RE: ENVIRONMENTAL INSPECTION SERVICES

MARION VETERANS AFFAIRS MEDICAL CENTER, BUILDING 16

ALLIANCE ENVIRONMENTAL GROUP PROJECT NUMBER 20-0035-ALP

Dear Ms. Smith;

Pursuant to your request beginning July 6, 2020 Alliance Environmental Group (Alliance) conducted a pre-renovation inspection of Building 16 at the Marion, Indiana, VA Medical Center for any suspect asbestos, lead paint and hazardous materials. The following serves to summarize results of the inspection performed and copies of the asbestos and lead data sheets from the inspection (Alliance project # 20-0035-ALP) are attached for your review.

ASBESTOS SAMPLING

Alliance was provided with an asbestos inspection report from 1993 which listed materials asbestos content without laboratory analytical. For purposes of this inspection, suspect materials identified in the 1993 report as ACM were included in the report without any further laboratory analysis. Remaining suspect ACM found by Alliance within the facility was collected and subsequently analyzed by EMSL Analytical, Inc. utilizing polarized light microscopy (PLM).

State and local regulations define asbestos-containing material as any material containing greater than 1% asbestos.

Suspect asbestos-containing material samples were taken from cove base and associated mastic, drywall, stair tread and associated mastic, ceiling tile, plaster and surfacing material, thermal system insulation (TSI), window glazing and window caulking, and old electrical wiring. The laboratory results and previous inspections indicate the following materials are asbestos-containing:

- Black mastic and associated floor tiles

- Fire door fill (old elevator)

- Linoleum (old elevator)

- Vibration cloth (attic HVAC ducts)

- Exterior window caulk (aluminum frame windows)

Due to the scope of work and the current state of the building, attempts were made to identify and quantify all asbestos-containing materials, however, some asbestos-containing materials may be present in inaccessible areas or voids in walls or above ceilings. If any suspect regulated asbestos-containing materials are discovered during demolition activities, work should cease until the material can be sampled by a licensed asbestos inspector or assumed to be asbestos and removed by a licensed asbestos abatement company. Fire doors can not be sampled at this time and should be inspected and sampled before disposal.

The state of Indiana licensed inspectors for this project were Michael Ardis: license number 19A007304, expiration date: 04/03/2020 and Sean Stults: license number 19A007184, expiration date: 01/23/2020.

LEAD PAINT SAMPLING

Paint chip samples were collected and subsequently analyzed by EMSL Analytical, Inc. utilizing Method SW-846- 3050B*/7000B. Specifically, a paint chip sample was collected from delaminated and/or peeling cream paint on the concrete ceiling deck. The laboratory reported that cream painted concrete ceiling deck surfaces throughout the building are lead-containing. There is approximately 2,200 square feet of peeling and/or delaminated cream ceiling paint throughout the building. Post-abatement, solid waste must be analyzed via the Toxic Characteristic Leaching Procedure (TCLP) to determine if the waste is hazardous.

Environmental Inspection Services Page 2 Marion VAMC August 10, 2020

Alliance also utilized an Innov-X Systems X-ray fluorescence (XRF) spectrometer to analyze non delaminated and/or peeling painted surfaces. The XRF identified the following painted surfaces as lead-containing:

- Green Ceiling Plaster – approximately 3,600 square feet

- Interior and Exterior White Wood Windows – approximately 420 square feet

- White Ceramic Tile Walls – approximately 300 square feet

- Gray Doors and Frames throughout Building – approximately 100 units

- Blue/Green Columns and Walls – approximately 550 square feet

- Fire Suppression Piping – approximately 500 linear feet

- Off/White Plaster Walls throughout Building – approximately 24,000 square feet

- White Panels above Exterior Windows – approximately 640 square feet

- Exterior Black Stair Railings – approximately 100 linear feet

- Exterior Small Red Window Grate – approximately 20 square feet

- Exterior Gutter Red Downspout – approximately 100 linear feet

- Exterior White Trim on Roof and Front Entrance – approximately 600 square feet

PCB SAMPLING

EPA regulations define PCB-containing material as any material containing greater than 50 ppm of PCBs. Alliance collected two samples of window caulk from framed windows and two samples of elevator hydraulic fluid. The laboratory reported that all samples collected contain PCB concentrations below 50 ppm.

At the time of inspection, the out-of-service elevator was not able to be inspected. Prior to commencement of renovation activities, Alliance recommends inspection of any oil reservoir associated with this elevator.

HAZARDOUS MATERIAL CATALOGING

Hazardous materials which may be impacted my renovation of building 16 were cataloged. The approximate quantities of the hazardous materials are as follows:

- Ballasts – 640 units

- Compressors requiring possible refrigerant recovery – 50 units

- Electronic Waste – 315 units

- Exit signs – 40 units

- Florescent bulbs – 1,280 units

- Hydraulic door openers – 74 units

- Elevator hydraulic fluid – 60 cubic feet

- Smoke detectors – 16 unit

- Switches which may contain mercury – 2 units

Ms. Lynn, Alliance appreciates the opportunity to have been of service to you and the Marion VAMC. Please contact the undersigned if you require any additional information.

Sincerely, Sean Stults Project Manager Alliance Environmental Group

1.0 INTRODUCTION & EXECUTIVE SUMMARY

Mabbett and Associates, Inc. (Mabbett®), with ECS Midwest, LLC as a sub-contractor, performed surveys for suspect asbestos containing building materials (ACM) and screenings of suspect lead-based paint on building surfaces utilizing an X-Ray Fluorescence (XRF) analyzer. Surveys were performed of Building 16 at the Marion VA Medical Center (Marion VAMC) located at 1700 East 38th Street in Marion, Indiana, under Contract GS-10F-0120T. Site survey work was performed on December 9, 2019 for lead, and on February 13, 2020 for ACM, by appropriately credentialed personnel as required. Collected suspect ACM samples were submitted to an analytical laboratory certified as an environmental testing laboratory pursuant to the provisions of the Health and Safety Code for bulk asbestos analysis by polarized light microscopy (PLM) using positive stop methodology. The survey effort involved the collection or screening of the following samples, resulting in the following conclusions:

One hundred and forty-six (146) bulk samples for suspect ACM were collected in this building.

Based on laboratory analysis of suspect ACM, fifteen (15) of the collected samples contained asbestos greater than 1%.

One (1) additional material was previously identified as ACM or Presumed ACM (PACM).

One hundred and twenty-four (124) lead screening measurements were taken in this building.

Twenty-five (25) lead screening measurements were greater than 1.0 mg/cm2.

This building report consists of a summary of findings, floor plans that indicate positive sample locations, detailed analytical findings for the specific surveyed building materials, relevant photos, and laboratory analytical reports.
