B.2 STATEMENT OF WORK

Part I: General Information

A. Introduction US Department of Veterans Affairs (VA), Battle Creek VA Medical Center (BCVAMC), VA Wyoming Health Care Center (HCC), VA Lansing Community-Based Outpatient Clinics (CBOC), VA Muskegon CBOC, VA Benton Harbor CBOC, VA Grand Rapids Health Care for Homeless Veterans (HCHV), and VA Grand Rapids Outreach Center require furniture/equipment moving and installation/removal. *All locations will be referred to as VA throughout contract

B. Scope The contractor shall provide all parts, toggles, on-site supervision, uniforms and identification, equipment, tools, transportation, supplies, and materials necessary to perform all tasks related to the transportation of furniture/equipment, installation/removal of government owned new and used freestanding, wall mounted, and modular furniture.

Part II: Work Requirements

A. Technical Requirements

· Contractor shall provide two (2) personnel one (1) supervisor and one (1) laborer one (1) day each week for a total 52 days/year. Contractor shall notify COR of key personnel substitution prior to work day. The intent is for the contractor to provide service one (1) day each week that is mutually agreeable to both the contractor and the VA. In the event no work is needed during a week, the work day shall be reserved to be applied toward a future week.

· Contractor shall provide and supply all equipment, hardware and tools for in-house moving including dollies, flat carts, jacks, panel carts, blanket wraps, vans, and floor protection and boxes as identified by the Contracting Officer Representative (COR). contractor shall supply all necessary equipment for transporting furniture and equipment from one destination to another and the trucks shall have proper lifts that can be used at various warehouse docks and interior strap supports inside the truck to ensure safe transportation of all items.

· Contractor shall have a minimum of three (3) years’ experience in moving and installation as well, as be acquainted with the daily operations of a healthcare setting. Contractor shall work along with the COR on projects and provide detailed account of deviations from original instruction and final inspection of all projects.

· Contractor shall be responsible for keeping all furniture storage neat and organized to include removal of flammable debris. Contractor shall ensure that product is clearly visible and easily inventoried.

· Contractor shall provide on-site instruction to end users of product. Contractor shall be knowledgeable of the various major furniture vendors including Steelcase/Coalesse, Herman Miller, Nemschoff, HON, Carolina, IOA, Unicor, KI, Cabot Wren, Allsteel, Humanscale, Ergotron, Neutral Posture, Ergogenesis, Midmark, Norix, Sauder, Furniture Lab, Inter-Metro, RT London, Kimball, National, Weiland, Transformations, Izzy, Beau furn, Cabot Wren, Knoll, Trendway.

· Contractor personnel shall complete installation/removal, reconfiguration, structural repairs, upholstery repairs and cleaning of furniture and seating in accordance with detailed blueprints, manufacturer certified installation practices to maintain manufacturer warranty, hand sketches or AutoCAD installation drawings provided or as identified by the COR with little to no guidance by the COR. Also, be able to read detailed furniture drawings and pull the necessary parts to install furniture required correctly and per manufacturer certified standards without any assistance from the VA staff.

· They shall possess the ability to problem solve with on the spot conflicts that may arise such as: end user modifications, utility issues, and lack of furniture inventory Also, be able to work with electricians, plumbers, carpenters, flooring installers, warehouse personnel, voice and data trades to coordinate timing and meet installation time lines as identified by the COR.

· Contractor shall have a qualified mover that shall work with COR with limited direction if necessary. This qualification requires the movers to move items with proper equipment with attention to safety, proper unloading and loading of boxes or rent-a-crates. All items to be moved shall be handled with care ensuring the walls, floors, ceilings, light fixtures, handrails, any other structures are not bumped, scratched, or dented. Any damage shall be the responsibility of the Contractor. Contractor shall provide vans when required. Box Trucks shall be a set fixed price for each daily usage and outlined on each invoice submitted.

Conformance Standards All services provided under this contract shall be performed in conformance with the National Fire Protection Agency (NFPA), https://www.nfpa.org/ Occupational Safety and Health Administration (OSHA), https://www.osha.gov/ The Joint Commission (TJC), https://www.jointcommission.org/ Health Insurance Portability & Accountability Act (HIPAA), https://www.hhs.gov/hipaa/for-professionals/privacy/laws-regulations/index.html Americans with Disabilities Act (ADA), https://www.ada.gov/ Battle Creek VAMC Policies, and Original Equipment Manufacturer standards and specifications to maintain manufacturer product warranties. https://www.va.gov/orpm/ All contractor and their employees shall be required to complete the necessary privacy training required by the VA.

B. Quality Control

· The contractor shall be responsible for any damages that result from improper installation or handling of government owned furniture/equipment. The contractor shall be responsible for any damages to new and old furniture replacement shall be at the contractor’s expense. Incorrect installation shall be re-worked at the expense of the contractor. (contractor is not responsible for install damage due to instruction and/or specifications given incorrectly by the government).

Deliverable’s/ Inventory Management:

· Contractor shall provide checklist to identify deficiencies in the quality of services performed. Checklist shall be signed by both the contractor Supervisor and the COR daily.

· Contractor shall be required to keep an up-to-date inventory of all spare furniture stored onsite at the BCVAMC in Building 39, Room B020 and/or warehouse. Inventory shall be updated whenever an item is removed or added to storage.

· The COR shall provide the contractor with a checklist identifying work to be performed and inspected daily. All work shall be completed by contractor and inspected by contractor Supervisor and the COR and signed by both for the COR’s file. Any deficiencies (damaged/errors,) that are discovered in conjunction with the Inspections shall be completed within three (3) days of the original work performed at the contractor expense.

Part III: Supporting Information

A. Security

· The Certification and Accreditation (C&A) requirements apply and a Security Accreditation Package is not required.

· Contractor shall ensure all personnel is registered with the BCVAMC Police, and to receive an identification badge. Badge shall to be worn in plain sight whenever on VA property. Prior to commencing work the contractor personnel shall report to BCVAMC in Building 24, Room 219 or the appropriate office. Upon arrival and prior to departure, to sign 'in' and 'out' turn-in contractor badges to the COR response log.

· Contractor shall ensure all personnel be in uniform identifying the contractor; while working on government property. Contractor shall ensure all personnel do not enter the project site without appropriate identification.

· Contractor shall not share any personnel, patient or government information acquired while working at a government facility.

· COR shall indicate to contractor where to put designed furniture. Contractor shall not resell formerly government owned furniture for their own personal gain.

B. Safety Compliance

· The contractor shall follow the safety precautions of the Safety Personnel Designee that may determine to be reasonably necessary to protect the lives and health of occupants of the building. Contractor shall abide by all warning signs at construction sited, infection control warnings or any other event that comes up during the term of this contract.

· The contractor shall immediately correct the conditions to which attention has been identified. Such notice, when served on the contractor or his representative at the site of the work, shall be deemed sufficient for the purpose aforesaid. If the contractor fails or refuses to comply promptly, the Contracting Officer may issue an order halting all or any part of the work, and contractor shall be held in default.

· Contractor personnel shall park only in designated parking areas.

· Smoking is not permitted within or around the VA Healthcare System facilities except in designated areas.

C. Travel The contractor shall provide travel and transportation necessary to perform duties.

E. Period of Performance The period of performance is January 28, 2019 through January 27, 2020 with two (2) one (1) year option periods to be exercised at the discretion of the Government.

Duty Hours.

Monday through Friday from 8:00 a.m. to 4:30 p.m., excluding federal holidays, or as otherwise arranged with the COR.

The ten holidays observed by the Federal Government are New Year’s Day, Martin Luther King Day, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. Also, any other day declared by the President of the United States to be a national holiday.

F. Out of Scope Work.

Contractor shall not perform outside of normal working hours of coverage. Approval is required before all out of scope work hours are performed. The contractor shall immediately notify the COR if the VA desires the contractor to perform out of scope work. A separate purchase order shall be issued to the contractor for the work.
