7.) Quote Format and Structure - To provide a basis for sound evaluation by the Government, Quoters must submit a complete quote. The information provided shall be concise, factual, complete, and demonstrate the Quoter's capacity and capability to satisfactorily perform the requirements outlined in the solicitation. Quotes will be considered only from Quoters that are regularly established in the business and in the judgment of the Government, are deemed financially responsible and able to show evidence of appropriate experience.
b. By submission of an offer, the offeror acknowledges the requirement that the prospective awardee shall be registered in the System for Award Management (SAM) database prior to award. Offerors who are not registered in SAM should consider applying for registration immediately. Registration in SAM is a requirement to conduct business with the Federal Government. If the prospective awardee is not registered in SAM within the timeframe specified by the Contracting Officer, the successful offeror will be removed from further consideration for award. Offerors can register in SAM at https://www.sam.gov.
c. By submission of an offer, the offeror acknowledges the requirement that at the time of offer submission the offeror shall be certified and listed in the SBA certification database located at https://veterans.certify.sba.gov/. This is in accordance with VAAR 852.219-73(b)(1) “offers received from entities that are not certified SDVOSBs and listed in the SBA certification database at the time of offer shall not be considered” and in accordance with VAAR 852.219-73(c) which further states that “pursuant to 38 U.S.C. 8127(e), only certified SDVOSBs listed in the SBA certification database are considered eligible to receive award of a resulting contract.” For a SDVOSB set-aside procurement, if the offeror is not certified and listed on the SBA certification database at the time of offer submission as set forth in the VAAR, the offeror will be removed from further consideration for award.
d. Simplified Acquisition Procedures of FAR Part 12 and 13 are being used to conduct this procurement.
e. Offeror must be certified in the SAM representations and certifications in FAR 52.212-3 that the firm meets the small business size standard for the NAICS Code assigned to the procurement.

STATEMENT OF WORK
1.0 SCOPE
The contractor shall provide all labor, materials, tools, equipment, and supervision to inspect, test, certify, and monitor the Siemens XLSV Fire Alarm System at the Richard L. Roudebush Veterans Affairs Medical Center (RLRVAMC) and the Honeywell Silent Knight system at the Veterans House. Services shall be performed in accordance with National Fire Protection (NFPA) 72, Original Equipment Manufacturer (OEM) specifications, and all applicable Veterans Affairs (VA), federal, state, and local regulations.
2.0 APPLICABLE STANDARDS

· VA Handbook 0730 and VA Directive 0055
· Occupational Safety and Health Administration (OSHA) Standards

3.0 PERFORMANCE REQUIREMENTS
3.1 Annual Inspection and Testing – Due in December yearly
· Functional testing of all initiating devices, notification appliances, fire panels, and interlocks
· Testing of duct detectors, tamper/flow switches, and relays (must be completed after normal business hours)
· Load testing of batteries and uninterrupted power supply (UPS) systems
· Elevator recall, door holder, damper release verification (must be completed after normal business hours)
· Electronic Releasing Devices on fire and smoke doors
· Notification testing from alarm portion only on fire pumps and kitchen hoods
· Documentation of results using Contracting Officer Representative (COR) approved format
3.2 Semi-Annual Services – Due in June yearly
· Visual inspection of devices
· Tamper and flow switch functional testing with pressure documentation
3.3 Fire Alarm Monitoring
· 24/7 Underwriters Laboratory (UL) listed central station monitoring service
· Daily phone line test documentation
· Two phone lines from the VAMC to the monitoring company
· Annual transmission testing between both locations
· Monitoring reports shall be required annually, semi-annually, or on request at any time of the year.
3.4 Documentation
· Contents: All documentation shall include the following:
· Activity
· Date
· Inventory of devices, equipment, or other items
· Frequency
· Contact information of the person performing the activity
· NFPA standard referenced
· Activity results
· Format: Forms shall meet the following criteria:
· All similar devices shall be grouped together by type and location.
· Each applicable device shall have a pass or fail indication.
· For water flow switches, the documentation shall include static pressure, residual pressure, and time to alarm.
· Submission: Documentation shall be provided within fourteen (14) days of completion of the test. Results shall be submitted to the COR within fourteen (14) days of the completion of service. Reports shall be sent to COR in PDF format via email.

3.6 Personnel Qualifications
The contractor's personnel shall be certified and trained in fire alarm systems, possessing relevant certifications such as the National Institute for Certification in Engineering Technologies (NICET) levels two (2) or three (3) in Fire Alarm Systems or an equivalent certification. Technicians shall have substantial experience, specifically with the Siemens XLSV and Honeywell Silent Knight fire alarm systems, and a thorough understanding of NFPA 72: National Fire Alarm and Signaling Code. The contractor shall ensure continuous training and recertification of their technicians to stay updated with the latest advancements in fire alarm technology.
4.0 HOURS OF WORK
· Routine work: Monday through Friday, 8:00 a.m. to 4:30 p.m.
· Disruptive testing (e.g., alarms, devices with secondary functions) shall be completed after normal business hours. All work outside of normal business hours shall be coordinated with and approved by the COR.
· No work on Federal holidays unless approved by COR
· The ten holidays observed by the Federal Government are New Year’s Day, Birthday of Martin Luther King, Jr., Washington’s Birthday, Memorial Day, Juneteenth National Independence Dat, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. Also, any other day declared by the President of the United States to be a national holiday.
5.0 PLACE OF PERFORMANCE
· Richard L. Roudebush VA Medical Center: Located at 1481 W. 10th Street, Indianapolis, IN 46202. This includes the following buildings: 1, 5, T6, 19, 21, 22, 23, and the employee parking garage.
· Cold Springs Road Campus: Located at 2669 Cold Springs Road, Indianapolis, IN 46202. This campus includes the following buildings: 7, 9, 42, and 43.
6.0 SECURITY REQUIREMENTS
· All contractor personnel shall wear VA-issued or contractor-supplied photo ID and uniforms
· Compliance with VA security and access requirements
· Smoking, weapons, and contraband are prohibited
· The C&A requirements do not apply; a Security Accreditation Package is not required
7.0 DELIVERABLES
· Annual and semi-annual test reports
· Monitoring signal reports
· Deficiency list with recommendations
8.0 PERFORMANCE MONITORING
· The government reserves the right to inspect services at any time
· Failure to meet contract terms may result in re-performance or corrective actions
9.0 GOVERNMENT-FURNISHED EQUIPMENT (GFE)/GOVERNMENT-FURNISHED INFORMATION (GFI)
The contractor shall provide all equipment, tools, materials, vehicles, and personnel necessary to perform the services described. Government property shall not be used.
10.0 COR AUTHORITY
The COR is authorized to monitor all technical aspects of the contract. The COR shall not make any changes to the terms and conditions of the contract. Any changes shall be authorized via written modification by the Contracting Officer.
