About this file

This is an Amendment/Modification (Standard Form 30) to solicitation 36C25526Q0333 issued by the Department of Veterans Affairs Network Contracting Office in Leavenworth, Kansas. The amendment extends the offer receipt deadline to 22:00 CDT on May 14, 2026, and updates the Statement of Work and pricing schedule for the procurement of a dental cone beam computed tomography (CBCT) imaging unit at Harry S. Truman VA Medical Center in Columbia, Missouri.

The solicitation requires contractors to deliver, install, and provide five years of maintenance and support (base year plus four option years) for a new Carestream 9600 CBCT imaging unit to replace an end-of-life Carestream 9000 system. Technical specifications include strict dimensional constraints (59.06 inches length × 78.74 inches depth × 86.61 inches height, maximum 463 pounds), chin height adjustment range of 41 to 67 inches, generator tube voltage of 60-90 kV at 140 kHz frequency, and capability to perform 15 specific radiological examination types. The amendment adds a new CLIN 1012 for removal and disposal of the old equipment (with potential trade-in credit), separates maintenance and support CLINs for each option year (CLINs 2001-4001), and removes a conflicting height requirement from the original SOW. Deliverables include an installation plan within 10 calendar days of award, staff training completion within 30 calendar days of installation, and a maintenance and support plan within 15 calendar days post-installation. The contractor must provide qualified key personnel (installation specialist, training specialist, and technical support engineer with minimum 5 years experience), cover all travel expenses, ensure Section 508 compliance, and comply with VA security protocols. Delivery is FOB Destination within 60 days after receipt of order (ARO) to the specified Columbia, Missouri VA Medical Center location.
