About this file

This document is an Amendment of Solicitation/Modification of Contract for a Total Small Business Set-Aside solicitation issued by the Department of Veterans Affairs Dayton VA Medical Center. The amendment extends the response deadline to 12/29/2025 at 1:00 pm EST and provides responses to vendor questions regarding the solicitation. Key details include the requirement for an Emergency Medical Flight Transport Service (EMFTS) license, clarification that no separate cost schedule is needed for cancelled transports, and confirmation that vendors will need to provide appropriate state licenses to operate in the Dayton service area covering Ohio, Indiana, and Kentucky.

The solicitation requires offerors to submit several items to remain compliant, including proof of insurance, an active EMFTS license, Department of Labor wage determination compliance, limitation on subcontracting compliance, signed forms, past performance documentation, and a technical approach with fleet listing, staffing plan, and contingency plan. The government will not change the set-aside from Small Business, and vendors are advised that the past performance questionnaire, while not specifically tailored to the ambulance industry, will be used to evaluate performance quality.

Questions and Responses

1. Can you clarify if this is a service disabled vet set aside or small business set aside?

Response: This Contract has been solicited as a Total Small Business Set-Aside.

2. Does the Government anticipate a separate cost schedule or allowable billing structure for dry runs and/or cancelled transports, or if these scenarios should be included elsewhere in the pricing submission?

Response: No, PWS section 4.5.2 address cancellations.

3. Is there going to be a CLIN on staging requirement in this solicitation?

Response: No. The VAMC may let you park an ALS or BLS at the Medical Center, but additional pay will not be authorized. Vendors are also free to look into setting up additional hub around the VAMC or subcontract after-hour trips with a local vendor. The logistic is up to the offeror, The VA will require 24/7 support and ALS night/weekends are often emergency where response times are important. Your staffing plan will need to address this.

4. Could you please confirm the correct response deadline for us?

Response: Amendment will extend the Response date to 12/29/2025 at 1:00 pm EST.

1. Page 74 states: “Items Required to be considered compliant and remain in competition for award.

a. Proof of insurance ( Section C.6 of this Comb/RFQ).

b. Copy of an Active EMTFS License for the prime contractor.

c. DoL WD compliance Memorandum (Attachment D.3)

d. FAR 52.214-19 Limitation on Subcontracting Compliance (Attachment D.4)

e. Signed SF 1449 (Page 1 of this Combo/RFQ)

f. Sign all SF 30 Amendments, complete table in section B(1) to acknowledge all Amendments.

g. Past Performance, CPARS, Questionnaires (Attachment D.5), or References.

h. Technical Approach: Fleet Listing, Staffing Plan, Contingency Plan” Question: For the items listed in g, it reads as if we can provide any of the 4 items listed. Can we provide any of the 4 items listed? Or, are we to provide all 4 items?

Response: Offeror are to provide past performance. They may come in any of the items. They can also be a mix and match of the items. If the references do not follow E.9 52.212-2 (b)(2) a through c that reference will not be accepted.

5. Can the set-aside be changed to Unrestricted?

Response: No, market research indicated we will receive adequate competition with a Total Small Business Set-Aside.

6. Page 56 states: “evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR part 4, longer period required by statute, or periods specified in other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.”

Is this provided in error? Or is it just incomplete?

Response: That is from clause FAR 52.215-2 which was deleted during review. Not all of the clause was deleted by error. Please disregard, it will not be in the contract.

7. Page 80 states: “j. Did the offeror submit an active EMFTS license” Question: Since contractors are also required to have an Indiana and Kentucky license to operate this Dayton service area, are you requiring our Ohio, Indiana and Kentucky licenses with our quotation?

Response: No, not with the quote, but the vendor will be required to acquire all appropriate licenses.

The EMFTS license is the only license we require before award due to the difficulty to acquire one.

8. Past Performance Questionnaire Question: The past performance questionnaire is not related to the ambulance industry and will be difficult for anyone to complete as most questions will be not applicable. Can you provide a past performance questionnaire more related to the ambulance industry?

Response: No, this is a standard past performance questionnaire. The questions that do not pertain to ALS/BLS can be marked as N/A or left blank. The CO reviewed the form and it has enough information for an overview of the quality of performance.
