3. CHANGE #3 FROM:
3.1 Email/Volume 1 - Technical Subfactor 1a- Personnel Qualifications/Staffing (provide what is requested in Addendum to 52.212-2 Paragraph 2.1.1 for staffing according to [Paragraph 2.1.1.1 Optometrist(s)] and [Paragraph 2.1.1.2 Ophthalmology Health Technician/Certified Ophthalmic Assistant (COA)].
CHANGE #3 TO:

4. CHANGE #4 FROM:
2.3 FACTOR 3 – Past Performance: Offerors shall submit the information outlined below as evidence of relevant past performance. Each offeror will be evaluated on his/her performance under existing and prior contracts for similar Optometrists and COA staffing services. Performance information will be used for both responsibility determinations and as an evaluation factor against which offeror’s relative rankings will be compared to assure best value to the Government. The Government will focus on information that demonstrates quality of performance relative to the size and complexity of the procurement under consideration.
The Offeror shall provide three (3) Contractor Performance Assessment Reporting System (CPARS) reports from previous or ongoing contracts, for the same or similar services called for in this solicitation, provided to the Department of Veterans Affairs or other Federal agencies, performed within the past five (5) years.

Furthermore, reports that are directly related to providing Optometrists and COAs will be considered as “SAME”, will be rated higher, and considered more relevant than those that are “similar”, meaning that they are related to providing staffing or not directly related to these positions sought.
If Offeror has no access to Contractor Performance Assessment Reporting System (CPARS) (https://www.cpars.gov ), or has problems obtaining reports, then the Offeror shall at a minimum, provide contract/task order numbers, name of Contracting Officer and/or COR, and email address of CO/COR and provide brief explanation that offeror was unable to obtain actual reports.
