STATEMENT OF WORK (SOW)

as of 03/11/2024

1. Contract Title Addition of Lights, Booms, X-Ray Shielding for Catheterization Lab

2. Background

The William S. Middleton Memorial Veterans’ Hospital Cardiology Service requires purchase and installation of three ceiling mounted flex arms. The three flex arms are required to meet best practices, design specifications for the construction project 607-CSI-106, and standardization for the Catheterization (Cath) Lab that will be in C4109 at the William S. Middleton Memorial Veterans’ Hospital. The flex arms need to be installed and ready for use before the start of phase 3 (phase 3 begins at the beginning of August 2024) of the construction project 607-CSI-106 so that patient care can continue without delay. The installation of the flex arms is required because construction project 607-CSI-106 will not support this installation. The installation must occur during phase 2 (specifically during the last week of July before the medical equipment under construction project 607-CSI-106 is installed) of the construction project 607-CSI-106. To ensure this the flex arms would have to meet the specifications of project 607-CSI-106 as well as be onsite at Madison VA as well as be installed. This must occur before August 1st to ensure no potential delays with construction project 607-CSI-106. The three flex arms must include required shielding that will be used to protect clinicians while x-rays are fired at the patient during procedures. A flex arm with an in-light camera which is required for patient care and a flex arm with an anesthesia boom that will support anesthesia hook ups as well as medical gas connections which must include Oxygen, Nitrous Oxide, Medical Air, WAGD, and Vacuum connection.

3. Selection Criteria

Limited Source, Brand name only: Skytron – Skytron ceiling mounted FlexArm with shielding, in-light camera, boom, and installation.

4. Project Scope:

General scope of work is to install three ceiling mounted flex arms that will support a shield, in-light camera and shield, and anesthesia boom. The location will be the new Cath Lab in room C4109, and installation must occur during the phase 2 of the construction project 607-CSI-106 which would be the last full week of July (installation must occur before the medical equipment installation occurs under construction project 607-CSI-106). Construction will need to occur during this time since the installation of the flex arms is required to install the medical equipment that will be installed under construction project 607-CSI-106. Without the installation of the flex arms the medical equipment that is being installed will be non-functional since the equipment relies on the flex arms for support.

During phase two the room will be unavailable for use and will already have construction on going so the vendor will be able to install the ceiling mounted flex arms without impacting patient care. The proposal will include the procurement of the 3 flex arms with x-ray shielding, in-light camera, and anesthesia boom. The installation must occur the last full week of July of 2024 or just before phase 3 begins (phase 3 is slated to begin the first week of August) to ensure no delays to the overall construction project 607-CSI-106 and provide the least impact to patient care. Project contractors will have to abide by the same working conditions as the contractors that will be working under 607-CSI-106.

No alterations, additions or deletions shall be made unless prior written approval is given by the Contracting Officer (CO). Under no circumstances is the contractor to begin any work on the project until the federal government provides a contracting officer signed Notice to Proceed issued through the contracting office. The installation shall occur Monday-Friday between the hours of 7:00 am to 4:00 pm or off-tour and on weekends/Holidays as necessary to reduce impact on direct patient care at the hospital and ensure no additional delays to the construction project 607-CSI-106. Times may be altered to evenings and weekends/holidays with proper notice to CO. Elevator use cannot interfere with patient care and clinical appointments.

All work must be in accordance with the most current Healthcare and VA Specifications procedures unless expressed written permission is received from the Contracting Officer Representative for approval of an exemption.

A. General Contractor Requirements Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, planning, supervision, etc. to fully install the flex arms that must include x-ray shielding, in-light camera with x-ray shielding, and anesthesia boom. Contractor shall provide everything necessary to achieve a fully functional system that meet the design specifications for construction project 607-CSI-106, fully supports the medical equipment being installed under construction project 607-CSI-106, and that can be fully installed before the beginning of Phase 3 (first week of August). All components will be designed for use in a sterile environment and be able to withstand VA clean agents. Contractor shall provide and install all equipment (brand name) listed below.
