48105 The VA Iliana Healthcare System in Danville Illinois requires software maintenance and software support for the Medication Reconciliation Tool for Veterans Integrated Service Network (VISN) 12 Site located in Danville, IL (VA Iliana Healthcare System) for a one (1) year base period with three (3) option years, exercisable at the Government’s discretion. The MedRec Tool works by combining lists of all medications from multiple sources into a single, easy to manage, master list. The medications are logically ordered so that the user can efficiently and accurately reconcile medications at patient admission and discharge. The MedRec Tool allows the provider to generate patient-friendly lists specifying new, changed, or discontinued medications.

The estimated value of the requirement is $177,600, if all options are exercised.

Please see the attached justification for further information.
