PERFUSIONIST SERVICES

Amendment 0001 – Questions and Answers

1. Who is the incumbent vendor? What is the contract number?

Answer: Circulatory Services, Inc., 10904 SW 27th Avenue, Gainesville, FL 32608. Contract Number 36C25019C0086.

1. What is the expected award date? What is the expected performance start date? Will the VA agree to have the performance start date no sooner than 60 days from the award?

Answer: The exact expected award date is unknown, but we anticipate awarding mid-January. The effective date of the award (performance start date) is 2/15/2025. The VA requires a start date of 2/15/2025, this does not change.

1. Consistent with common practice nationally in the perfusion field, would the VAMC allow the contractor to provide a transitional bridge team until the permanent team is onboarded?

Answer: No

1. How often are perfusionists called to cover cases outside normal hours?

Answer: Historically, it has been approximately one to two times per month.

1. What is the estimated number of concurrent perfusion cases by day of the week? Maximum concurrency vs average concurrency.

Answer: less than one.

1. Will VA consider extending the on-call response time from 30 to 60 minutes?

Answer: No.

1. How many Cardiac Surgeons do you have? Are the Cardiac Surgeons Government employees or contractors?

Answer: Five, and they are VA employees.

1. Is the Perfusionist required to be in the hospital even when no cases are scheduled?

Answer: Yes, primarily for Thoracic and occasionally cardiac catheterization lab.

1. What is the historical volume of perfusion cases (not hours) for 2023 and 2024 YTD? How many average hours per case?

Answer: 244, 245, averaging eight hours per case.

1. How many non-cardiac cell saver cases does this VAMC do annually? Will the perfusion contractor be responsible for these?

Answer: No non-cardiac cell saver cases are performed at this site.

1. Are there any services beyond open hearts that the perfusion contractor must cover? Examples: PRP, Transplants, Non-Cardiac Cell Savers/ATS, HIPEC, NRP. If yes, please provide estimated annual volumes of each that require support.

Answer: Just Thoracic Cases, none of the others listed.

1. For ECMO, how many cases are started at the VAMC annually? How many ECMO hours, on average, are monitored at the VAMC? How many ECMO patients are transported to other facilities? Will the perfusionist be required to transport it?

Answer: Approximately two cases a year. Usually all cardiac cases are around eight hours. Historically, the ECMO patients are transported to other facilities. The Cleveland VAMC will transport.

1. Who provides the perfusion-related supplies: the perfusion contractor or the hospital?

Answer: Cleveland VAMC

1. Section 5.1 lists government-provided equipment. This list does not include ECMO machines. Will the government provide ECMO equipment and supplies?

Answer: Cleveland VAMC will provide.

1. Current Services: Does the medical center have issues receiving perfusionist services?

Answer: No.

1. PWS Section 3: VA Hours of Operation/Scheduling,“ 3.1.1. Work Schedule: Regular work hours are 6:30am – 4:30pm Monday through Friday will be on an as needed basis to support scheduled surgeries. Occasional excursions from regular work hours will be required depending on the surgical schedule and patient care needs” If the government requires significant call back outside of the outlined 7280 hours per year expected for the positions, how will this be billed to the government? Would the government consider adding a separate pool of “call back hours” differentiated from the standard CLIN, billed at the same rate as standard hours?

Answer: As seen in our answer to #4, we do not anticipate a significant number of call back hours and there is enough hours built into the contract maximum to cover.

1. PWS Section 3.1.3: Off Hours Coverage “The contractor shall provide one (1) full time Perfusionist for on-call services 24-hours a day and seven (7) days a week including all Federal Holidays. Contactor shall report to the facility within 30 minutes of notification of an unscheduled or emergency case. On-call service is considered part of the Perfusionist services and will not be paid separately. Any hours worked during an on-call status for an emergency procedure shall be paid the same rate as stated in the appropriate LIN/SLIN in Section B.2 Schedule of Services “0002 - Overtime Rate: Base pay for hours worked over 40 hours per week”

It is stated that the contractor is expected to have at least one available Perfusionist on call 24/7 to meet the needs of the site. Is this call responsibility shared at all with permanent staff at the VA, or is it the sole responsibility of the contractor? Is there an expected hourly amount of on call hours or is it any hour during the period of performance that a contractor is not present on site? Understanding your statement that call will not be line itemed separately, can we clarify how this will be monitored by the government for scheduling and billing purposes?

Answer: Contractor provides on-call responsibility. See PWS 3.1.3, the contractor shall provide one (1) full time Perfusionist for on-call services 24-hours a day and seven (7) days a week including all Federal Holidays. Submission of on-call hours considered the same as working hours, so no special rate or separate billing.

1. Could you please confirm the correct set-aside if it is for Small business or Veteran-Owned Small Business?

Answer: This is a VOSB set-aside. See page 1 of the SF1449, in Block 20 it states, “This Procurement Is Set-Aside 100% For Veteran Owned, Small Business Concerns (VOSB)”.
