STATEMENT OF WORK
I. DESCRIPTION OF SERVICES:
GENERAL:
The contractor shall provide all personnel, equipment, tools, materials, supervision, transportation, and other items and services necessary to perform the replacement of the (6) overhead doors as defined in this Statement of Work (SOW). This task includes removal and replacement of the overhead dock doors in Building 1 at the VA Northern Indiana Health Care System –Fort Wayne Campus at 2121 Lake Avenue, Fort Wayne, IN 46805.
SCOPE:
Automatic Doors shall consist of the following:
A. Removal and disposal of existing doors, tracks, and springs.
1. The contractor shall remove all existing door package as specified per task order and dispose of it properly off site. The government will not provide dumpsters or access to government-controlled dumpsters to the contractor.
B. ICRA barrier required per infection control requirements.
C. Install 6 new dock doors.
1. Doors shall be 1 3/4” or 1 5/8” thick with steal front and back.
2. Doors shall have urethane injected to meet an R14 insulation factor.
3. The contractor shall perform an on-site inspection of the work area to determine/verify dimensional requirements. It is the responsibility of the contractor to notify the COR, within two (2) working days prior to performance start date, of any discrepancies in the dimensions between the data provided by the government and the contractor’s survey.
4. Doors, tracks, springs, and hardware shall be installed as per manufacturer’s specifications.
5. Installer Qualifications: The contractor must be a company specializing in exterior door installation with a minimum three (3) years’ experience and a valid OSHA 10 certification.
D. Evaluation Controllers.
1. Installer shall evaluate current controllers to determine remaining life expectancy of the controllers.
2. Installer shall inspect the controllers for any failing parts.
E. Remove Rubbish
1. The contractor shall remove all waste daily. The contractor shall not leave removed waste material inside the facility overnight.
2. The contractor shall provide an inventory of waste items by weight to the VA.
CONTRACTOR-FURNISHED ITEMS AND SERVICES:
A. Contractor-Furnished Material Submittals
1. The contractor shall be responsible for providing the necessary materials/supplies required to perform the requirements. This includes, but not limited to, the doors, frames, hardware, etc. for a complete installation and finished appearance. A set of shop drawings shall be provided prior to material submittals.
B. Notification to Commence Work
1. The contractor shall begin work installation within five (5) working days after service notification is received from the contracting officer. The contractor will receive work order information from the COR by means of face-to-face communication, e-mail, or phone and acknowledgment of job order must be submitted to COR.
GENERAL INFORMATION:
A. Project Schedule
1. A project schedule shall be developed and approved by the COR no later than ten (10) days after contract award.
B. Quality Control
1. The contractor shall develop and maintain a quality control program to ensure the specifications of the contract are provided in accordance with the standards accepted in commercial practices. The contractor shall develop and implement procedures to identify and prevent defective services from recurring. The contractor shall submit the Quality Control Plan (QCP) no later than ten (10) days after contract award.
C. Material Application Guarantee
1. The contractor shall guarantee the workmanship for a one-year period from the date of completion.
D. Quality Assurance
1. The government will periodically evaluate the contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received. The government representative will evaluate the contractor’s performance through intermittent on-site inspections of the contractor’s quality control program.
2. The government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections.

1. The contractor may perform the on-site services required under this contract during business hours.
2. The medical center clinical business hours are 8:00am - 4:30 pm, Monday through Friday, except on U.S. holidays.
F. Phasing
1. The docks are in constant use during working hours, not only for deliveries but also recycling. Therefore, the project must be done in 2 phases, keeping two of the docks and 1 recycling bay open at all times. It is the CONTRACTOR'S responsibility to develop a work plan and schedule detailing, at a minimum, the procedures to be employed, the equipment and materials to be used, the interim life safety measure to be used during the work, and a schedule defining the duration of the work with milestone subtasks.
G. The Contractor shall coordinate project expectations with the subcontractors to ensure everything in the SOW and on the specifications and drawings is included in subcontractor bids and work is properly completed prior to the final inspection.
H. Contractor shall provide a site-specific safety plan.
I. Formal review of project requirements shall include but is not limited to:
1. Designation of COR, Task Order and Site-Specific Information, Infection Control, Interim Life Safety Measures, Safety, Badging and Identification requirements, parking access to premises and building; space for material and equipment storage; delivery of materials and use of approaches; and use of corridors, stairways, elevators, eating spaces, and restrooms for Contractor personnel.
J. The CO will issue the Notice to Proceed before any work or mobilization is authorized to begin.
K. Contractors shall clearly include the solicitation number (prior to award) or the contract number (as appropriate) on all documents submitted to the VA.
L. TO Identification: After successful award of each task order the contractor shall clearly include the contract number, task order number, obligation number and project number on all correspondence submitted to the VA.
M. The solicitation number, contract number, task order number, obligation and project numbers are displayed and be easily found on all official government documents issued by the VA.
OPERATIONS AND STORAGE AREAS:
A. The Contractor shall confine all operations (including storage of materials) on Government premises to areas authorized or approved by the Contracting Officer. The Contractor shall hold and save the Government, its officers, and agents, free and harmless from liability of any nature occasioned by the Contractor's performance.
B. Working space and space available for storing materials shall be determined by the COR.
C. Laborers are subject to rules of Medical Center. Execute work to interfere as little as possible with normal functioning of Medical Center as a whole, including operations of utility services, fire protection systems and any existing equipment, and with work being done by others. Use of equipment and tools that transmit vibrations and noises through the building structure, are not permitted in buildings that are occupied, during construction, jointly by patients or medical personnel, and Contractor's personnel, except as permitted by the COR.
INFECTION CONTROL:
A. Do not store materials and equipment in other than assigned areas. Schedule delivery of materials and equipment to immediate construction working areas within buildings in use by Department of Veterans Affairs in quantities sufficient for not more than two workdays. Provide unobstructed access to Medical Center. Where access by Medical Center personnel to vacated portions of buildings is not required, storage of Contractor's materials and equipment will be permitted subject to fire and safety requirements.
B. Contractor shall take all measures and provide all material necessary for protecting existing equipment and property in affected areas of construction against dust and debris, so that equipment and affected areas to be used in the Medical Centers operations will not be hindered.
C. Contractor shall permit access to Department of Veterans Affairs personnel and patients through other construction areas which serve as routes of access to such affected areas and equipment. These routes whether access or egress shall be isolated from the construction area by temporary partitions and have walking surfaces, lighting etc. to facilitate patient and staff access. Coordinate alteration work in areas occupied by Department of Veterans Affairs so that Medical Center operations will continue during the construction period.
D. Infection Control is critical in all medical center facilities. Interior construction activities causing disturbance of existing dust, or creating new dust, must be conducted within ventilation-controlled areas that minimize the flow of airborne particles into patient areas. An AHA associated with infection control will be performed by VA personnel in accordance with FGI Guidelines (i.e., Infection Control Risk Assessment (ICRA)). The ICRA procedure found on the American Society for Healthcare Engineering (ASHE) website will be utilized. All task orders associated with this contract will be Class III or lower Risk Classification and will require approval by the Project Manager or Government Designated Authority before beginning any construction work.
E. In general, the following preventive measures shall be adopted during construction to keep down dust and prevent mold.
1. Exhaust hoses shall be exhausted so that dust is not reintroduced to the medical center.
2. Adhesive Walk-off/Carpet Walk-off Mats shall be used at all interior transitions from the construction area to the occupied medical center area. These mats shall be changed as often as required to always maintain clean work areas directly outside construction area.
F. Vacuum and wet mop all transition areas from construction to the occupied medical center at the end of each workday. Vacuum shall utilize HEPA filtration. Maintain surrounding area frequently. Remove debris as it is created. Transport these outside the construction area in containers with tightly fitting lids.
G. The contractor shall not haul debris through patient-care areas without prior approval of the COR. When, approved, debris shall be hauled in enclosed dust proof containers or wrapped in plastic and sealed with duct tape. No sharp objects should be allowed to cut through the plastic. Wipe down the exterior of the containers with a damp rag to remove dust. All equipment, tools, material, etc. transported through occupied areas shall be made free from dust and moisture by vacuuming and wipe down.
H. There shall be no standing water during construction. This includes water in equipment drip pans and open containers within the construction areas. All accidental spills must be cleaned up and dried within 12 hours. Remove and dispose of porous materials that remain damp for more than 72 hours.
I. At completion, remove construction barriers and ceiling protection carefully, outside of normal work hours. Vacuum and clean all surfaces free of dust after the removal.
DISPOSAL AND RETENTION:
A. Materials and equipment accruing from work removed and from demolition of buildings or structures, or parts thereof, shall be disposed of as follows:
1. Reserved items which are to remain property of the Government shall be specified by the COR in each task order and shall be identified by attached tags or noted on drawings or in specifications as items to be stored.
2. Items that remain property of the Government shall be removed or dislodged from present locations in such a manner as to prevent damage which would be detrimental to re-installation and reuse.
FINAL CLEANUP:
A. Upon completion of project, or as work progresses, remove all construction debris from above ceiling, vertical shafts and utility chases that have been part of the construction. Perform HEPA vacuum cleaning of all surfaces in the construction area. This includes walls, ceilings, cabinets, furniture (built-in or free standing), partitions, flooring, etc.
PERIOD OF PERFORMANCE:
A. Contractor shall complete project with 30 days of notice to proceed. Additional time can be provided at no cost to the government to account for material delays or long lead items as approved during the submittal process.
PAYMENT FOR SERVICES:
A. Payment is to be made in arrears by certified invoice. Failure to provide purchase order number on the invoice may result in the return of the invoice.
B. Invoices shall be certified by the COR prior to submission in Tungsten.
"No Government personnel, other than the Contracting Officer, have the authority to change or alter these requirements. The COR shall clarify technical points or supply relevant technical information, but no requirements in this scope of work may be altered as a sole result of such verbal clarification."

PERFORMANCE PERIOD: As stated in SF 1442 block 11, the contractor shall (a) commence work under this contract after receiving the notice to proceed within the days specified, (b) prosecute the work diligently, and (c) complete the work, clean the work site completely and return the premises to the VA ready for us within the number of days specified.
RESPONSIBILITY REQUIREMENTS: A responsibility check will be accomplished for the apparent awardee prior to award. SECURITY
BACKGROUND INVESTIGATIONS: All contractor employees are subject to the same level of investigation as Dept of Veterans Affairs (VA) employees who have access to sensitive information. The level of background investigation is commensurate with the level of access needed to perform the statement of work. This requirement is also applicable to all subcontractor personnel requiring the same level of access.
Levels and associated types of Background Investigations:
· If the project requires that workers have only limited physical access to the VA facility for a period of one year or less, a local flash badge with fingerprint check is issued.
· If the project requires that workers require un-supervised, logical and/or physical access for 180- consecutive or aggregate days in a 365-day period, over a three-year period, a Special Agency Check (SAC) background investigation is required which must be completed prior to being allowed access.
· If the project requires workers have un-supervised, full-time, logical, and/or physical access for more than six months or more than 180 aggregate days in a one-year period, a SAC and National Agency Check with Written Inquiries (NACI) background investigation is required; access is not allowed until a favorable SAC completion.
Supplemental Information
▪ Seasonal influenza with Vaccines (See attached VHA Directive 1192.01)
The influenza season is generally considered to span the four-month period from December 1 through March 31; however, it can vary from one season or geographic location to another. Individual VHA facilities may expand (but not reduce) this time period based on local influenza activity and other epidemiological circumstances.
Health Care Personnel (HCP). HCP are individuals who, during the influenza season, work in VHA locations or who come into contact with VA patients or other HCP as part of their duties. VHA locations include, but are not limited to, VA hospitals and associated clinics, community living centers (CLCs), community-based outpatient clinics (CBOCs), domiciliary units, Vet centers and VA-leased medical facilities. HCP include all VA licensed and unlicensed, clinical and administrative, remote and onsite, paid and without compensation, full- and part-time employees, intermittent employees, fee basis employees, VA contractors, researchers, volunteers and health professions trainees (HPTs) who are expected to perform any or all of their work at these facilities. HPTs may be paid or unpaid and include residents, interns, fellows and students. HCP also includes VHA personnel providing home-based care to Veterans and drivers and other personnel whose duties put them in contact with patients outside VA medical facilities.
INSTRUCTIONS, CONDITIONS AND OTHER STATEMENTS TO BIDDERS/OFFERORS
2.1 52.222-5 CONSTRUCTION WAGE RATE REQUIREMENTS—SECONDARY SITE OF THE WORK (MAY 2014) (a)(1) The offeror shall notify the Government if the offeror intends to perform work at any secondary site of the work, as defined in paragraph (a)(1)(ii) of the FAR clause at 52.222-6, Construction Wage Rate Requirements, of this solicitation.
(2) If the offeror is unsure if a planned work site satisfies the criteria for a secondary site of the work, the offeror shall request a determination from the Contracting Officer.
(b)(1) If the wage determination provided by the Government for work at the primary site of the work is not applicable to the secondary site of the work, the offeror shall request a wage determination from the Contracting Officer.
(2) The due date for receipt of offers will not be extended as a result of an offeror's request for a wage determination for a secondary site of the work.

2.2 52.222-23 NOTICE OF REQUIREMENT FOR AFFIRMATIVE ACTION TO ENSURE EQUAL EMPLOYMENT OPPORTUNITY FOR CONSTRUCTION (FEB 1999)
(a) The offeror's attention is called to the Equal Opportunity clause and the Affirmative Action Compliance Requirements for Construction clause of this solicitation.
(b) The goals for minority and female participation, expressed in percentage terms for the Contractor's aggregate workforce in each trade on all construction work in the covered area, are as follows:

2.3 52.225-10 NOTICE OF BUY AMERICAN REQUIREMENT—CONSTRUCTION MATERIALS (MAY 2014)
(a) Definitions. "Commercially available off-the-shelf (COTS) item," "construction material," "domestic construction material," and "foreign construction material," as used in this provision, are defined in the clause of this solicitation entitled "Buy American—Construction Materials" (Federal Acquisition Regulation (FAR) clause 52.225-9).
(b) Requests for determinations of inapplicability. An offeror requesting a determination regarding the inapplicability of the Buy American statute should submit the request to the Contracting Officer in time to allow a determination before submission of offers. The offeror shall include the information and applicable supporting data required by paragraphs (c) and (d) of the clause at FAR 52.225-9 in the request. If an offeror has not requested a determination regarding the inapplicability of the Buy American statute before submitting its offer, or has not received a response to a previous request, the offeror shall include the information and supporting data in the offer.
(c) Evaluation of offers.
(1) The Government will evaluate an offer requesting exception to the requirements of the Buy American statute, based on claimed unreasonable cost of domestic construction material, by adding to the offered price the appropriate percentage of the cost of such foreign construction material, as specified in paragraph (b)(3)(i) of the clause at FAR 52.225-9.
(2) If evaluation results in a tie between an offeror that requested the substitution of foreign construction material based on unreasonable cost and an offeror that did not request an exception, the Contracting Officer will award to the offeror that did not request an exception based on unreasonable cost.
(d) Alternate offers.
(1) When an offer includes foreign solicitation in paragraph (b)(2) of the clause at FAR 52.225-9, the offeror also may submit an alternate offer based on use of equivalent domestic construction material.
(2) If an alternate offer is submitted, the offeror shall submit a separate Standard Form 1442 for the alternate offer, and a separate price comparison table prepared in accordance with paragraphs (c) and (d) of the clause at FAR 52.225-9 for the offer that is based on the use of any foreign construction material for which the Government has not yet determined an exception applies.
(3) If the Government determines that a particular exception requested in accordance with paragraph (c) of the clause at FAR 52.225-9 does not apply, the Government will evaluate only those offers based on use of the equivalent domestic construction material, and the offeror shall be required to furnish such domestic construction material. An offer based on use of the foreign construction material for which an exception was requested—
(i) Will be rejected as nonresponsive if this acquisition is conducted by sealed bidding; or
(ii) May be accepted if revised during negotiations.
