Replace 3A Flooring 541‐18‐507

Statement of Items:

Item 1: Base Item Project 541‐18‐507, Replace Surgery Flooring.

This is a single‐phase project located at the third floor of the Louis Stokes Cleveland VA Medical Center that will remove approximately 3,600 square feet of the existing luxury vinyl tile plank flooring, carpet flooring and associated wall base in the East Wing (3A), north corridor and some adjacent rooms. The project will also remove existing integral sheet vinyl base and associated 8” wide portion of sheet flooring adjacent to the base for the entire room perimeter of select patient rooms. The flooring in the East Wing (3A) is failing and in need of replacement. The project will prepare the existing flooring substrate to receive new sheet vinyl flooring with coved wall base and repair failing heat welded seams in select rooms. This project will provide new transition strip material between new and existing flooring materials. The project area is currently under construction to convert the area from the SICU function to accommodate the Hemo‐Dialysis and Respiratory Therapy groups and the new Flooring work will be a component of this larger project.

The Contractor shall provide all labor, equipment and materials necessary to complete the work as shown on the drawings and specifications. Work includes site investigation, demolition, flooring preparation/ installation and general construction.
