About this file
This is an amendment (Amendment 0002) to a solicitation for fire, smoke, and automatic door inspection, maintenance, and repair services at the Dwight D. Eisenhower Veterans Affairs Medical Center (VAMC) in Leavenworth, Kansas. The amendment extends the quotation deadline to Tuesday, November 10, 2026, at 3:00 PM Central Standard Time, with submissions to be made via email to michael.murphy7@va.gov. The solicitation (36C25526Q0617) is issued by the Department of Veterans Affairs Network Contracting Office (NCO) 15 and is for a total small-business set-aside Indefinite Delivery/Indefinite Quantity (IDIQ) contract covering five ordering periods from November 1, 2026, through October 31, 2031.
The amendment revises multiple SOW provisions and incorporates 54 vendor questions with government responses addressing inventory quantities, door identification, inspection procedures, billing methodology, personnel qualifications, subcontracting limitations under FAR 52.219-14, wage determinations, asbestos precautions, and Infection Control Risk Assessment (ICRA) requirements. Key changes include: updating the manual fire/smoke door inventory from 124 to 126 assemblies with 252 estimated inspections per ordering period; specifying that the first inspection round must begin within 30 calendar days of award and be completed within 60 calendar days; clarifying that X001/X002 unit prices include minor hardware replacement, durable identification tags, and permanent AAADM inspection labels; and establishing that X001 and X002 inspections may only be invoiced after the full semiannual inspection round is complete and accepted. The amendment also clarifies that electrified locking hardware and access-control components are excluded from scope, addresses return-trip billing without a second one-hour minimum, requires inspection reports within 10 working days of completing the full semiannual round, and defers final determinations on wage determination revision, federal equivalent rates, fire door inspector certification equivalency, and ICRA containment pricing conditions to a subsequent amendment before quotation closure.

DRAFT FOR CO REVIEW | NOT ISSUED / NOT FOR POSTING
SF 30 Block 14 Continuation Solicitation 36C25526Q0617 | Amendment number: [CONFIRM NEXT NUMBER] | Planned issue date: October 30, Working-copy basis: 59-page “Fire Smoke Auto Door IDIQ RFQ - FINAL” and available facility correspondence.
Verify against the actual issued RFQ and prior amendments before release. This working copy does not establish final clearance.
1. Quote deadline Replace the quotation deadline wherever stated with Tuesday, November 10, 2026, 3:00 PM CST. Submit by email to michael.murphy7@va.gov. The submission method and existing volume/file requirements otherwise remain unchanged.
2. Manual inventory and quantities In B.2 and SOW paragraph 2, change 124 manual fire/smoke assemblies to 126. Change estimated quantities for 0001, 1001, 2001, 3001 and 4001 from 248 to 252 inspections per ordering period. Building 122 manual item 20 is three assemblies. No change is made to the 57 automatic-door count or 114 scheduled inspections per period.
3. Door identifiers Automatic items 3 and 4 shall be identified as AD26-1 and AD26-2, respectively, retaining their listed locations and separate schedule entries. Item 34, Surgery Entrance, asset D0047232, is AD49. Item 40, Building 122 NW Entrance, asset D0047234, has no confirmed physical tag and shall be identified by schedule item, asset number and location pending assignment of a verified unique tag. Item 41, Building 122 Main Interior, asset D0047235, is AD56. Item 42, Building 122 Main Exterior, asset D0047236, retains AD57; AD57 shall not be assigned to item 40.
No door is removed or combined by this correction.
4. First inspection round Add to SOW paragraphs 4.1 and 5.1: The first round shall begin within 30 calendar days after contract award and be completed within the following 30 calendar days, no later than 60 calendar days after award, unless the task order expressly specifies a different start date and corresponding schedule. Work must be ordered before performance.
5. Manual-door minor hardware Replace SOW paragraph 4.3: Minor adjustment, lubrication and replacement of screws, fasteners, gasketing, smoke seals, astragals, silencers and closer arms, including associated labor, are included in the X001 unit price and are not separately billable under repair CLINs.
6. Automatic-door included work and exclusions Replace SOW paragraph 5.3 with: “The X002 unit price includes parts and associated troubleshooting, adjustment, repair, installation and replacement labor for normal-wear malfunctions identified during a scheduled inspection, including sensors, activation devices, controls, belts, batteries and similar components. Excluded are complete replacement of an operator, header assembly or door panel; damage from impact or misuse and other causes beyond normal wear; VA-requested upgrades; and malfunctions reported between scheduled inspections. Separately payable work requires authorization under paragraphs 6 and 7, as applicable. No work included in X002 may be billed separately. Replace AAADM-required signage and affix an AAADM inspection label to each compliant door.”
Add to each CLIN 0002, 1002, 2002, 3002 and 4002 description: “Includes the parts and labor identified in SOW 5.3;
RFQ 36C25526Q0617 | Draft October 29, 2026 | Unsigned its stated exclusions apply. Includes durable identification tags and ANSI/BHMA A156.27 inspection of automatic item 5.”
7. Revolving-door standard and identification tags Add ANSI/BHMA A156.27 for automatic item 5 to SOW paragraph 3. Add permanent durable identification tags during first-round inventory validation to SOW paragraph 2; use the contractor’s standard durable tags and include them in X001/X002 prices. These are separate from required AAADM labels.
8. Return-trip billing Add to SOW paragraph 6.4: The one-hour minimum applies once per service call. Return visits to complete the same repair after parts arrive are billed for actual authorized on-site time in 0.5-hour increments, without a second one-hour minimum. Travel time is not billable.
9. Inspection reporting Replace the opening of SOW paragraph 10.1 with: Within 10 working days after completing the full semiannual inspection round, the Contractor shall deliver the complete report in the formats and with the door-level information otherwise required by this paragraph. SOW paragraph 10.2 repair reports remain unchanged.
10. Acceptance and invoicing Replace B.1 paragraph 3 with: “Invoices shall be submitted monthly in arrears for otherwise completed, accepted and separately authorized services/repairs. For X001/X002, the Contractor shall submit the complete report for the full semiannual inspection round for Government review and acceptance before invoicing that round. Partial monthly inspection acceptance is not authorized. Inspection invoices shall identify accepted assemblies and applicable unit prices; the same work shall not be billed under both an inspection CLIN and a repair/service-call CLIN.” Add to SOW paragraph 11: “The full-round report and invoicing requirements in B.1 paragraph 3 apply to X001/X002. VA plans up to 10 business days to review the complete report and identify acceptance or deficiencies. This review target does not change applicable payment, interest or constructive-acceptance requirements.” In the QASP Performance Requirements Summary, replace the fire/smoke inspection report deadline with “within 10 working days after completion of the full semiannual inspection round.” Add the same full-round trigger for scheduled automatic-door inspection reports. SOW paragraph 10.2 repair-report requirements are unchanged.
11. Asbestos Add to SOW paragraph 12.2: “The facility’s Industrial Hygiene representative reports that the doors covered by this requirement do not contain asbestos. This confirmation is limited to the doors and does not establish that frames, walls or adjacent materials are asbestos-free. Asbestos assessment and abatement are not included in this contract.
Do not drill, cut, sand, grind or otherwise disturb material known or suspected to contain asbestos. If suspect material is encountered, stop the affected work and notify the COR immediately. Do not resume the affected work until VA has evaluated the condition and provided written direction through the appropriate officials. Only the Contracting Officer may authorize a change in contract scope or price.”
12. ICRA and access In SOW paragraphs 8.2 and 12.2, preserve coordination and safety requirements and incorporate Attachment A, Section D, Q19 and Q32. No blanket Surgery/ED exemption, unspecified containment allowance or waiver of required vetting is established. Additional conditions affecting pricing/performance will be issued by amendment.
13. Automatic-door brand experience Add to SOW paragraph 9.1: Documented brand experience may be demonstrated by a work history listing customer, dates, door type and work performed. Current AAADM certification remains required.
14. Technical page count and subcontractor past performance Replace the Instructions paragraph 5 heading with: “Volume I - Technical Capability (10-page maximum, excluding cover page, table of contents, certificates and sample report).” Add: “A cover letter containing technical content counts toward the limit. Past Performance under paragraph 6 is separate and does not count toward the Technical Capability limit.” Add to Instructions paragraph 6 and E.2 Factor 2: “The Government will consider relevant past performance of a proposed subcontractor performing a meaningful part of the work, evaluated for that subcontractor’s proposed role. Identify the subcontractor, its role and which references are its own.”
15. QASP response times In the QASP Performance Requirements Summary, replace the Emergency/Service Call Response performance standard with: “Routine calls received by noon local time on a business day: qualified technician on site the same day before close of business. Calls received after noon, on a weekend or Federal holiday: qualified technician on site by 10:00 AM the next business day. Life-safety/security emergencies: callback within one hour and technician on site within four hours, any time of day. SOW paragraphs 6.2 and 6.3 govern.” No QASP AQL excuses a missed individual contractual response obligation. Retain the stated surveillance/AQL provisions except as expressly amended here.
16. Deferred determinations The applicable wage-determination revision, completed federal equivalent rates, specific NFPA certificate determination, final ICRA/containment pricing conditions and pending-PIV access prerequisites remain deferred. This amendment does not reject the specific certificate, impose a DHI-only qualification requirement, waive required vetting or establish an unspecified containment allowance. Material determinations will be issued by further amendment before quotations close, with additional response time as necessary. All terms not expressly amended remain unchanged.
17. Subcontractor identification and measurement period Replace Instructions paragraph 5(e) with: “List every planned subcontractor, the NAICS code properly assigned to its subcontract, its size status under that code’s size standard, the work it will perform and its estimated share of the amount paid. Explain compliance with FAR 52.219-14. An executed subcontract is not required at quotation submission. For this total small-business set-aside, similarly situated status is determined under the incorporated clause, not automatically by the prime-contract NAICS alone. Under the paragraph (f)(1) fill-in, compliance is determined by the end of each order’s performance period.” The incorporated clause governs; service/repair labor is not excluded merely because it is charged under X005.
18. Access-control exclusion and CLIN standards Add to SOW paragraph 2: “Electrified locking hardware and access-control components, including card readers, electric strikes, magnetic locks and delayed-egress devices, are outside this contract’s service/repair scope.” Existing door operation and fire-alarm interface inspection requirements are retained. Add ANSI/BHMA A156.27 for automatic item 5 to each X002 CLIN standards list. Permanent durable identification tags are included in X001/X002 prices;
required AAADM labels are separate.
19. Freight and fully burdened pricing Add to SOW paragraph 7.1: “Identify any proposed freight separately. Freight is payable only if expressly approved in the CO-issued order and not included in the part price, markup or another charge. Expedited shipping requires justification and CO approval. No separate freight is payable for X001/X002-included parts/work. No separate travel, mileage, trip or mobilization charges are payable.”
20. Price-evaluation arithmetic Replace the first two sentences of E.2(b) with: “For the five ordering periods, total evaluated price includes the sum of the extended prices of each X001, X002, X003 and X004 CLIN and the Government-established $30,000 estimate for each X005 CLIN, counted once per period ($150,000 total for X005 over five periods). The Government will also evaluate FAR 52.217-8 by adding one-half of the complete Ordering Period V total once; that period total includes its X005 estimate once.” The remainder of E.2(b), including price-reasonableness/balance review and no obligation to exercise the extension, is unchanged.
21. Clause cross-reference correction In D.2 paragraph 1, replace “see C.9” with “see C.1” for FAR 52.222-41. The personnel-vetting clause is C.9. Vendor questions in Attachment A retain their original wording; Government responses supply corrected references where needed.
Acknowledgment Acknowledge this amendment and all prior amendments with the quotation, identifying each amendment number and date. Quotations already submitted may be revised before the extended deadline. Except as expressly amended, all other solicitation terms and conditions remain unchanged.
Attachments to this draft Attachment A: consolidated vendor questions and Government responses (54 questions). The historical inspection record and company-only site-visit attendee list are not included in this working copy. Their reviewed release copies and final attachment index remain required before the corresponding release commitments are used.
DRAFT FOR CO REVIEW | NOT ISSUED / NOT FOR POSTING
RFQ 36C25526Q0617: Vendor Questions and Government Responses Fire, Smoke and Automatic Door Inspection, Maintenance and Repair Services, Dwight D. Eisenhower VAMC, Leavenworth, KS
Updated October 30, 2026 with available facility correspondence. Proposed closing: November 10, 2026, 3:00 PM CST. Material deferred answers will be issued by further amendment before quotations close.
Prepared for Michael Murphy, Contracting Officer, NCO 15
Section A: Questions 1-4 Q Question Government Response 1 Personnel (Instructions para. 5(b); SOW para. 9.3): Will the
Government accept a quote that commits to the SOW para. 9 qualification standards and delivers technician names, locations and current certificates to the CO after award and before work begins, without being rated Unacceptable under Factor 1?
No. Instructions para. 5(b) requires technician names, locations, and copies of current AAADM and fire door inspector credentials with the quote, and they are evaluated under Factor 1. SOW para. 9.3 is a separate performance requirement and does not replace the quote requirement.
2 Wage Determination (Section D.2): The RFQ incorporates WD 2015-5105 Revision 27. Revision 30, dated 8/27/2026, is published for the same counties. Should offerors price to Revision 27 or Revision 30?
The wage-determination revision applicable to the revised quotation deadline will be identified by a subsequent amendment. Do not rely on the earlier draft reference to Revision 27 or assume that a vendor-cited revision has been adopted. The Government will allow additional response time if the wage determination changes pricing requirements.
3 Subcontracting (Instructions para. 5(e)): For an offeror with no subcontractor under agreement at the time of quote, is a statement that any post-award subcontractor will be small under NAICS 541350, identified to the CO before it performs, responsive to para. 5(e)?
No. A general statement is not enough. Instructions para. 5(e) requires the quote to identify each planned subcontractor, its size status under the NAICS code assigned to the subcontract, the work it will perform, and its estimated share of the amount paid. An executed subcontract is not required at quote submission.
4 Parts (SOW para. 7.1): May itemized parts quotes under CLIN X005 include a handling markup over supplier invoice cost?
Yes, if reasonable. Prices are fully burdened under B.2.1(g). Each itemized repair quote under SOW para. 7.1 should show the part price, including any markup, and be supported by a manufacturer price list, catalog, or supplier invoice on request. The CO will review parts pricing for reasonableness before issuing any order.
Section B: Questions 1-8 Q Question Government Response 1 Door inventory / CLIN X001 unit (SOW para. 13; B.2): Should a fire/smoke door pair be priced as one assembly or two? Please clarify apparent duplicates (auto doors #3/#4 and #56/#57;
SPD1 Stair 1 twice in Bldg 160; SPB1 Stair 1 twice in Bldg 88/89;
Price each numbered manual fire/smoke-door entry as one assembly, whether single or paired, except Building 122 item 20, which contains three assemblies. The manual inventory is 126 assemblies, with 252 estimated inspections per ordering period. The other questioned
RFQ 36C25526Q0617 | DRAFT FOR CO REVIEW | Not issued
Bldg 90 item 18 repeating Bldg 88 item 1; tag AD54 on two doors) and the count for Bldg 122 item 20, "Corridor Smoke Doors." Will the Government provide a corrected inventory (Excel if available) with leaf counts, ratings, and auto-door operator make/model?
listings are separate doors. Automatic items 3 and 4 remain separate doors, identified as AD26-1 and AD26-2. Item 34, Surgery Entrance, asset D0047232, is AD49. Item 40, Building 122 NW Entrance, asset D0047234, has no confirmed physical tag; identify it by schedule item, asset number and location pending assignment of a verified unique tag.
Item 41, Building 122 Main Interior, asset D0047235, is AD56. AD57 is already assigned to item 42, Building 122 Main Exterior, asset D0047236; do not assign AD57 to item 40. No door is removed or combined. Leaf counts, ratings and operator models beyond those listed are not provided.
2 Historical data: Please identify the current or most recent contractor(s), contract number(s), and approximate annual spend over the last 2-3 years. Do the estimates of 200 normal hours, 40 after-hours hours, and $30,000 in parts per period reflect actual history?
This is a consolidated requirement. Facility staff performed the most recent fire/smoke-door inspection in-house and report that automatic-door repairs have occurred, but supporting prior repair contract numbers and pricing are not currently available for release. No incumbent contract has been identified in the records reviewed for this response. The stated labor-hour quantities and parts allowances are Government estimates for evaluation and planning, not verified historical expenditures.
3 Response and billing (SOW paras. 6.2-6.4; QASP): For a para. 6.3 emergency call placed by VA outside 8:00 AM-4:30 PM, will labor be billed under CLIN X004? Does the SOW para. 6.2 timeline govern over the QASP's "same-day or next-day" wording?
Yes. Authorized emergency service/repair work requested by the Government outside normal hours (Monday-Friday, 8:00 AM-4:30 PM, excluding Federal holidays) is billed at the CLIN X004 rate. Work already included in X001/X002 is not separately billable. SOW paragraphs 6.2 and 6.3 govern response times; the amendment replaces the conflicting QASP response-time wording.
4 Limitations on subcontracting (FAR 52.219-14; Instructions para. 5(e)): Confirm that work performed by a subcontractor that is small under NAICS 541350 is "similarly situated" and counts toward the prime's compliance. Are CLIN X005 parts and materials included in or excluded from the 50% calculation?
FAR 52.219-14 applies. For this total small-business set-aside, a similarly situated entity is a first-tier subcontractor with the same small-business program status that qualified the prime and that is small under the size standard for the NAICS code properly assigned to its subcontract. Work it performs with its own employees does not count against the 50-percent limitation; work it further subcontracts does. Apply the limitation to the services portion, excluding the properly segregated supply portion; include service and repair labor. Under the paragraph (f)
(1) fill-in in the RFQ used for this amendment, compliance is determined by the end of the performance period for each order issued under the contract.
5 Past performance (Instructions para. 6): Will the Government consider the past performance of a proposed major subcontractor that will perform the inspection and repair work?
Yes. The Government will consider past performance of a proposed subcontractor that will perform a meaningful part of the work, judged by its relevance to that subcontractor's proposed role. Identify the subcontractor, its role, and which references are its own. Instructions para. 6 and E.2 are amended accordingly.
6 Wage determination (Sections D.2 and D.4): The RFQ incorporates Revision 27, but SAM.gov shows Revision 30
See Section A, Q2 regarding the wage determination. The completed Statement of Equivalent Rates for Federal Hires will be provided by
(08/27/2026) as current. Which revision applies? Section D.4 is blank. Will it be completed?
amendment. The statement is informational and is not a wage determination.
7 Start-up and access (852.204-72; SOW paras. 4.1, 5.1, 12.1):
When are the first semiannual inspections expected after the estimated 11/1/2026 start? May technicians work under escort while their Tier 1 background investigation and PIV issuance are pending?
The first semiannual inspection round shall begin within 30 calendar days after contract award and be completed within the following 30 calendar days, no later than 60 calendar days after award, unless the task order expressly establishes a different start date and corresponding schedule. Only ordered work is authorized. See Section D, Q32 for access requirements; this schedule does not waive vetting or credentialing requirements.
8 Repair parts pricing (SOW para. 7.1; CLIN X005): May contractor parts quotes include a markup over supplier cost, or should parts be quoted at manufacturer list price or documented cost?
See Section A, Q4.
Section C: Questions 1-7 Q Question Government Response 1 Limitations on subcontracting: Confirm that FAR 52.219-14 applies to this set-aside, and that the prime may subcontract the inspection and repair work to one or more similarly situated small business subcontractors (small under NAICS 541350), with that work counting toward the prime's 50% requirement.
Yes. See Section B, Q4. The prime remains responsible for all performance.
2 Confirm that the cost of repair parts and materials (CLINs X005, $30,000 per period) is excluded when calculating the 50% limitation for services.
The properly segregated supply portion is excluded from the services calculation; service and repair labor is not excluded merely because it is charged under X005. See Section B, Q4 and Section C, Q7.
3 Is this a new requirement, or is there an incumbent contractor?
If there is an incumbent, can the Government share the current contract number and pricing?
See Section B, Q2.
4 Confirm that all five ordering periods (11/1/2026 through 10/31/2031) must be priced, and that offerors may apply annual escalation between periods.
Price every CLIN in all five ordering periods, and leave the Government estimate in each X005 CLIN unchanged (Instructions para. 7(b)).
Offerors may propose different unit prices in later periods, subject to price reasonableness and balance review. Do not include a contingency for wage increases, which are handled under FAR 52.222-43(b). This is a five-year IDIQ with five ordering periods, not a base year plus options.
The dates shown are estimates.
5 Will the Government issue a Q&A amendment, and will the offer due date of October 23, 2026 remain unchanged?
Yes. This amendment provides Government responses. Quotations are due Tuesday, November 10, 2026, at 3:00 PM Central Standard Time (CST), by email to michael.murphy7@va.gov. Acknowledge this amendment and all prior amendments. If further clarifications materially affect quotation preparation, the Government will amend the solicitation and provide additional response time as needed.
6 May a subcontractor that is not small under NAICS 541350 A subcontractor that is not a similarly situated entity may not perform perform 100 percent of the services, or is its share limited by
FAR 52.219-14?
100 percent of the services where that would violate FAR 52.219-14. For this total small-business set-aside, similarly situated status depends on being a first-tier small-business subcontractor under the size standard for the NAICS code properly assigned to its subcontract, not automatically on the prime-contract NAICS alone. No more than 50 percent of the amount paid for services may be paid to firms that are not similarly situated, subject to the incorporated clause. Work a similarly situated entity further subcontracts counts against the limitation. The Government is not determining any named firm’s size or approving a particular subcontracting arrangement in this answer.
7 Are repair parts and materials under X005 excluded from the services limitation-on-subcontracting calculation?
The properly segregated supply portion is excluded from the services calculation. Service and repair labor remains in the services portion.
Labeling a charge X005 does not convert labor or other service costs into excluded supplies. Identify and segregate the costs and demonstrate compliance with the incorporated FAR 52.219-14 requirements.
Section D: Questions 1-35 Q Question Government Response 1 General: What is the current contract number for this work, if it is currently performed under contract?
See Section B, Q2.
2 General: Will the Government provide the most recent fire/smoke door and automatic door inspection reports, including any open deficiencies?
The facility reports that its carpenter verified completion of the most recent in-house fire/smoke-door inspections in June 2025. The historical record contains test-sheet markings of “3/24”; those original markings are not changed by this clarification. Automatic doors were not included. A reviewed, redacted copy of the available historical record is planned for release with the amendment; it is historical information, not a representation that the same conditions exist today.
3 Instructions para. 2: Will the Government post the site visit sign-in sheet with the question-and-answer amendment?
The Government will provide a company-name-only list of attendees at the October 15, 2026 site visit. Individual names, telephone numbers and email addresses will not be included. The list will be identified in the amendment attachment index when released.
4 SOW paras. 2 and 13: Will the Government provide a campus map and floor plans showing the location of each door in the Door Schedule?
No. A campus map and floor plans will not be provided. Door locations were shown during the site visit, and the SOW para. 13 Door Schedule identifies each door by building and location.
5 SOW para. 4.1: When does the Government expect the first semiannual inspection to be completed after award?
The first semiannual inspection must begin within 30 calendar days after award and be completed within the following 30 calendar days.
See Section B, Q7.
6 SOW para. 13, Bldg. 122 item 20: How many door assemblies are included in the entry "Corridor Smoke Doors"?
Three door assemblies. The amendment corrects the Door Schedule and the CLIN X001 quantity accordingly.
7 SOW para. 13, manual doors: Are these separate assemblies or These are separate assemblies, not duplicate listings. Price each one.
duplicate listings: Bldg. 160 items 19 and 20 (SPD1 Stair 1);
Bldg. 88/89 items 13 and 21 (SPB1 Stair 1); Bldg. 90 items 3 and 7 (DH-1-1 and 1-2); Bldg. 90 items 4 and 8 (DH-2-2 and 2-1);
Bldg. 90 item 18 and Bldg. 88/89 item 1 (88 DH-2-8 and 2-7);
and Bldg. 90 item 11 and Bldg. 91 item 28 (163 DH-2-1 and 2-2)?
8 SOW para. 13, automatic doors: Are these separate doors or duplicate listings: items 2 and 47 (Horton bi-part slider, Bldg. 71 basement tunnel); items 3 and 4 (Besam swing single, Bldg. A business suite, D0098802-AD26); and items 56 and 57 (Besam slider, Sterile Processing outer door)?
These are separate doors, not duplicate listings. Price each one. The corrected Door Schedule will distinguish the doors at items 3 and 4 (tag D0098802-AD26) with a -1 and -2 suffix.
9 SOW paras. 3 and 5, automatic door item 5: Should the Horton automatic revolving door at the main entry be inspected to ANSI/BHMA A156.27, which is not listed in SOW para. 3 or CLIN X002?
Yes. Inspect the Horton automatic revolving door at the main entry (automatic door item 5) to ANSI/BHMA A156.27 and price it under CLIN X002. The amendment adds A156.27 to SOW para. 3.
10 CLINs X001 and X002: If any automatic doors are also fire- or smoke-rated assemblies, will their NFPA 80/105 inspection be paid under CLIN X001 in addition to CLIN X002?
To VA's knowledge, none of the automatic doors in the Door Schedule is a fire- or smoke-rated assembly, so automatic doors are priced under CLIN X002 only. If inventory validation finds a rated automatic door, report it to the COR. Any added NFPA 80/105 inspection will be addressed by the Contracting Officer.
11 SOW para. 2, CLIN X001: Does the manual door inventory include any rolling steel fire doors, fire shutters, or horizontal sliding fire doors that require drop testing under NFPA 80? If so, how many?
No. The door inventory does not include rolling steel fire doors, fire shutters, or horizontal sliding fire doors.
12 SOW para. 2: Will the Contractor be required to install permanent door ID tags as part of the inventory validation during the first inspection?
Yes. During the first inspection, the Contractor will install a permanent ID tag on each door that matches the validated Door Schedule. The Contractor's standard durable tag is acceptable; VA has no specific tag specification. Include the tags in the CLIN X001 and X002 unit prices.
The amendment adds this to SOW para. 2. This is separate from the AAADM inspection label required on automatic doors.
13 SOW para. 4.3, CLIN X001: Does "minor hardware" include replacement of gasketing, smoke seals, astragals, silencers, and closer arms, or will those be quoted as repairs under SOW para. 7?
Yes. Under SOW para. 4.3, minor hardware includes gasketing, smoke seals, astragals, silencers, and closer arms. Include the parts and labor to replace them in the CLIN X001 unit price. They are not quoted separately as repairs.
14 SOW para. 5.3, CLIN X002: Are the "adjustments needed to bring a door into ANSI compliance" limited to labor, with replacement components such as sensors, controls, belts, and batteries quoted as repairs under CLIN X005?
No. Replacement components are not all separately payable under X005. The X002 unit price includes parts and associated troubleshooting, adjustment, repair, installation and replacement labor for normal-wear malfunctions identified during a scheduled inspection, including sensors, activation devices, controls, belts, batteries and similar components. Excluded from X002 are complete replacement of an operator, header assembly or door panel; damage from impact or misuse and other causes beyond normal wear; VA-requested upgrades;
and malfunctions reported between scheduled inspections. Separately payable work must be authorized under SOW paragraphs 6 and 7, as applicable. Do not bill separately for any work included in X002.
15 SOW paras. 4.2 and 5.2: Will VA, or its fire alarm contractor, place the fire alarm system in test and initiate the alarm signals needed to verify hold-open release and the automatic door fire alarm interface?
Yes. VA or its fire alarm contractor will place the fire alarm system in test and initiate the signals needed to verify hold-open release and the automatic door fire alarm interface. The Contractor will coordinate timing with the COR.
16 SOW paras. 2 and 6.3: Are electrified locking hardware and access control components on the listed doors (card readers, electric strikes, magnetic locks, delayed egress devices) within the service and repair scope?
No. Electrified locking hardware and access control components, including card readers, electric strikes, magnetic locks, and delayed egress devices, are not within the scope of this contract.
17 SOW paras. 4.2 and 7: When an assembly has a missing or illegible fire rating label, should the Contractor recommend field relabeling by a third-party labeling service as a repair under SOW para. 7?
Yes. Report a missing or illegible fire rating label as a deficiency and recommend field relabeling by a qualified third-party labeling service as a repair under SOW para. 7. Relabeling may be performed only when authorized by an order issued by the Contracting Officer.
18 SOW paras. 7 and 12.2: If a repair requires drilling or cutting a door or frame that contains asbestos, such as an older fire door core, will VA provide the abatement?
The facility’s Industrial Hygiene representative reports that the doors covered by this requirement do not contain asbestos. Only the Contracting Officer may authorize a change in contract scope or price.
19 SOW paras. 8.2 and 12.2: For work in operating rooms, sterile processing, and the pharmacy clean room, will VA furnish any required ICRA containment, or should the Contractor include it in its pricing?
Work that produces dust, including drilling or grinding, must be referred through the COR for facility infection-prevention review before it begins. The assigned foreman initiates the permit process, and VA Infection Prevention/Patient Safety determines the required protective measures. The Government has not established a blanket exemption for routine work in Surgery or the Emergency Department. Area-specific requirements, permit-approval authority and the containment pricing basis remain under review and will be clarified by further amendment before quotations close. Do not assume that solid-wall containment or negative air is required for every door, or that an unspecified containment allowance is included in the inspection unit prices. Existing safety and coordination requirements remain applicable.
20 CLIN X005: Is freight, including expedited shipping for emergency parts, billable under CLIN X005?
Show any proposed shipping charge separately in the itemized repair quote. Freight is payable only if expressly approved in the CO-issued order and not already included in the quoted part price, markup or other contract charge. Expedited shipping requires justification and CO approval. No separate freight is payable for parts or work already included in X001/X002. Travel, mileage, trip and mobilization charges remain included in contract prices and are not separately billable.
21 SOW paras. 6.4 and 7.2, CLINs X003 and X004: Is technician time spent diagnosing a service call and preparing the itemized repair quote billable under CLIN X003 or X004?
On-site troubleshooting time on an authorized service call is billable under CLIN X003 or X004 (SOW para. 6.4). Preparing and submitting the written repair quote required by SOW para. 7.1 is included in the contract prices and is not separately billable (B.2.1(g)).
22 SOW para. 6.4: When a repair requires a return trip after parts arrive, does the one-hour minimum apply to each trip?
No. The one-hour minimum applies once per service call. If a repair needs a return trip after parts arrive, the return trip is billed for actual time on site in 0.5-hour increments, without a second one-hour minimum. Travel time is not billable. The amendment adds this to SOW para. 6.4.
23 SOW paras. 6.3 and 6.4, CLIN X004: Will emergency responses under SOW para. 6.3 that occur outside normal hours be billed at the CLIN X004 rate?
See Section B, Q3.
24 SOW para. 8.1, CLINs X001 and X002: If the COR requires scheduled inspections of operating room or emergency department doors outside administrative hours, will the Government pay an after-hours premium, or must the CLIN X001 and X002 unit prices cover that work?
Price CLINs X001 and X002 to include any scheduled inspections the COR asks to be performed outside administrative hours (SOW para. 8.1).
No separate premium will be paid. Only the CO can change pricing
(VAAR 852.201-70).
25 Section B.1 para. 3: May CLINs X001 and X002 be invoiced monthly for the doors inspected that month, or only after the full semiannual round and report are complete?
No. For X001 and X002, submit the complete report for the full semiannual inspection round for Government review and acceptance before invoicing that round. Partial monthly inspection acceptance is not authorized by this amendment. B.1 monthly-in-arrears invoicing continues for otherwise completed, accepted and separately authorized services and repairs. Inspection invoices shall identify the accepted assemblies and applicable unit prices; do not bill the same work under an inspection CLIN and a repair/service-call CLIN.
26 Section D.2, FAR 52.222-43: Will the Government incorporate a revised wage determination at the start of each ordering period, with a corresponding price adjustment under FAR 52.222-43?
Yes. Under FAR 52.222-43(c), the wage determination in effect on each anniversary date applies. Adjustments cover only actual increases in wages and fringe benefits needed to comply, plus related payroll taxes and insurance, with no overhead, G&A, or profit (para. (e)). Request any increase within 30 days of receiving the new wage determination (para.
(f)).
27 Section D.4: Section D.4, Statement of Equivalent Rates for Federal Hires, contains blank placeholders. Will the Government issue the completed statement by amendment?
See Section B, Q6.
28 SOW para. 10.1: Does the 10-working-day report deadline start when the full semiannual inspection round is complete, or on each day doors are inspected?
The contractor shall submit the complete inspection report within 10 working days after completing the full semiannual inspection round, not after each day of field inspections. VA plans up to 10 business days to review the complete report and identify acceptance or deficiencies. This review target does not change applicable payment, interest or constructive-acceptance requirements. See Section D, Q25 regarding full-round acceptance and invoicing. SOW paragraph 10.2 repair-report requirements remain unchanged.
29 SOW para. 10.1: Is there a VA-required template or software for inspection reports, or may the Contractor use its own format that meets SOW para. 10.1?
No. VA does not require a specific template or software. The Contractor may use its own format if it includes everything required by SOW para.
10.1.
30 SOW para. 9.2: Will the Government accept the NFPA "Inspection, Testing, and Maintenance of Swinging Fire Door Assemblies" certificate as equivalent to DHI FDAI?
An NFPA course-completion certificate is not automatically accepted as proof of equivalent inspector qualifications. SOW paragraph 9.2 permits DHI FDAI, ICC or equivalent qualifications and requires knowledge and understanding of the door assemblies being inspected. The Government is reviewing the specific NFPA program and supporting evidence of component/hardware inspection, operational testing, deficiency identification and reporting competency. A final determination on the identified certificate will be issued by further amendment before quotations close. This response does not impose a DHI-only requirement. AAADM certification and automatic-door brand training/experience under paragraph 9.1 are separate requirements.
31 SOW para. 9.1: What documentation will the Government accept as "documented experience" for automatic door brands on which a technician is not factory trained?
All automatic door technicians must hold current AAADM certification.
For brands on which a technician is not factory trained, provide a work history showing experience on that brand, including the customer, dates, door type, and work performed.
32 Section C.10: May technicians perform work under escort or with a temporary badge while their Tier 1 background investigation is pending?
The personnel-vetting clause is Section C.9 in the RFQ used for this amendment. This amendment does not authorize technicians to begin work while required personnel vetting is incomplete. Any temporary or escorted access while PIV issuance is pending must be specifically authorized by the responsible VA security officials, subject to their prerequisites, area restrictions and escort arrangements. An escort does not waive vetting, fitness, credentialing or other applicable contract requirements. The facility is confirming the applicable conditions; any clarification affecting performance or quotation preparation will be issued by further amendment.
33 Instructions para. 5: Does the 10-page limit apply only to the Technical Capability section, or to all of Volume I, including Past Performance?
Only Technical Capability (Instructions para. 5). Past Performance (para.
6), certificates, and the sample report do not count toward the 10 pages.
34 Instructions para. 5: Are the cover page, table of contents, and cover letter excluded from the 10-page limit?
Yes. The cover page and table of contents do not count toward the 10-page limit. A cover letter does not count unless it includes technical content, in which case it counts. The amendment revises the Instructions para. 5 heading to match.
35 Instructions para. 6: Will past performance of a proposed subcontractor be considered when evaluating the offeror's Past Performance?
See Section B, Q5.
