CONTINUATION PAGE

RENOVATE VACATED SURGURY WARD 6 QUESTIONS:

Q1. What is the space below the project?

A1. The 5th floor is a hodge podge of administrative, Union, Veteran Service Organizations, Employee Health and Lean Learning Organization.

Q2. Is overtime required?

A2. As this is a hospital, all services need to have the ability to continue normal operations. The VA makes all attempts to coordinate works during normal hours (0730-1600) however this is not always possible. There are some activities that will only be able to occur after normal hours or on weekends due to noise, debris or occupancy.

Q3. Is there an elevator contractor for the whole hospital?

A3. The elevator service contractor is Thyssenkrupp.

Q4. Who is the roofing manufacturer?

A4. Garland holds the warranty for the roof.

Q5. What is the extent data/phone and electrical cabling demo?

A5. As addressed in different locations, remove all mounted wire mould and receptacles pulling wire back to panel; phone and data cables are to be removed all the way to patch panel. It is highly recommended that these are done by an electrician rather than general demo.

Q6. Will OT be required for the piping work on the 5th floor?

A6. As this is a hospital, all services need to have the ability to continue normal operations. The VA makes all attempts to coordinate works during normal hours (0730-1600) however this is not always possible. There are some activities that will only be able to occur after normal hours or on weekends due to noise, debris or occupancy.

Q7. The Facility Sanitary & Vent Piping and Storm Drainage specification sections do not include PVC-DWV piping. Is PVC-DWV piping acceptable?

A7. Yes

Q8. Are 3D coordination drawings required for this project or will typical layout coordination drawings be acceptable?

A8. Per Spec 230511 (Basic Mechanical Materials and Methods), coordination drawings shall include plan view and elevations.

Q9. For Bid Item #3, will the sink be installed in Break Room 620? If not, where should the water/w&v piping installation stop?

A9. Refer to Plumbing General Notes on Sheets 2-PL1.1 and 2-PL2.1. The sink will not be installed. Associate vent piping will be omitted. Water supply piping will be stubbed out maximum 6” from main with valve and cap for future extension. Waste piping will be roughed in and capped above the 5th floor ceiling to approximate location of future drain line through the floor. Room shall be coordinated to allow for future core-drill and extension of piping to sink.

Q10. For Bid Item #4, Restroom 632A is to be omitted. Will any of the water/w&v piping serving this room still be required?

A10. Refer to Plumbing General Notes on Sheets 2-PL1.1 and 2-PL2.1. The sink and toilet will not be installed. Associated vent piping will be omitted.

Q11. For Bid Item #5, will the sink be installed in Galley 656A? If not, where should the water/w&v piping installation stop?

Q12. For Bid Item #3, will VAV’s 5-38, 5-41, & 5-42 still be installed? If not, where should the heating water/ductwork installation stop?

A12. If Bid Item #3 is accepted, VAVs 5-38, 5-41 and 5-42 will not have to be purchased or installed. Stub into space (and cap) ductwork and mechanical piping branches that serve VAVs 5-38, 5-41 and 5-42 or the associated rooms. For ductwork and mechanical piping that taps off mains within the Bid Item space, provide taps then cap. Provide isolation valves in piping taps and dampers in return air ductwork taps.

Q13. For Bid Item #4, Restroom 632A is to be omitted. Will the exhaust ductwork serving this room still be required?

A13. If Bid Item #4 is accepted, the 8" diameter exhaust duct from the main to 623A is not required.

Q14. For Bid Item #5, will VAV’s 5-27, 5-28, & 5-29 still be installed? If not, where should the heating water/ductwork installation stop?

A14. If Bid Item #5 is accepted, VAVs 5-27, 5-28 and 5-29 will not have to be purchased or installed. Stub into space (and cap) ductwork and mechanical piping branches that serve VAVs 5-27, 5-28 and 5-29 or the associated rooms. For ductwork and mechanical piping that taps off mains within the Bid Item space, provide taps then cap. Provide isolation valves in piping taps and dampers in return air ductwork taps.

Q15. Is a professional duct cleaning subcontractor required or will SMACNA duct cleanliness intermediate installation practices be acceptable?

A15. SMACNA duct cleanliness intermediate installation practices will be acceptable. HOWEVER, if these practices are found to not be followed, then a professional duct cleaning will be required at no additional cost to the government.

Q16. Is this project sales tax exempt? If the project is sales tax exempt can the government provide a sales tax exemption certificate?

A16. Yes this is sales tax exempt. At the NTP meeting, tax exempt documents will be provided.

Q17. Will the Government provide comprehensive written badging requirements for the project if badging is required?

A17. There is no written requirements to provide. Badging is required and will be explained at NTP. There is no cost to the contractor. It is a safety briefing and facility specifics. When complete, workers will be provided a badge, good for one year, that is to be visible at all times while on the grounds.

Q18. Where will the designated parking area be for workers accessing the construction site and dumpster be located?

A18. As discussed at the site visit, there is contractor parking on the south west corner of the facility for contractors. Space is limited. The dumpsters are located in this same area.

Q19. Will the government accept the use of the industry leading Procore Project Management software as the primary submittal and data tracking tool for this project? The software is compatible with the VA’s networks and is currently in use at many VA locations.

A19. The facility has not used this program. If it is an equal to Submittal Exchange, it would be acceptable. Specs would need to be provided.

Q20. Will the fire sprinkler system and/or the fire alarm system need to be disrupted during construction? If yes, what if any additional security measures will the government require e.g. fire watch, temporary fire alarms, adjustments to sprinkler heads, etc.

A20. Provide at least one smoke detector within the project limits hard wired into the building fire alarm system that is activated during unoccupied times. Reference Sheet 2-FP0.1 General Note 7: contractor shall provide a fire watch during construction hours. Each system (fire alarm and fire sprinkler) must be in service at the end of each construction day. Portions of the system (fire alarm and fire sprinkler) that are to remain must remain in service. Contractor is responsible for providing temporary feed (sprinkler main and connections) such that these areas may remain in operation during construction.

Q21. During demolition and while ceiling grids are out, will the fire sprinkler heads have to be turned upside down an/or reach a certain distance from the roof deck to follow Codes NFPA 13 or will other NFPA code apply ex. 241?

A21. NFPA 13 only applies to the completed installation. VA Design standards does not require uprights be installed during construction phases only to be replaced with pendants upon completion. Also, pendent sprinklers turned upside-down would result in ineffective coverage, as the throw would be primarily directed upward (unlike an upright which directs water up and down). Fire watch is required when the system is out of service.

Q22. Does the Government have a fire alarm servicer that they currently work with? What is the name and contact information of the person or persons that will need to be coordinated with for security and fire alarm and suppression systems work and/or disruptions?

A22. Seimens

Q23. Has the work area been checked for asbestos?

A23. There is a report that has been completed, but it is not all inclusive. There is a possibility during demo to find fittings that were not known.

Q24. What is the Governments’ policy if asbestos products are encountered during construction?

A24. You can find this information in the general specifications. Notify and coordinate with VA COR Q25. Due to current potentially significant tariffs being placed on the importation of building materials such as wood or steel by the current Presidential Executive Branch, if a tariff is placed after bid time and before award will the government consider a price escalation on a contract value for products such as steel or wood should the executive branch levee tariffs effecting such materials? Most of these materials would still have been Buy American compliant as the tariffs would increase pricing on all steel or wood materials, regardless of (question cut off)

A25. With the expected quick turn around from bid to award, we do not see this as an issue. Would be treated on a case by case (material) basis and this would have to be clearly documented in bid documents. The view of the facility is that prices will not drastically change from bid to award time.

Q26. Will there be access to elevators? If so, can the VA list the dimensions?

A26. As discussed and shown during the site visit, there are two elevators available for use Q27. Will there be the need for a VA escort while working on 5th floor areas?

A27. This will be based on the areas that are being worked in, but generally no.

Q28. Is there space below to place a dumpster with a trash chute from one of the windows in this renovation?

A28. No.

Q29. Please confirm that all TVs, projector and screens will not be included on those rooms that will be white boxed on alternate #2.

A29. TV’s, Projectors, and Screens will not be included in these white boxed rooms.

Q30. On same alternate, will gypsum on walls be required? What about insulation?

A30. Gypsum will be required; however, no taping, mudding, or priming will be required unless the wall is noted to be fire rated. In a case where a wall is noted to have a fire or smoke rating, all UL-Rating requirements must be met.

Q31. Please confirm if Gypsum or Insulation will be required for partition walls facing Alternate #4 rooms to be White boxed?

A31. Gypsum and insulation will be required; however, no taping, mudding, or priming will be required unless the wall is noted to be fire rated. In a case where a wall is noted to have a fire or smoke rating, all UL-Rating requirements must be met.

Q32. Will Sign in sheet be released?

A32. Yes, it is part of this amendment.

Q33. Have sink specs been selected (restroom)?

A33. See 22 40 00, para 2.10 Q34. Is there a spec for ACM material for demolition?

A34. There are two spec sections 02 82 13.13 and 02 82 13.19

Note: Executive Order (EO) 13706, Establishing Paid Sick Leave for Federal Contractors applies to all contracts subject to the Davis-Bacon Act for which the contract is awarded (and any solicitation was issued) on or after January 1, 2017. If this contract is covered by the EO, the contractor must provide employees with 1 hour of paid sick leave for every 30 hours they work, up to 56 hours of paid sick leave each year.

Employees must be permitted to use paid sick leave for their own illness, injury or other health-related needs, including preventive care; to assist a family member (or person who is like family to the employee) who is ill, injured, or has other health-related needs, including preventive care; or for reasons resulting from, or to assist a family member (or person who is like family to the employee) who is a victim of, domestic violence, sexual assault, or stalking. Additional information on contractor requirements and worker protections under the EO is available at www.dol.gov/whd/govcontracts.

Unlisted classifications needed for work not included within the scope of the classifications listed may be added after award only as provided in the labor standards contract clauses (29CFR 5.5 (a) (1) (ii)).

The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of "identifiers" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate).

Union Rate Identifiers

A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than "SU" or "UAVG" denotes that the union classification and rate were prevailing for that classification in the survey. Example:

PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014.

Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.

Survey Rate Identifiers

Classifications listed under the "SU" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier.

Survey wage rates are not updated and remain in effect until a new survey is conducted.

Union Average Rate Identifiers

Classification(s) listed under the UAVG identifier indicate that no single majority rate prevailed for those classifications; however, 100% of the data reported for the classifications was union data. EXAMPLE: UAVG-OH-0010 08/29/2014. UAVG indicates that the rate is a weighted union average rate. OH indicates the state. The next number, 0010 in the example, is an internal number used in producing the wage determination. 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier.

A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/CBA rate of the union locals from which the rate is based.

WAGE DETERMINATION APPEALS PROCESS

1.) Has there been an initial decision in the matter? This can be:

* an existing published wage determination

* a survey underlying a wage determination

* a Wage and Hour Division letter setting forth a position on a wage determination matter

* a conformance (additional classification and rate) ruling

On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the Davis-Bacon survey program. If the response from this initial contact is not satisfactory, then the process described in 2.)

and 3.) should be followed.

With regard to any other matter not yet ripe for the formal process described here, initial contact should be with the Branch of Construction Wage Determinations. Write to:

Branch of Construction Wage Determinations Wage and Hour Division U.S. Department of Labor 200 Constitution Avenue, N.W.

Washington, DC 20210

2.) If the answer to the question in 1.) is yes, then an interested party (those affected by the action) can request review and reconsideration from the Wage and Hour Administrator (See 29 CFR Part 1.8 and 29 CFR Part 7). Write to:

Wage and Hour Administrator U.S. Department of Labor 200 Constitution Avenue, N.W.

Washington, DC 20210

The request should be accompanied by a full statement of the interested party's position and by any information (wage payment data, project description, area practice material, etc.) that the requestor considers relevant to the issue.

3.) If the decision of the Administrator is not favorable, an interested party may appeal directly to the Administrative Review Board (formerly the Wage Appeals Board). Write to:

Administrative Review Board U.S. Department of Labor 200 Constitution Avenue, N.W.

Washington, DC 20210

4.) All decisions by the Administrative Review Board are final.
