.7.5.3. For contracts with a period of one year or less, the contracting officer will perform a single evaluation when the contract is complete. For contracts exceeding one year, the contracting officer will evaluate the Contractor’s performance annually. Interim reports will be filed each year until the last year of the contract, when the final report will be completed. The report shall be assigned in CPARS to the Contractor’s designated representative for comment. The Contractor representative will have sixty (60) days to submit any comments and re-assign the report to the CO.
4.7.5.4. Failure for the Contractor’s representative to respond to the evaluation within those sixty (60) days, will result in the Government’s evaluation being placed on file in the database with a statement that the Contractor failed to respond; the Contractor’s representative will be “locked out” of the evaluation and may no longer send comments.
4.7.5.5. Employment Eligibility Verification. The contractor shall enroll in the E-Verify program within 30 calendar days of contract award at http://www.dhs.gov/E-Verify gov/E-Verify. For each employee assigned to the contract, initiate verification within 90 calendar days after date of enrollment or within 30 calendar days of the employee’s assignment to the contract, whichever date is later. If the contractor is enrolled as a Federal Contractor in E-Verify at time of contract award, the contractor shall use E-Verify to initiate verification of employment eligibility of all new employees. The contractor shall log into the E-Verify program at least every three months after contract award to maintain employment eligibility of all employees. See E-verify Job Aid attached at section D3.
