B.2 STATEMENT OF WORK
Scope of Work:
VISN 16 medical centers have a requirement for services and management of Drawing / Space / Capital Asset Inventory (CAI) database and reporting functions, with graphical tools to store and share architectural engineering drawings, assign space, and report usage among organizational groups. It shall be a multi-user, multi-tasking stand-alone system supporting multiple users in organizations spanning many buildings and sites.
Due to the requirements for on-site activities, the vendor shall be required to make physical visits to the VA Healthcare System locations.
Drawing / Space / CAI Reporting - Database Management:
There are three mandatory vendor tasks:
1. Survey, field verify and update architectural drawings
2. Build a Capital Asset Inventory (CAI) database and reporting system
3. Update and maintain the drawings and the databases on an on-going basis
Task 1. Field verify and update architectural drawings The Healthcare System consists of a total built-up square footage of approximately 7,974,000 square feet - see attachment A. The architectural drawings were developed in AutoCAD, BIM, PDF or other electronic formats some time back and since then projects have taken place affecting certain areas of certain buildings. There were two types of projects: (1) minor projects that were done in-house for which project drawings may not be available, and (2) major remodeling projects for which a design firm was utilized. These design firms and contractors may have supplied the medical centers with design and as-built drawings and the medical centers will make all drawings available to the vendor. The VA cannot guaranty the accuracy of these drawings and therefore the vendor may need to conduct a field survey to capture today’s layouts. In planning for this survey, the vendor must be aware that some areas may not be readily available to be surveyed and they must plan in consultation with the occupying service line and conduct the surveys when it is convenient for the service line. Any field survey site visits must be coordinate with the COR and the COR’s delegate at the specific station (POC). These site visits must be planned at least three weeks in advance and confirmed with the COR and station POC. Subsequent to the field survey, the vendor shall update all CAD drawings. In updating the CAD drawings, the vendor shall comply with VA CAD standards. The drawings shall be organized in the form of one drawing per contiguous floor per building wherever it is possible.
These drawings are the property of the government and updating and manipulation of these drawings under this contract do not confer any rights to the contractor.
Task 2. Build a Space Management Capital Asset Inventory (CAI) database and reporting system The Healthcare System requires a space management system that can track the assignment of all spaces and capable of generating reports including Capital Asset Inventory (CAI) reports. The space database shall, as a minimum, contain the following pieces of data:
1. Room number
2. Space function
3. Service line assignment
4. CAI Space Driver assignment
5. Net and Gross Square footages
The medical centers will provide to the vendor the available information on this data. However, the vendor shall conduct an escorted walkthrough of the buildings to capture current information from every room. This information includes overall dimensions, square footages, and service ownership. In planning for this survey, the vendor must be aware that some areas such as pharmacies, OR’s, and certain patient areas may not be readily available to be surveyed and they must plan in consultation with the occupying service line to conduct the surveys only when it is convenient for the service line. Critically sensitive areas such as pharmacies and IT server rooms must be identified on the drawings. The vendor will work with the COR and Information Security Officers to ensure compliance with this requirement; however, it is still expected that these spaces and associated square footages will be reported accurately within the system. The vendor will identify a secure method to accomplish this requirement.
The database must be able to support multiple users simultaneously. It shall be able to assign privileges to users in such a manner that only certain users will be able to modify data whereas all users will be able to view the data.
Task 3. Update and Maintain the Drawings and The Databases:
This includes Alexandria, Fayetteville, Houston, Jackson, Little Rock (both campuses), New Orleans and Shreveport. Ongoing additions and renovations require changes to drawings to maintain the integrity of the drawing database. Also, the medical centers may expand, or contract as new facilities are added, or existing facilities are closed. The VA will require that these changes be incorporated in the existing drawings so that the drawings in the system will be current. The frequency and extent of such changes will depend on completion of projects and may be unpredictable.
Periodically, the medical centers will send to the vendor drawings and other documentation that indicate the alterations in certain areas. Typically, these changes will be forwarded as AutoCAD files. The vendor shall, upon receipt of such documents, consult with the medical center staff and clarify the changes, as necessary. The vendor shall incorporate the changes in the drawings and update all relevant modules. The vendor shall make the changes in drawings in a reasonable amount of time – less than fifteen (15) business days. The VA may decide to add additional buildings or suites to the system. The size and locations of these are not known now. When such projects occur, the contractor shall add such new buildings on to the system.
The vendor shall at least once a quarter send a notification to each medical center reminding the medical center and asking for data and drawing changes that may have taken place since the previous update.

Other Terms and Conditions Some field verification work will be done on site at the medical centers and shall be coordinated with the Contracting Officer Representative (COR) appointed by the CO. Generally, work will be done during normal business hours (M-F 8 to 4:00), holidays excluded.
All drawings and data produced by the vendor will become the property of the government. PDF & AutoCAD format drawings shall be provided to each station upon completion of development. Upon contract expiration, the vendor will be required to destroy all data associated with the facility drawings and data sets and verify the method of destruction with the VA COR & Information Security Officer.
Contractor Security Requirements The Contractor’s employees shall bring photo ID to obtain a VA Contractor identification badge. The COR will identify personnel who will escort and coordinate access to the areas of work. All contractors, subcontractors, and third-party servicers and associates working with VA information are subject to the same investigative requirements as those of VA appointees or employees who have access to the same types of information. The level and process of background security investigations for contractors must be in accordance with VA Directive and Handbook 0710, Personnel Suitability and Security Program. The Office for Operations, Security, and Preparedness is responsible for these policies and procedures.
Quality Assurance Plan Performance will be measured by vendor timeliness in responding to service requests, and the quality of the maintained drawings and database. The facilities drawings created by the vendor shall be compliant with VA standards and be compatible with generally accepted industry practices. When the vendor creates new facility drawings the vendor shall submit a preliminary set of drawings for review by the medial center staff. The vendor shall hold in person or on-line discussions to discuss the review findings and make agreed-upon modifications.
The facilities related data such as occupancy by service lines collected or validated by the vendor must be reviewed by medical center staff and the vendor shall make all agreed-upon changes. When the medical centers send out existing drawings for updating to the vendor, vendor shall acknowledge the receipt within seven days of receiving them. Within the next seven days the vendor shall hold discussions with the VA staff - if the vendor has questions or requires clarifications. Once the clarifications are done, the vendor shall update the drawings and load them on to the platform within a further period of 15 days.
The quality of the images maintained in the vendor’s database shall be legible and high quality.
The vendor shall make all appointments scheduled with the COR on time and without delay.
Attachment A Facilities to be included in the Scope of Work.

Some buildings have interstitial spaces, and the above list may not include such spaces. The contractor shall include those spaces as well in the system to be developed under this contract. However, the interstitial spaces shall be specified in the system in such a way that they can be separately treated for analysis and reporting.

SECTION C - CONTRACT CLAUSES
C.1 ADDENDUM to FAR 52.212-4 CONTRACT TERMS AND CONDITIONS—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES Clauses that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. The following clauses are incorporated into 52.212-4 as an addendum to this contract:
C.2 VHA SUPPLEMENTAL CONTRACT REQUIREMENTS FOR ENSURING ADEQUATE COVID-19 SAFETY PROTOCOLS FOR FEDERAL CONTRACTORS
1. Contractor employees who work in or travel to VHA locations must comply with the following:
a. Documentation requirements:
1) If fully vaccinated, shall show proof of vaccination.
i. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination.
2) If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor’s order or an FDA approved over-the-counter test.
3) Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel.
4) Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.
2. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates.
a. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.
3. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.
C.3 52.228-5 INSURANCE—WORK ON A GOVERNMENT INSTALLATION (JAN 1997)
(a) The Contractor shall, at its own expense, provide and maintain during the entire performance of this contract, at least the kinds and minimum amounts of insurance required in the Schedule or elsewhere in the contract.
(b) Before commencing work under this contract, the Contractor shall notify the Contracting Officer in writing that the required insurance has been obtained. The policies evidencing required insurance shall contain an endorsement to the effect that any cancellation or any material change adversely affecting the Government's interest shall not be effective—
(1) For such period as the laws of the State in which this contract is to be performed prescribe; or
(2) Until 30 days after the insurer or the Contractor gives written notice to the Contracting Officer, whichever period is longer.
(c) The Contractor shall insert the substance of this clause, including this paragraph (c), in subcontracts under this contract that require work on a Government installation and shall require subcontractors to provide and maintain the insurance required in the Schedule or elsewhere in the contract. The Contractor shall maintain a copy of all subcontractors' proofs of required insurance and shall make copies available to the Contracting Officer upon request.

C.4 SUPPLEMENTAL INSURANCE REQUIREMENTS
In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract:
(a) Workers' compensation and employers’ liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.
(b) General Liability: $500,000.00 per occurrences.
(c) Automobile liability: $200,000.00 per person; $500,000.00 per occurrence and $20,000.00 property damage.
(d) The successful bidder must present to the Contracting Officer, prior to award, evidence of general liability insurance without any exclusionary clauses for asbestos that would void the general liability coverage.

(a) This solicitation includes VAAR 852.219-10 VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside.
(b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. (a) Pursuant to 38 U.S.C. 8127(k)(2), the offeror certifies that—
(1) If awarded a contract (see FAR 2.101 definition), it will comply with the limitations on subcontracting requirement as provided in the solicitation and the resultant contract, as follows:

The offeror further acknowledges that this certification is subject to Title 18, United States Code, Section 1001, and, as such, a false, fictitious, or fraudulent certification may render the offeror subject to criminal, civil, or administrative penalties, including prosecution.
(3) If VA determines that an SDVOSB/VOSB awarded a contract pursuant to 38 U.S.C. 8127 did not act in good faith, such SDVOSB/VOSB shall be subject to any or all of the following:
(i) Referral to the VA Suspension and Debarment Committee;
(ii) A fine under section 16(g)(1) of the Small Business Act (15 U.S.C. 645(g)(1)); and
(iii) Prosecution for violating section 1001 of title 18.
(b) The offeror represents and understands that by submission of its offer and award of a contract it may be required to provide copies of documents or records to VA that VA may review to determine whether the offeror complied with the limitations on subcontracting requirement specified in the contract. The Contracting Officer may, at their discretion, require the Contractor to demonstrate its compliance with the limitations on subcontracting at any time during performance and upon completion of a contract if the information regarding such compliance is not already available to the Contracting Officer. Evidence of compliance includes, but is not limited to, invoices, copies of subcontracts, or a list of the value of tasks performed.
(c) The offeror further agrees to cooperate fully and make available any documents or records as may be required to enable VA to determine compliance with the limitations on subcontracting requirement. The offeror understands that failure to provide documents as requested by VA may result in remedial action as the Government deems appropriate.
(d) Offeror completed certification/fill-in required. The formal certification must be completed, signed, and returned with the offeror’s bid, quotation, or proposal. The Government will not consider offers for award from offerors that do not provide the certification, and all such responses will be deemed ineligible for evaluation and award.

C.8 VAAR 852.237-75 KEY PERSONNEL (OCT 2019)
The key personnel specified in this contract are considered to be essential to work performance. At least 30 days prior to the Contractor voluntarily diverting any of the specified individuals to other programs or contracts the Contractor shall notify the Contracting Officer and shall submit a justification for the diversion or replacement and a request to replace the individual. The request must identify the proposed replacement and provide an explanation of how the replacement’s skills, experience, and credentials meet or exceed the requirements of the contract. If the employee of the Contractor is terminated for cause or separates from the contractor voluntarily with less than thirty days notice, the Contractor shall provide the maximum notice practicable under the circumstances. The Contractor shall not divert, replace, or announce any such change to key personnel without the written consent of the Contracting Officer. The contract will be modified to add or delete key personnel as necessary to reflect the agreement of the parties.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form.
