A.1 Valet Service for Cincinnati VA Medical Center, Question and Answers:

Q1: Can you tell me if there is an incumbent? If so, contract information?

Contracting: APEX Transit Solutions, Inc is the incumbent. 36C25023D0013 is the current bridge/gap contract by APEX Transit Solution, Inc.

Q2: I’m emailing regarding the above titled solicitation. Can you kindly advise what the contract duration is?

Contracting: Delivery Schedule has the Period of Performance (PoP) for each CLIN. The estimate PoP is as follows:

· Base: 1 July 2023 to 30 June 2024

· OY1: 1 July 2024 to 30 June 2025

· OY2: 1 July 2025 to 30 June 2026 Q3: Page 16- BASE RATE. The base rate is defined as the rate paid for transportation from a designated pick-up point to a designated delivery point. This rate will be paid for all authorized trips requested under this contract with employees receiving at a minimum the Department of Labor Service Contract Act (SCA) wage rates for the CUYAHOGA COUNTY, as specified by the contracting officer for this requirement, during all phases of the trip made on behalf of the Government. Service Contract Act (SCA) wage rate determinations will be updated each option year, when/if new revisions are issued. The base rate shall constitute full compensation for trips.

Should this be read - wage determination (WD) hourly rate) in the county which the primary VA medical facility supported for this requirement resides?

COR: The contracting officer will determine which wage determination (i.e. Warren, Ravenna) that will be used.

Contracting: The employees for the vendor while providing services for this contract that require them to enter onto federal property will be paid at the minimum rate which is listed in Department of Labor (DoL) Wage Determination (WD) plus Health and Wealth fare (H&W) for Cuyahoga, Guega, Lake, Lorain and Medina Counties in Ohio. WD 2015-4727 (Rev 20) covers all these counties and will be used until a new revision is published by DoL. The Government will not dictate which labor category that is providing the service.

Q4: 5.2.2 5.2.2 Scheduled Patient Transport Requests. The government will normally send to the contractor by 1530-EST each business day by encrypted email all scheduled patient transport requests for the next business day. By 1600-EST each day, the contractor shall confirm via encrypted email the receipt of all patient transport requests for the next business day. Requests or changes will only be made by the authorized government representative(s) designated in writing by the COR and/or CO. Changes will be made by encrypted email, telephone, or fax which shall be confirmed immediately upon receipt by the contractor. A log for all trips will be maintained by the authorized government representative(s) designated in writing by the COR and/or CO. A list of VA personnel authorized to place patient transport requests will be made available upon award of the contract. VA employees may be added or deleted from the list during the term of the contract at the discretion of the COR and/or CO in writing. The contractor will be notified of such changes as they occur. The awarded contractor should establish a system to remind patients that they are scheduled for transportation with the contractor, with at least one (1) day advance notice. When ordering services (whether it be by telephone, facsimile, mail, e-mail and/or in person), the VA Medical facility will provide the contractor with the following type of information:

5.2.5 The Cleveland Travel Department uses VETRIDE portal to assign trips to the contractor. The contractor must be set up in the VETRIDE Third Party portal to receive transportation requests. The contractor must actively use the VETRIDE Third Party portal daily and accept/deny 100 % of trips assigned to said contractor within the portal. The contractor must also enter mileage and/or cost information for each trip in VETRIDE Third Party portal that is not automatically calculated. Transportation requests will only be sent to the contractor via fax or email as a secondary option; in the event there are technical issues with the VETRIDE portal, or the VA Travel Department staff determines that due to the urgency of a transportation request that the portal is not conducive. Once the technical issues have been resolved and/or the urgent issues have been addressed then all transportation requests that were faxed or emailed will be submitted through the VETRIDE Third-Party Portal and the contractor should complete all the necessary actions. The Travel Department staff will provide training on the VETRIDE portal to the contracted vendor upon the award of this contract Will both ways listed above be utilized to assign trips to the contractor? And if so, how will they work in conjunction together?

COR: Both ways will not be used simultaneously unless advised otherwise.

Q5: 3.2.2 Base Rate. The BASE RATE as established in the SCHEDULE OF SERVICES – PRICE SCHEDULE is for authorized patient transports with a point-to-point. The contractor shall utilize the latest edition of Rand McNally maps to calculate the travel distance for all point-to-point trips, or other agreed upon source in the event website outage.

5.3.2.3 Mileage Rate. The MILEAGE RATE as established in the SCHEDULE OF SERVICES – PRICE SCHEDULE shall only be authorized for patient transports at the point of pick up to the drop off point. MILEAGE will only be paid when a VA beneficiary is being transported. There will be no payment for mileage accrued without a VA approved passenger onboard. The contractor shall utilize the latest edition of Rand McNally maps to calculate the travel distance for all point-to-point trips, or other agreed upon source in the event website outage.

Rand McNally was deconditioned last year. Is there another accurate and consistent system being used to calculate the travel distance for all point-to-point trips?

COR: Per previous convo please remove rand Mcnally language as it is no longer in service, we currently use bing maps. However, once we start using VetRide that portal will cal. All trips mileage for them Q6: 5.2.4 Wait Time | Patient Transport Requests.

5.3.2.4 Wait Time Rate (Cancellation on Site).

Is the Item number on the price/cost schedule used for the 15-minute increments or the COS charge after waiting the 15 minutes waiting and the contractor is unable to perform a scheduled pick-up for reasons beyond the contractor’s control, i.e., incorrect address, or patient absence, or patient refusal? And what is the average wait time currently for the pick-ups before charged wait-time? before a COS charge is incurred??

COR: There is not a cost schedule for the 15 minute increments as we do not utilize it. The cos charge is after the 15 mins of waiting. 15 mins is the average wait time, after that a cos charge will apply.

Q7: MILAGE RATE. Mileage rate for service is the rate paid for each mile traveled beyond the specified mileage threshold for each applicable medical facility.

What is the specified Mileage threshold when calculating mileage for this contract?

COR: There is no mileage threshold. Mileage rate is paid point to point. Please see section 5.3 Q8: 1.2.1.8 A log to account for all requests for service. The log shall indicate the date and time of service call, name of beneficiary requiring services, any required Medicare/Medicaid forms with Doctor’s signatures, type of transportation requested, designated pick-up and delivery points, actual time of arrival at pick-up and delivery points and actual waiting time at pick-up and delivery points, if waiting charges are claimed.

Is the contractor required to obtain Medicare/Medicaid forms or signatures from the special mode travel beneficiaries?

COR: Signatures are not normally needed from Staff/Patients. Medicare or Medicaid forms are not used at the time.

Q9: Is Executive Order 14043 still in effect for this solicitation/contract. This mandate involves the vaccine mandate.

Contracting: No, Executive Order 14043 has no effect on this solicitation. Executive orders 14042 and 14043 have been revoked and no longer may be enforced. Therefore, 52.223-99 is not in this RFP.

Q10: For the fuel surcharge calculator, what is included in the price per mile and wear and tear per mile?

Contracting: What the vendor counts toward their “wear and tear” is not information that will be evaluated. It is the Government’s intent to establish a constant in the gas formula that counts for maintenance and use of the vehicles while enabling EPA—Fuel charge. When fuel cost fluctuates (increase or decrease) the change in rate for milage is only the delta in fuel cost per mile. Here is an example using figures that are in no way are realistic to the price per mile. It is purposely unrealistic, so it is understood to be used for an example of the formula and not what the Government is expecting to see as a rate.

Initial

a) Fuel Avg for last 3 month (Per Gallon) : $1.00

b) MPG for vehicle established in Proposal (Per Gallon) : 10

c) Average Price per mile : $0.10 ( a ÷ b )

d) Cost of “wear and tear” (Per Mile) : $0.30

e) Proposal Price per mile : $0.40 per mile ( c + d ) Next Quarter

f) Updated Fuel Avg for last 3 months (Per Gallon) :$1.50

g) MPG for vehicle established in Proposal (Per Gallon) : 10

h) Average Price per mile : $0.15 ( f ÷ g )

i) Cost of “wear and tear” (Per Mile) : $0.30

j) New Price per mile : $0.45 per mile ( c + d )

Q11: How are the volume and milage calculated for this contract? It appears that there has been a significant increase in the number of trips and milage when compared to the last contracts’ totals. For the Base Rate do we give the rate of the pickup and drop off fee i.e., round trip or just the cost of the single leg pick up?

COR: Yes, there has been increase. The volume was based off estimates from previous trends. Base rate is calculated for each leg of the transport.
