About this file

This file is an Amendment 0001 to Solicitation 36C25026Q0444, consisting entirely of questions and answers related to teleradiology services for Dayton VA Medical Center (VAMC). The amendment extends the offer receipt deadline from July 31, 2026, to August 11, 2026, at 2:00 PM EDT.

The Q&A document addresses 19 key inquiries from potential offerors regarding technical requirements, operational scope, and system integration. Critical details include: this is a new requirement with no incumbent contractor; the contractor will interpret approximately 15,807 radiology exams annually (8,669 daytime exams and 7,138 after-hours/weekend exams) across modalities including X-ray, CT, MRI, and ultrasound; in-house VA radiologists will continue interpreting exams during weekday business hours (0800-1630); and the contractor is responsible for all equipment and infrastructure on its end of the connection. Technical integration requires connection to the Dayton VAMC Philips PACS system via DICOM through a Laurel Bridge Compass router, HL7 interface compatibility with the Oracle Health EHR platform (post-June 2026 transition), MPLS/VPN connectivity to VISN 10, and support for Query/Retrieve functionality across both PACS and CAMM archives. The contractor must maintain in-house HL7 expertise and propose its own monitoring and dashboard solutions for tracking study status. An MOU/ISA with VA OIT is required before service commencement but not at quotation submission. Past performance evaluation will consider CPARS records, completed Past Performance Questionnaires, and other supporting evidence of successful performance on similar contracts.

10. FAR 12.206 encourages Contracting Officers to consider past-performance information from a wide variety of sources both within and outside the Federal Government, in accordance with FAR Subpart 9.1, FAR 13.106, or FAR Subpart 15.3, as applicable. Accordingly, we respectfully request that the Government consider all available and relevant past-performance information and not limit its evaluation solely to complete Past Performance Questionnaires. If a PPQ is not returned by a customer despite an offeror’s documented good-faith efforts, will the Government accept and consider other supporting evidence of successful past performance?

Answer: Yes. In accordance with (IAW) the Addendum to FAR 52.212-1 Instructions to Offerors, “Provide a minimum of three (3), but not to exceed five (5), recent contracts of similar size, scope and complexity of this requirement.”. Additionally, IAW E.10 52.212-2 Evaluation, “Additionally, each quoter’s Contractor’s Performance Assessment Reporting System (CPARS) record(s) will be reviewed. While the Government may elect to consider data obtained from other sources, the burden of providing current, accurate and complete past performance information rests with the quoter.”

11. Obtaining completed PPQs can be challenging because CORs and other customer representatives may be unavailable or unable to respond by the solicitation deadline. We respectfully request that the Government accept recent and relevant CPARS evaluations in lieu of PPQs.

Answer: As stated for question #10, CPAR evaluations will be reviewed by the Government for the past performance evaluation.

12. PWS §4.6.4.2 states the Site-to-Site VPN connection to VISN 10 will be "configured and maintained by the VAO IT WAN Manager or their designee," while §4.6.5.1 and Section L, Item 4 require the Contractor to "establish and maintain" MPLS connectivity. Can the Government clarify whether the MPLS circuit itself is provisioned/owned by the VA (with the contractor connecting via VPN into the VA's MPLS-capable gateway), or whether the Contractor is required to independently procure and maintain its own MPLS circuit to a demarcation point specified by the VA?

Answer: In environments where Multiprotocol Label Switching (MPLS) is used, secure network segmentation and traffic isolation are provided at the transport layer. While MPLS does not perform authentication itself, it supports secure connectivity between trusted endpoints and may be used in conjunction with VPN or TLS-based authentication protocols. Architectural design must account for certificate trust relationships, VPN endpoint restrictions, and ACLs required to support secure connectivity.

VA Medical Center Joint Security Architecture is a Virtual Private Network (VPN) service providing access to the VPN for authorized Department of War (DOW) and VA users. The service provides mission partners the ability to connect to via a Multiprotocol Label Switching (MPLS) layer 3 VPN.

The vendor will connect to the VA owned MPLS through a VPN

13. §4.6.4.2 states "No GFE will be provided other than the equipment necessary to make this connection." Can the Government identify specifically what equipment (e.g., router, CSU/DSU, VPN endpoint device) will be provided as GFE, and what equipment the Contractor is expected to furnish on its end?

Answer: The Contractor is responsible for furnishing all equipment necessary to fulfill the requirements of the contract. The Government (VHA) will provide the connection point necessary to exchange required information and interfaces but will not furnish any equipment on the contractor's end.

14. PWS §4.6.5.5 requires the Contractor to maintain in-house HL-7 expertise, and Section L, Item 2 requires evidence of "an effective HL-7 interface." Given Dayton VAMC's June 2026 transition to the Oracle Health platform, what specific HL7 order (ORM/OMG) and results (ORU) message specifications does Dayton VAMC now require for the teleradiology interface, and have these changed from the prior VistA/CPRS-based specification referenced implicitly in the PWS?

Answer: Outbound Orders (ORM^O01)

• Uses OBR‑2 (Order ID), OBR‑3 (Accession), OBR‑4.1 (Procedure Code Alias), OBR‑20 (Study Instance UID), modality, patient demographics, etc.

Inbound and Outbound Results (ORU^R01)

• (Receive HL7 ORU) and 4.2.2.5 (Send HL7 ORU)

• Results include OBX segments for findings, structured text, RAD codes, critical/abnormal flags (OBX‑8), addenda handling, preliminary/final status.

Tele‑radiology ORU behavior

• Teleradiology preliminary reports use ORU (optionally MDM).

• OBX‑11 and OBR‑25 carry “P”, “R”, etc.

• Addenda are bundled with final reports.

• RAD Codes transported as OBX CE elements.

15. Will the Contractor's teleradiology system interface with Dayton VAMC imaging through the VA's Enterprise Vendor Neutral Archive (VNA), or through a Dayton VAMC site-specific PACS repository? PWS §4.6.4.1 references "VA EHR Imaging PACS and commercial PACS", can the Government confirm which of these, post-Oracle Health go-live, is now the operative integration point for image receipt and report return?

Answer: The operative integration point is the Dayton VAMC Philips PACS for sending images to the Contractor's teleradiology group via DICOM through Laurel Brige Compass router. However, CAMM integration is also necessary to enable Query/Retrieve (Q/R) functionality across both the PACS and CAMM archives. Reports, however, route via hl7 interface.

The workflow is as follows:

· Image sending: Philips PACS sends DICOM studies, along with associated priors, through a Compass router to the Contractor's teleradiology group. This is the primary pathway.

· Query/Retrieve of priors: Q/R functionality must be supported against both PACS and CAMM. Additional priors may be retrieved through the CAMM archive or PACS. All imaging is brokered through the Compass router.

16. PWS §4.6.4.9 requires the Contractor to provide a transcribed final report "uploaded to the respective VAMC's hospital information system(s) and electronically signed." Post-transition, are teleradiologists required to sign and finalize reports directly within the Oracle Health platform (e.g., via a remote portal session over the MPLS/VPN connection), or will reports continue to be transmitted via an HL7 interface for auto-population into the patient record?

Answer: No. Teleradiologists are not required to log in to or otherwise use the Oracle Health platform/EHR to sign and finalize reports. Teleradiologists Interpret, sign, and finalize reports within the Contractor's managed platform which is transmitted back via hl7 to the Oracle EHR.

17. What are the Government’s required technical and operational procedures for teleradiology order intake, image access, report completion, critical-results communication, and reconciliation during scheduled or unscheduled outages of Oracle Health, PACS, HL7 interfaces, or the primary network connection? Please identify any approved fallback tools or manual workflows.

Answer:

· If the outage prevents image transmission to the contractor, in-house contingency plans will be activated, and the exams will be interpreted and reported by VA radiologists.

· If the outage allows images to continue to be transmitted to the contractor, the contractor will continue to interpret and report. The reports may need to be called in to the ordering provider if the situation warrants. Once the outage is over, the transmission of finalized reports will be handled appropriately by either contractor staff and/or VA staff. The solutions will be put into effect based on the situation.

18. Given Dayton VAMC’s transition to the Oracle Health Federal EHR, can the Government confirm whether the bandwidth, latency, and redundancy requirements in PWS §§4.6.4.1–4.6.5.1 remain accurate for the current production architecture, or whether updated network specifications will be issued by amendment?

Answer: The requirements from the PWS remain accurate currently. If there are significant future changes to the production architecture and/or network specifications, then an amendment will be issued.

19. Section L, Item 5 asks how offerors will track and monitor traffic and whether the VA can monitor study status via a dashboard/portal. Is there a required or preferred monitoring standard/tool, or is the offeror free to propose its own monitoring solution?

Answer: The offeror is free to propose their own solution.

Amendment 0001 is for questions and answers. The closing date has been extended from 7/31/26 to 8/11/26 at 2:00 pm local time.

File details come from the government source that posted it. Updated .
