CONTINUATION PAGE
A.1 QUESTIONS AND ANSWERS
Question: “Which category in the wage determination should be used for this contract? (i.e. janitor, housekeeper, etc.) Employee Class:”
Answer: Employee Class is 11122 Housekeeping Aide; see Amendment 0001.
Question: “Is the total mat service count 106 total mats? Please verify.”
Answer: Count is 75 carpet mats and 21 rubber “Kitchen Mats”. The winter mats are to be removed and replaced with summer mats (carpet mats) in the summer time.
Question: “Some of the mat sizes are not standard size and would have to be special made. Can mat sizes be substituted? For example, can the 3x8 be exchanged for a 4x8.”
Answer: 3x 8 is what is needed, space constraints in entrance area of the 2 mat locations specified will not allow for a 4x8 mat to fit in the tight space.
Question: “Are services required on holidays?”
Answer: Scheduled service is to be completed the day before or immediately after the holiday (if we go with a Tuesday or Wednesday or Thursday delivery, we would avoid a lot of federal holidays, as historically, most holidays have been on a Monday or Friday).
Question: “If the COTR or designee is not present during delivery and the contractor needs to gain access but cannot, what is the protocol or plan to execute delivery without long delays?”
Answer: The COR (Contracting Officers Representative) will ensure that there is a competent back-up assigned in case of COR absence. There are no “secured areas” that will need to be accessed. Only a few areas are locked, and someone is staffing the door to let people in.
Question: “In our experience with these types of contracts floor mats have been moved by hospital staff on several occasions and some have never been found.”
Answer: We have had good communication with Facilities staff in the past and have had matting services for several years now, most everyone knows to leave the mats in place. The COR specified brown to help eliminate confusion between government owned vs. contracted mats. The facility would be responsible for replacing any lost matting that we have moved. The contractor will be responsible for reporting any missing mats immediately to the COR, same day as service. We have 1 area that floods on a regular basis and those mats get moved and hung to dry and we put them back down when dried.

Request for additional mats must only come from the contracting officer and must only be implemented after bilateral modification to the contract documenting quantity and price change to the contract.
10. All kitchen matting must be exchanged at a minimum of six (6) times per year, exchanges shall be spaced evenly throughout the year. Example: 1st week every two (2) months. Set day and time of schedule for delivery must be agreed upon between COR and Contractor. Damaged kitchen matting must be replaced as needed and contractor must be notified by COR of any damages/deficiencies noted related to kitchen matting throughout the terms of this contract. The color for Kitchen Mats shall be black.
