About this file
This document contains an amendment to a solicitation for medical instrument maintenance and repair services. The amendment extends the deadline for receipt of offers to May 26, 2020 at 11:59 PM local time. The services are required by the Clement J. Zablocki VA Medical Center and Milo C. Huempfner Outpatient Clinic in Milwaukee, Wisconsin. The amendment includes answers to questions from potential offerors to clarify requirements. Services must be provided through weekly on-site visits to both facilities, and pricing must cover all instruments listed for maintenance, whether repaired on-site or off-site. Historical service volumes and percentages of on-site versus off-site repairs are not precisely tracked by the agency. The contract will transition maintenance services from the current rate-based model to a fixed monthly price.

Questions & Answers/Clarifications
1. In reviewing the instrument list, we found that the list also includes items that are not typically serviced in the mobile vehicle. These items include rigid endoscopes, light cords, power hand pieces, etc. We are assuming these pieces of equipment were included with the inventory list, as it was pulled from an instrument/tray tracking system. Should we assume these items would not be part of an annual service agreement? For reference, here are several rigid scopes that were included on the list:
ACMI
G27L-12A
SCOPE 12DEGREES 4MM-12IN
ACMI
G27L-30WA
SCOPE 30DEGREES
ACMI
M3-30A
HYSTEROSCOPE 4MM
Please note, we have the capability to repair this equipment. If required, we can include these in an annual service agreement, but these items would need to be sent to our National Repair Center for service. The repairs on these items are not performed in our mobile units.
The intent behind use of a mobile lab to perform on-site maintenance and repair of the medical instruments is to facilitate a faster turnaround time. Should more extensive repairs be required, or the necessary maintenance/repairs otherwise not be able to be performed on-site, it is expected that the necessary work will occur off-site:
The price for maintaining all of the instruments listed in Attachment 1, whether they can be serviced on-site or require shipping to an off-site location, should be included in the quoted price.
2. On the inventory list, there are rigid scopes, semi rigid scopes, power equipment, light cords, etc. that would need to be sent out for repair and could not be done onsite. Are we to assume that the repair of this equipment as well as the case cart/push cart repairs will be handled on a time and material basis outside of the firm fixed price of this contract? Or are those repairs to be included in the firm fixed price?
Off-site maintenance/repairs and case cart/push cart repairs should be included in the overall price. See answer to Question 1 for additional details.
3. Are weekly visits required at both Clement J. Zablocki VA Medical Center and Milo C. Huempfner Outpatient Clinic? If so, are the weekly visits to Milo C. Huempfner Outpatient Clinic a full 8-hour days?
It is anticipated that weekly visits are required at both facilities. The weekly visit to the Milo C. Huempfner Outpatient Clinic typically lasts less than a full day.
4. Please provide the current average number of trays being serviced per visit.
The Zablocki VAMC has not been tracking the number of trays or instruments serviced per visit. Rather, the determining factor to date has been how many instruments could be serviced within a weekly visit.
5. What is the Onsite vs. Offsite instrument repair mix?
The Zablocki VAMC has not tracked maintenance/repair metrics for its instruments. Empirically, it appears that most instruments are serviced on-site, but a precise percentage of on-site versus off-site maintenance cannot be given.
6. What is your current volume of Onsite repairs? Offsite repairs?
See the answer to Question 5, above.
7. Will any other VA clinics be included in this service agreement that not outlined within the RFP?
No. This contract is specifically for service to the Zablocki VAMC and Huempfner Clinic. Any additional clinics or other work will be addressed via bilateral modification should the need arise.
8. Can you inform us how service is being deployed presently? Daily Rate, Time & Material, Monthly Service Agreement?
Currently, maintenance service is being provided on a per instrument rate, based on the type of instrument and service required. This process has proven to be cumbersome; hence this requirement for a flat-rate monthly price.
9. Do you expect the total number of instruments to be serviced to increase, decrease, or stay the same? If the volume changes, by how much?
It is expected that the current volume will remain relatively unchanged.
10. Can you educate us on what process is used to determine which trays are picked up for inspection?
Trays/instruments in need of service will be coordinated by the Sterile Processing Service (“SPS”), which will serve as the Contractor’s POC for pickup and return of instruments:
