Questions and Answers:

The following questions (Q. #) have been asked. Government responses (A. #) are provided under each question.

** Questions Q. #1 through Q. #25 were answered in previously issued Amendments.

Q. #26: Section 2.9.6.: Are the forms submitted at the time of proposal or after contract award? Some forms in Section 6.1. state “with proposal”. Does offeror need to conform with 2.9.6. or 6.1.?

A. #26: Question refers to the VSC security documents listed in Section 2.9.5. Review of the question identified an error for correction in addition to the answer, as follows:

1. Section 6.1. (table following 6.1.1.), Additional Deliverables on page 38, clearly states the “Proposed physician VSC Document (OF 306, other related documents)” and “Within 5 workdays after Notice of Award”.

2. Page 22 is replaced to correct an associated error in Section 2.9.4, first sentence, by replacing “with its offer” with “after award” in that sentence. The conformed copy of the solicitation reflects this page replacement.

Q. #27: Section 6.2.2.1.: Will VA-provided CPT code information be identified by differentiating between DR (diagnostic) physician work and IR (interventional) physician work, referencing the previously issued Mock-up Sample spreadsheet’s tab for Radiologist A and Radiologist B?

A. #27: Yes, and:

1. The CPT codes performed by DR radiologists will be identified in a separate spreadsheet tab by the name of the DR physician and the CPT codes performed by that physician during the stated timeframe (month).

2. If more than one DR performed during that month, that physician’s CPT information will be listed as a separate spreadsheet tab.

3. The same would apply for IR physicians.

4. The names and position (DR or IR) will follow the Price Schedule listings of physician and designation as DR or IR.

Q. #28: Mock-up Sample (provided with Amendment 0005): Will the CPT codes shown in the Mock-up Sample are not AMA CPT codes. Will the monthly spreadsheets provided by the VA during contract performance be the actual AMA CPT codes?

A. #28: Correct, and Yes, and:

1. The mock-up sample was intentionally shown to be fictitious and with exaggerated data (i.e. made-up codes, along with intentionally mis-matched coding, descriptions, wRVUs and quantities).

2. The actual information to be provided will include AMA CPT codes, the abbreviated descriptions of those codes, and wRVU values and quantities.

Q. #29: Solicitation Page 84, Addendum to FAR 52.212-1 Instructions to Offerors – Commercial Items, item 1.

(b), item (9)(iv.): What is Health Safety Items – Provide proof of Health Record as required by solicitation?

A. #29: Question refers to the original requirement for proof of personnel testing and immunizations, which was changed by following amendment action, and:

1. This item was missed during those amendments. This item is now stricken.

2. Page 84 is replaced to remove the requirement and adjust the enumeration of items iv. through viii. by changing to iv. through vii. The conformed copy of the solicitation reflects this page replacement.

Q. #30: Solicitation Page 85, Addendum to FAR 52.212-1 Instructions to Offerors – Commercial Items, item 1.

(d): Offers due date still shows 01/14/2021, 4:30 PM Central Time and didn’t change per the last amendment extending the due date to 01/21/2021, 4:30 PM Central Time. Which is correct?

A. #30: The offer due date is 01/21/2021, 4:30 PM Central Time, as extended by Amendment 0005. However, page 85 is replaced to reflect the accuracy of that prior extension. The conformed copy of the solicitation reflects this page replacement.

Q. #31: Solicitation Page 88, Proposal Submittal regarding (Check as applicable) items for insurance: Should the offeror submit only the page 88 with the appropriate entry checked or the entire solicitation (all 114 pages)?

A. #31: Review of the question identified a required corrective change to the proposal submittal requirements shown in the Addendum to FAR 52.212-1 Instructions to Offerors – Commercial Items, item 1. (b), Submission of offers, item (1). (page 84, also referenced in A. #29, 2. above).

3. Page 84 is replaced, per A. #29, 2. above, with section 1. (b) item (1) changed from “Completed and SF 1449 and Section B.1, 1.a.” to “Completed and signed SF 1449 and all solicitation pages, attachments and proposal submittal documents, including all ‘fill-in’ information.” The conformed copy of the solicitation reflects this page replacement.

B. #32: Solicitation Page 101 through 114, (2) Offeror to identify the applicable paragraphs at (c) through (v) of this provision that the offeror has completed for the purposes of this solicitation only, if any. Check the appropriate boxes and send only Pages 101-114 or the entire solicitation (all 114 pages)?

A. #32: Question refers to the solicitation’s Section E.9, Federal Acquistion Regulation (FAR) Provision 52.212-3, Offeror Representations and Certifications – Commercial Items (Oct 2020), paragraph (b)(2) (page 101).

1. Offerors should refer to the introductory paragraph of the provision at Section E.9, 52.212-3 (page 97), that provides instructions on completion of the annual representations and certifications electronically in www.sam.gov and instructions when not completed electronically in www.sam.gov.

2. Pursuant to the answer provided by above A. #31, the full solicitation, “Completed and signed SF 1449 and all solicitation pages, attachments and proposal submittal documents, including all ‘fill-in’ information” is required.
